EE smacked EE's finger with a hammer

Employee was breaking a sideline this process involves striking the wedges swinging the hammer with a side-to-side motion. While doing this the employee missed the wedge and struck the wall causing the hammer to deflect and rotated the employee rapidly causing a strain to the lower lumbar.

Miner was struck in the arm and hard hat by a jack handle while setting track for saw. Miner was transported to clinic to be evaluated out of precaution. No significant injuries reported by clinic.

Miner was adding a section of drill steel using a wrench to tighten the connection. The miner's left thumb was caught between the drill boom and the wrench causing a small avulsion to the left thumb.

EE bent over to fit the chain underneath a piece of granite that was on the conveyor and felt a sharp pain in EE's lower back.

EE WAS LOOSENING A 5' LENGTH OF DRILL STEEL TO REMOVE FROM EE'S D30 DRILL WHEN THE WRENCH CAME LOOSE AND HIT EE'S RIGHT PINKY FINGER

Attempting to free a piece of Plug gear and EE hit finger with a sledge hammer and cut finger on the stone.

Swinging a 12lb sledge hammer and employee twisted wrong and injured back

While doing secondary breakage operations EE struck a wedge and EE struck off and a piece sheared off lodging in EE's left forearm

While walking by a stone being split the individual swinging the sledge hammer struck off and debris hit the injured person in the chin area causing a laceration injury

Using a bullset on a piece of curb and when the set was hit with the hammer a piece of either steel or stone struck EE's face causing and imbedded foreign object

Miner was pulling an airbag (~25lbs) on a rope behind self across ledge. Pulled muscle in back.

EE was moving a wire saw with a come along winch and strained back.

Using an iron bar to move a piece of curb and pinched finger between two pieces of curb.

Swinging hammer missed wedge hit stone bounced back hitting EE in left eyebrow area causing laceration requiring sutures

Employee was breaking rock with an eight pound sledge hammer and missed wedge causing hammer to come back and strike them in the face, forehead, causing a laceration.

Swinging a sledge hammer and heard a pop in EE's lower back followed by pain

EE was attempting to use a bar to stop a slab from falling on EE's foot. The slab knocked the bar out of EE's hand and the bar came down across right foot, on top of the steel toe area of boot. EE has bruising on right big toe and second toe.

Razor knife cut wrist while trying to swap out curtains on Dual saw

was splitting a stone with a wedge and EE struck off with the hammer and a piece of the wedge broke off and hit EE's left hand causing a laceration

EE was cranking the bench vice when EE felt a sharp pain in the center of EE's back.

Using an iron bar to move a small horizontal DTH drill when the drill started to tip over the miner reached out to grab the drill to try to stop it from rolling over and dislocated shoulder

Employee was using a safety knife to strip the plastic off the end of a wire and cut right leg just above the knee.

Employee was cutting a zip tie off of a rail using a box cutter. His left hand was holding the rail and right hand using the box cutter. His left hand slipped as he was cutting and was cut across left index finger.

Miner was utilizing a folding knife in the course of his job. Knife unexpectedly folded and slipped, sticking him in the leg.

BROKE LEFT THUMB WHEN EMPLOYEE ACCIDENTALLY STRUCK IT WITH A HAMMER.

EMPLOYEE WORKS IN MAINTENANCE; WAS HITTING A WEDGE WITH A HAMMER, A SMALL PIECE OF METAL SPLINTERED OFF AND STRUCK HIM IN THE NECK.

Working on Fantini saw tracks, replacing leveling pads. One pad very stuck, left shoulder had major pain. Iced, no treatment by doctor. Employee reported to me 5/27 that he had gone in to doctor for pain and given a cortisone shot. Awaiting decision regarding surgery.

Miner was hitting a wheel with a 3 lb hammer to adjust the angle. Missed and hit is own right hand. Bruised right hand. Was x-rayed, no broken bones.
