Employee was using a knife to open packaging when EE nicked right thumb. The laceration required two stitches and EE was released to full duty.

The two employees were making adjustments to the nuts at the top of the separator. They were using a chain-fall to keep tension on the separator. A link in the chain-fall broke hitting two employees.

Tightening a nut and bolt with a spud wrench and the wrench slipped and hit EE in the nose. Doctors are saying that it is suspected to be broken. The practitioner report says that this person is on 'modified duty'. Modified to include limited lifting, minimize bending and no heavy pulling or pushing. This person has been off of work since the injury happened.

Employee was using a bar to dislodge another bar at a slag feeder. When attempting to dislodge the bar, he pinched his finger against the bar that was stuck suffering a laceration to his right index finger.

Employee was shoveling up a spill with 3 other workers when another worker's shovel struck him on the left wrist. He suffered a contusion to the left wrist.

Employee was using a come-along to move over a screw auger trough with a wire rope sling wrapped around it. Whilst pulling the lever the wire sling broke and the employee fell backwards and hit his upper back on the edge of a steel beam. This contact caused a contusion on his back. The doctor's note stated "ee was seen today in the ER. He should not work tomorrow".

Employee was poking out a coal choke in the chute of a coal bunker, he pulled the bar he was using to remove the choke towards him and felt a sharp pain in his lower right back. Employee was transported to the hospital, he has a muscle strain and was placed on restricted duty.

EE was working on a rotarty diverter when he hit his left thumb with a hammer, smashing his finger

EE was using a pry bar to move a switch gear. When applying force, ee pulled a muscle in his back

EE was attemting to free up some material in the Gypsum material by using a pipe to stick into the access hole. While ramming the pipe in the hole, EE's hand struck the frame of the weigh feeder which resulted in a laceration to the right thumb.

Employee was attempting to unplug a chute going to a bucket elevator with a piece of flatbar. The flatbar contacted a moving elevator bucket, causing the piece of flatbar to jar, causing a laceration to his rt. hand.

EE was trying to clear a plugged chute below the 1st stage cyclone when hot dust flowed out of the porthole. EE received 1st and second degree burns to both hands, wrists, and the back of his neck.

Employee was pulling nail from a board, hammer slipped and hit right knee.

Employee was using a pry bar to free up a stuck diverter gate. The ram to the gate had been disconnected. Apparently, the gate became free and slammed against the pry bar pulling his forearm into the opening and crushed his forarm between the edge of the bin and gate. He suffered a broken radius bone above his RT wrist.

Employee was operating a winch when his back become sore.

EE was securing a rail car with a wheel lock type pry bar. While positioning the pry bar, the rail car rolled slightly causing the pry bar to unexpectedly move in an upward direction, striking the EE under the right mandible, causing a laceration. EE recieved stiches at the local ER and returned to work.

EE was using a prybar to close a door in the preheater tower, when the bar slipped and he fell spraining his knee.

Employee was swinging a sledgehammer to clear a plugged chute. They swung the sledgehammer and missed the chute causing left thumb to be smashed in between the hammer's handle and the steel structure. This caused a laceration but did not require any stiches. The employee was prescribed antibiotics and a pain reliever.

Employee was attempting to unplug a rotary feeder with a 48" pipe wrench. The employee did not feel any discomfort at work but later started to feel pain in his neck/shoulder area.

Using bar to move slide on stone belt, bar slipped and employee experienced pain in right shoulder.

EMPLOYEE WAS STRIPPING CABLE WITH POCKET KNIFE. HE WAS PULLING KNIFE TOWARDS HIMSELF. THE KNIFE CAUGHT ON CABLE AND WHEN HE PULLED HARDER IT FREED THE KNIFE AND CUT HIS HAND.

A head pulley was plugged with material and an employee was getting ready to clean the material out. The affected employee reached in from the opposite side to test the hardness of the material when the other employee rammed a bar into the material stricking the affected employee's finger.

Employee reached into his tool bucket and cut his middle finger on his right hand . He had left his knife open in the tool bucket .

Employee was tightening bolts with a socket wrench. The wrench slipped off of the bolt and struck the employee in the mouth.

HE WAS CUTTING A LID OFF A PLASTIC 5 GALLON BUCKET. HIS HAND SLIPPED. THE LEFT PALM WAS CUT WITH THE UTILITY KNIFE.

EE WAS TAKING A BEARING RACE OFF, AND THE RACE BROKE INTO PIECES. A PIECE OF RACE FLEW AND HIT HIM IN THE CHEEK. THE PIECE WAS LODGED IN HIS LEFT CHEEK.

EE WAS DRIVING A WEDGE INTO DOG ON DUCT WORK, APPROX. 8 INCHES FROM TOP. WHILE MAKING THE SWING, THE HAMMER STRUCK THE FLANGE DEFLECTING THE SWING AND CAUSED THE HAMMER TO HIT EE ON THE HAND.

EMPLOYEE FELT PAIN IN RIGHT SHOULDER WHILE PULLING ON A WRENCH.

EMPLOYEE WAS STANDING ON A PILE OF MARL WHILE WORKING ON STAMLER. HIS FOOT SLIPPED AS HE WAS PULLING ON A PIPE WRENCH AND HE FELT A PAIN BETWEEN HIS SHOULDER BLADES. HE CONTINUED TO WORK BUT P AIN CAME BACK LATER THAT DAY AND HE WENT TO THE DOCTOR.

WAS HITTING ON DISCHARGE DUST COLLECTOR PIPE USING A SLEDGEHAMMER. THIS WAS NOT REPORTED DUE TO MEDICAL TREATMENT DID NOT OCCUR UNTIL 8/28/03.

REMOVING SCRAPER AND PLOWS WITH SLUGGING WRENCH AND HAMMER GLANCED OFF AND HIT THUMB.

EE WAS TRYING TO HIT ADJUSTING WEDGE: HE WAS CHOKING HAMMER CLOSE TO THE HEAD & HIS FINGER GOT BETWEEN WEDGE & HAMMER CAUSING HIS FINGER TO BE CAUGHT & SMASHED.

EE WAS USING A LONG PIPE TO BEND BOLTS BACK & FORTH, BREAKING THEM OFF AT FLOOR LEVEL TO REMOVE A TRIP HAZARD CREATED BY THE STUDS PROTRUDING OUT OF THE CONCRETE PAD. DURING ONE SUCH EFFORT TH E PIPE SLIPPED ALLOWING HIS RT HAND TO CONTACT ANEARBY STRUCTURE MASHING THE LITTLE FINGER OF HIS RT HAND CAUSING A LACERATION REQUIRING SUTURES.

EMPLOYEE WAS CUTTING BELTING FOR SKIRTING ON C-3.

EE WAS TIGHTENING BOLT ON COAL MILL ROLL WHEN THE WRENCH SLIPPED AND HIS ELBOW STRUCK METAL GRATE.

EE WAS USING A WRECH TO TIGHTEN BOLT WHEN THE WRENCH SLIPPED OFF BOLT AND HIS FINGER GOT CUT ON A METAL FRAME.

HOLDING WATER LINE IN LEFT HAND. TRIED TO CUT LINE WITH KNIFE IN RIGHT HAND, KNIFE SLIPPED AND CUT LEFT FOREARM.

EMPLOYEE WAS WRENCHING BOLTS LOOSE IN THE NEW FINISH MILL WHEN HE FELT PAIN IN HIS LEFT BICEP AREA.

EE WAS USING A SLEDGE HAMMER AND PULLED A MUSCLE IN HIS SIDE.
