Tunnel feeder had concrete chunks get stuck on crossbeams and needed to be removed before any more material is fed into the feeder. Employee took a 12lbs sledgehammer and tried to break material, this did not work. After this task the Employee felt pain in their right hand. Employee did not report the pain until 03/27/2026 at 630am after medical treatment

EE later states that on 9/1 EE was attempting to obtain leverage with EE's shoulder on a wrench handle, and injured EE's shoulder trying to break a screen bolt loose. Last date of work was 9/11, on 9/17 was seen by medical provider and released to work with restrictions. No-call, no-showed and terminated 9/21.
MINER WAS USING A PRY BAR TO LIFT UP THE LATCH OF A PINTLE HITCH. THE PRY BAR LOST PURCHASE ON THE LATCH WHILE THE MINER WAS PUSHING DOWN ON THE END OF THE BAR RESULTING IN THE LOSS OF BALANCE. MINER FELL FORWARD AND STRUCK THEIR FACE ON THE TOP SUPPORT BAR OF A MUDFLAP RESULTING IN THE DISLODGEMENT OF A DENTAL CROWN. THE UNDERLYING TOOTH WAS CHIPPED AND STILL AFFIXED TO THE CROWN.
WHILE PERFORMING MAINTENANCE ACTIVITIES ON AN HP 400 CONE CRUSHER, TEAM MEMBER WAS REMOVING PIN FROM HYDRAULIC CYLINDER ON AN HP 400. WHEN PIN WAS REMOVED, THE HYDRAULIC CYLINDER CAME DOWN PINCHING LEFT PINKY FINGER BETWEEN CYLINDER AND OBJECT USED TO REMOVE PIN. FINGER REQUIRED STITCHES.
Employee cut a finger using a utility knife performing maintenance.
Miner reported to employer on 09/29/2016 that activities on 05/24/2016 related to cleaning out a drive-over hopper with a long spade bar led to shoulder pain.
Miner was using a utility knife to cut the tip off tube of silicone and sliced thumb.
Employee was cutting a belt with a locking razor knife. Knife slipped and struck employee in the leg.
Employee was using a 36" pry bar to bring lower roller on D9 into position to be bolted. Felt burning sensation in left shoulder during process.
Miner was dragging a razor blade knife cutting a belt, knife slipped and went into left leg about 5 inches above the knee and and cut leg. 10 stiches were incurred, back to work, no lost time.

A SOCKET WAS TOSSED UP TO EE AND ACCIDENTLY STRUCK HIS GLASSES, CAUSE THEM TO CUT HIS FACE.
EMPLOYEE WAS USING A 5 FOOT LONG PRY BAR TRYING TO BEND A SKIRT BOARD ON A CONVEYOR # CC2 WHEN THE PRY BAR SLIPPED OFF AND HIS THUMB WAS SMASHED BETWEEN THE HANDRAIL AND THE BAR. THOUGHT ONLY BRUISED, BUT CONTINUED TO BE PAINFUL, SAW DR. HESAID IT WAS BROKEN.
