Individual attempted to lift a 5-6 ft. long chunk of I-beam (weighing approximately 150 lbs.) and toss it over the handrail of an elevated screen deck to the ground below. The I-beam shifted and pinched their right thumb between the beam and handrail. Individual complained of extreme pain and was transported to Urgent Care where an X-Ray discovered a broken bone in the right thumb.

On 9/11 individual reported injury of unknown origin. EE later states that on 9/1 EE was attempting to obtain leverage with EE's shoulder on a wrench handle, and injured EE's shoulder trying to break a screen bolt loose. Last date of work was 9/11, on 9/17 was seen by medical provider and released to work with restrictions. No-call, no-showed and terminated 9/21.

Had wrench resting on left shoulder for leverage. Pushed upward with arm and shoulder, felt a pop in shoulder then pain.

Employee was driving an end spill and was backing up the equipment when they fell a pop in the shoulder.

Installation of guard. When guard was put into place employee's hand got caught in a pinch point.

Employee positioned their upper body between the belts to knock a rock loose from tail pulley and belt using a hammer. When rock was freed stored up energy caused the belt to move in an unexpected forward motion catching the employee's hands between the belt and the pulley. Employee locked out the breakers but conveyor was not blocked from motion.

Work was being performed on screens. Employee 2 was on an upper level shoveling off material to the ground and employee 1 walked under the area where employee 2 was shoveling material off to. Employee 1 had a small rock make contact with rib cage. No medical was requested at time.

Employee was coming off of scalper when foot came down onto an incline and twisted their ankle.

Employee was installing new boards to lowboy trailer. They bent over to pull out a new board, when they felt a pop in their back, no pain at the time. Employee did not request help to move the board when other employees were in the area.

Employee was operating a loader when a Haul truck backed into the loader jostling the driver in the seat and hurt lower back.

Employee held on to the head of a worn plow bolt while another employee tried to take the nut off. Bolt spun in employees fingers and cut them.

Employee was helping to install a V-Belt on motor sheave, and got ring finger caught between the two crushing the finger.

Using a wrench and hammer to remove a scraper from a reject conveyor

Employee smashed finger against concrete wall behind tail pulley while adjusting the belt using a ratchet. Was on modified duty for two days and then released to full duty.

Employee cutting belt with knife and cut right hand pointer finger.

Employee was putting tools away in vehicle after repair of water line when loader backed up and pressed employee against vehicle. Employee sought medical attention at approximately 1 PM and was diagnosed with muscle strain. Employee has been working modified duty since the incident.

Employee did not report the incident to his supervisor until 12/16/15 which was diagnosed by a doctor to be tendonitis. Employee incident report stated that he was dismantling the suspension on a 1 ton service truck on 12/8/15. When he awoke on 12/9/15 he was experiencing pain in his left wrist. He worked for one week without telling anyone.

At approximately 9:20 PM an employee was removing and replacing a section on 9505 and while taking a better stance he slipped and fell on his RT elbow causing a contusion.

Lifting and throwing rock out of haul road. Employee developed lower back pain.

Employee was using a hand grinder with a cutoff wheel to remove a seized up bolt. The grinder grabbed and threw itself into his leg, causing a cut to left knee.

Employee was removing an I-beam when one end of it unexpectedly dropped, raising the other end, pinching his finger between the beam and a chute causing his right middle finger to be cut.

Walking down stairway, slipped and caught himself causing a strain to the SI joint in his back.

Pulling on screen gates when he felt a burning sensation in his abdominal muscles.

Employee was lifting a piece of screen cloth to the screen deck when he bumped a piece of screen cloth that was staged to be lifted causing it to shift and strike his left hand cutting his fingers. He received stitches to his middle and ring fingers.

Employee felt pain and numbness on left side at approximately 9:00 a.m.

Employee was installing a gear box. The gear box slid off the shaft and pinched the employees finger causing a cut on the finger.

Employee was replacing a sheave on a motor overhead. The taper lock came loose and the sheave struck the employee on the back of his hard hat causing a slight strain to the employees neck muscles.

The employee was operating an end dump and was preparing to travel down a haul road to enter the pit. As he began down the haul road he reported the brakes had malfunctioned. There was a front end loader at the bottom of the haul road. The end dump hit the front side of the loader. There was moderate damage to the haul truck. The driver sustained bruised ribs.

The employee was operating a front end loader to clear rocks from a haul road. It was reported that the brakes on an end dump malfunctioned as it traveled down the haul causing the end dump to hit the front side of the loader. The loader sustained moderate damage. The loader operator has soreness in his back.

Doing cleanup shoveling under a conveyor, injured back.

The employee was disconnecting the road widener machine from the front of the loader boom. The left coupling pin got stuck and he was using a hammer and a large bolt to knock the pin loose. As he struck the bolt, the pin freed up, and the finger he was holding the bolt with was pinched between the couplers cutting his finger. He received stitches from this incident.

The outside contractor was onsite working on the rock drill. A piece of drill steel started to fall out of the carousel. The contractor grabbed it to push it back in place as the operator engaged the grabbers. The carousel turned pinching the contractors little finger.

Employee was replacing a conveyor belt. The new belt was being pulled on by using ropes. While pulling the new conveyor on, one of the ropes broke, striking the employee in the face, causing lacerations to the right side of his face and chin.

Employee was in endspill, when another endspill operator in a 2nd endspill, backed into his endspill.

Employee was operating the dozer (D8N cat), pushing out a shot that could not be dug out, and his neck started to hurt.

Box cutter blade was stuck while cutting belting. When EE pulled the blade free, his hand slipped and cut the knuckle of left thumb with blade.

INCIDENT NOT REPORTED UNTIL OCTOBER 9, 2006. Employee claims original injury occurred 06/02/06. He did not inform the company until 10/09/06. EE claimed, "Nothing out of the ordinary" the he could remember to cause the injry. EE indicated he has injured this body part (lower back strain) before.

WORKER WAS CARRYING A STEEL GRATE. GRATE SLIPPED FROM HIS HAND AND STRUCK HIS LEG CAUSING LEG TO BE CUT & SCRAPED.

WORKER WAS UNLOADING 128TH ANVILS AND SMASHED/CUT FINGER ON LEFT HAND WHEN 2 ANVILS COLLIDED.

WORKER WAS MAKING REPAIRS TO CONVEYOR WHEN HE FELL TO THE GROUND LANDING ON RIGHT SIDE. THIS RESULTED IN BRUISING OF NERVE IN LOWER BACK

WORKER WAS DESENDING LADDER. LADDER TIPPED OVER AND WORKER FELL TO GROUND LANDING ON LEFT FOOT RESULTING IN BROKEN HEEL.

WORKER WAS USING GRINDER & WELDER WHEN SMALL PIECE OF METAL BECAME IMBEDDED IN RIGHT EYE.

EMPLOYEE WAS CUTTING WET CONVEYOR BELTING. SAID THE CUTTING BECAME EASIER TOWARDS THE END OF THE CUT SO THE KNIFE CUT THROUGH EASIER THAN EXPECTED AND HE STABBED THE KNIFE IN HIS LEG.

EE HAD BEEN CHANGING SCREENS ON A CONVEYOR. WHEN HE FINISHED HE WALKED OUTSIDE & WAS HIT BY A GUST OF WIND THAT BLEW A SMALL PIECE OF METAL INTO HIS EYE.

EMPLOYEE DROPPED HEAVY OBJECT ON LITTLE FINGER OF LEFT HAND AND CUT IT OPEN.

EE BENDED TO GO UNDERNEATH CONVEYOR AND POPPED HIS BACK OUT OF PLACE.

EE WAS REMOVING OLD WEAR PARTS FROM INSIDE THE CRUSHER. HE WAS HITTING ON CAST STILL INNER WHEN A PIECE OF COASTING HIT HIM IN THE EYE.

EE WAS WELDING LOW TO THE GROUND. GRINDINGS AND SPARKS WERE COMING STRAIGHT OUT TO LOCATION EE HAD TO WORK IN. EE WAS WEARING PROPER EYE PROTECTION.

REPAIRING ROCK HOPPER-GRINDING METAL-HAND HELD GRINDER CONTACTED LEFT WRIST. WORKING IN VERY TIGHT QUARTERS. THE URGENT CARE DR PUT IN 7 STITCHES THE NEXT DAY AND ORDERED MODIFIED DUTY FOR 1 D AY. HE RETURNED TO WORK NEXT SHEDULED SHIFT.

WHILE OPENING INSPECTION DOOR ON UD OF VERTICAL SHAFT IMPACT CRUSHER, HE LOST HIS GRIP ON THE DOOR AND IT FELL CLOSED, STRIKING RIGHT BIG TOE. EMPLOYEE DID NOT GO TO DR FOR 2 WEEKS.

EE WAS SHOVELING MATERIAL SPILLAGE FROM UNDER A CONVEYOR AND FELT SHDDEN PAIN IN RIGHT SHOULDER.

EE WAS IN SHOP WELDING BALL. HE WAS WALKING WHEN HE TRIPPED OVER WELDING LEAD AND THIGH STRUCK RIB CAGE.

EMPLOYEE WAS GOING TO BURN SOME PAPER IN A BURN BARREL. EMPLOYEE SPRAYED WD-40 ON THE PAPER. WHEN EMPLOYEE LEANED DOWN TO LIGHT PAPER WD-40 IGNITED BURNING HIS HAND.

EMP WAS MOVING A SMALL ELECTRICAL STARTED WHEN SHE LOST HER GRIP AND DROPPED IT ON HER FOOT.

EE WAS LIFTING ANFO BAGS OUT OF DELIVERY VAN

EE WAS SHOVELING DIRT FROM UNDERNEATH CONVEYOR WHEN HE FELT A SHARP PAIN ACROSS HIS LOWER BACK.

EE WAS CLIMBING DOWN OFF CONVEYOR AND LOST HIS FOOTING. HE FELL TO THE GROUND, HITTING AND INJURING HIS HEAD ON ANGLE IRON LEG.

EMPLOYEE WAS WORKING AT SECONDARY CRUSHING PLANT INSIDE FINISH SCREEN. HE WAS INSTALLING A CROWN BAR RUBBER ON SCREEN DECK. THE RUBBER WAS TIGHT FITTING SO HE WAS USING A HAMMER TO POUND IT DO WN AND HIT HIS THUMB.

EE GOT UP ON CONVEYOR. TO CHECK BELT SCALE. HE JUMPED OFF AND INJURED HIS ANKLE.

EMP WAS CLIMBING ON CAT INSTALLING RADIATOR. SLIPPED ON WET BLADE AND BUMPED KNEE ON TRACK.

EE WAS BANDING CRUSHER PARTS TO BE SHIPPED AND CUT HIS HAND W/BANDING MATERIAL.

EE WAS SHOVELING MATERIAL FROM UNDER A CONVEYOR BELT WHEN HIS SHOVEL CONTACTED THE BELT AND PULLED HIS ARM UNDER A RETURN ROLLER.

EMPLOYEE WAS INSTALLING A FLIP GATE. HE WAS TRYING TO PUT THE CONNECTING PIN IN THE LINKAGE WHEN THE GATE MOVED SMASHING HIS FINGER BETWEEN THE CYLINDER AND THE LINKAGE ARM.

EE WAS WALKING DOWNHILL & STEPPED ON A ROCK, TWISTING KNEE. EE HAS STRAINED CARTILAGE IN HIS KNEE. EE REINJURED PREVIOUS INJURY.

EMPLOYEE WAS UNTANGLING A CABLE ON THE CRANE WHEN HE SLIPPED AND FELL OFF THE CRANE.

EE WAS OPERATING AIRTRACK. EE LIFTED A PIECE OF STEEL TO PUT IT IN THE RACK WHEN HE LOST STRENGTH IN ARM AND FELT SHARP PAIN.

EMPLOYEE WAS PULLING BAR OFF OF THE CONVEYOR WHEN THE BELT COVE SLID DOWN THE BAR SMASHING 3,4,5DIGITS ON RIGHT HAND BETWEEN THE BELT COVE AND FLAT STOCK ON END OF THE BAR.

EMPLOYEE WAS WORKING AROUND SCREEN TOWER, WHEN STRONG WINDS BLEW A PIECE OF STEEL INTO LEFT EYE.

EE WAS MOUNTING A GEARBOX AND MOTOR ON A CONVEYOR. HE ATTEMPTED TO PUT THE BOLT IN THE TORQUE ARM. THE GEARBOX AND MOTOR CAME DOWN SMASHING HIS HAND.

EMPLOYEE WAS GRINDING & WELDING IN THE SHOP AREA IT WAS A HOT DAY EMPLOYEE REMOVED HIS SAFETY GLASSES TO WIPE SWEAT FROM HIS FACE SOMETHING GOT IN HIS EYE CAUSING IRRATIATION TO EYE

OPERATOR WAS TRYING TO BACK MACHINE UP TO LEVEL GROUND WHEN THE ENGING STALLED THE LOADER WAS LOW ON FUEL & POSSIBLE INTRODUCTED AIR INTO THE FUEL SYSTEM LOADER THEN ROLLED AHEAD OVER THE FACE OF THE QUARRY LANDING APPROXIMATELY 40OFT BELOWON THE NEXT BENCH OPERATOR WAS THEN THROWN FROM THE LOADER THE LOADER STOPPED APPROXIMATELY 30 POINT OF IMPACT & THE MACHINE

EMPLOYEE WAS GRINDING WITH A 9" INDUSTRIAL HAND GRINDER. GRINDER WHEEL BOUND UP IN A GROOVE & DICKED BACK STRIKING EMPLOYEE IN LOWER RIGHT CHEEK.

THE LOADER WAS WET AND MUDDY. EMPLOYEE FEEL OFF LOADER WHILE CLIMBING OFF AND LANDED ON TAILBONE.

EE WAS ROLLING 1 FULL BARREL AND 1/3 OF A BARREL OF USED OIL ONTO THE FORKS OF THE HYSTER FORKLIFT.
