Team member got dirt and a small cut on their left eyelid on 6/24/2024. By 6/26/2024, the eye was swollen and became painful. Team member sought medical care at a local eyecare center.
MINER WAS USING A PRY BAR TO LIFT UP THE LATCH OF A PINTLE HITCH. THE PRY BAR LOST PURCHASE ON THE LATCH WHILE THE MINER WAS PUSHING DOWN ON THE END OF THE BAR RESULTING IN THE LOSS OF BALANCE. MINER FELL FORWARD AND STRUCK THEIR FACE ON THE TOP SUPPORT BAR OF A MUDFLAP RESULTING IN THE DISLODGEMENT OF A DENTAL CROWN. THE UNDERLYING TOOTH WAS CHIPPED AND STILL AFFIXED TO THE CROWN.
LIFTING 8" PIECE OF STEEL CHANNEL TO REPLACE A LEG/SUPPORT BEING INSTALLED ON AN OVERLAND CONVEYOR. CARRIED ITEM APPROX 30' AND SET IT DOWN. MINER EXPERIENCED RIGHT TESTICULAR PAIN.
Team members were picking up a sand screw equipment off a trailer with a crane. One of the rear rigging legs was hung up on a bolt/stud. Team member went to correct the rigging. As EE's hand got near the chain it released tension, taking EE's hand with it towards the sand screw frame rail, pinning it between the chain and the frame. Tip of right index finger was fractured.
WHILE CLIMBING ONTO LOADER, EMPLOYEE CAUGHT RIGHT FRONT TOE OF BOOT ON DECK CAUSING EE TO FALL FORWARD AND JAM RIGHT MIDDLE FINGER INTO CORNER OF RADIATOR GUARD.
WHILE SEPARATING ELECTRICAL CONNECTION IN ENGINE COMPARTMENT, CONNECTOR CAME LOOSE, CAUSING EMPLOYEE'S RIGHT HAND TO STRIKE THE FAN ON THE ALTERNATOR, CUTTING A KNUCKLE.
Miner strained back and neck while operating skid steer.
Miner reported to employer on 09/29/2016 that activities on 05/24/2016 related to cleaning out a drive-over hopper with a long spade bar led to shoulder pain.
The worker has a pinched nerve in the neck area. Unknown how it happened exactly. Possible cause, pulling on large wrench and it slipped with a fall to the ground from ground level.
WHILE REPAIRING A CONVEYOR HE LOOKED UP AND GOT ROCK DUST IN HIS EYE.
WHILE STEPPING OFF OF A CONVEYOR FRAME EE TWISTED AND STRAINED HIS LOWER BACK.
EMPLOYEE WAS GETTING OFF A FLATBED TRAILER WHEN HE HIT HIS KNEE ON THE EDGE OF THE DECK.
