Employee was roof bolting in #2 Entry when a rock dislodged from the mine roof while drilling, struck EE's hand, and caught EE's left ring finger between the rock and drill pot. Employee was evaluated at Boone Memorial, diagnosed with a fracture, and returned to work 6/4/2026 with no restrictions. No Days Lost

Employee stated as EE was bolting in 3 left crosscut a rock fell from the top hitting EE in the back.

Employee stated EE noticed some push that needed pulled up the hill from the refuse, EE stated EE got the 325-cat excavator to remove the brush and has EE was doing so EE pulled a limb into the front glass of the excavator hitting employee on the right leg.

EE was in the process of removing a J-Bolt from a bottom hanger using a 1/2-inch impact to remove the loop nut. J-Bolt swung around and hit EE's finger.

Employee was bolting in the #4 Left Entry when a piece of rock fell from the mine roof striking EE on the right hand. Employee was diagnosed with a laceration of the right 5th distal finger (pinky) and received 6 sutures to close the wound. Employee was given release and return to work on 8/26/2025 performing EE's normal work duties.

Employee was installing a roof bolt as EE pushed the bolt up, EE used fast feed, and the bolt bent causing the bolt wrench EE was using to brake hitting EE in the mouth chipping one of EE's teeth.

After notching a tree to be cut, EE started boring into the tree and the saw kicked back and ran through EE's chaps and into EE's leg.

Witness to the accident stated that when shutting the miner off to hook up a rope on the miner, the witness heard the injured employee call out for help. The injured employee was caught between the boom of the miner and the coal rib where the employee sustained a fractured L1 vertebra.

Employee was assisting a roof bolter move from Entry #6 to Entry #5 when the bolter slid into a hole in the mine floor causing its rear to drop onto the employee's left foot, catching it between the bolter and mine floor. Employee was transported to a hospital, where employee was diagnosed with two fractures and two dislocated toes in left foot. This is a Lost Workdays Incident.

Employee stated EE lifted up EE's welding hood and slid EE's safety glasses down EE's nose towards the tip. Employee stated EE wiped sweat off EE's forehead with EE's welding glove. Employee stated EE started to work again when EE's right eye started to have discomfort. Employee had been welding and grinding on a dozer blade.

Employee was injured when EE's right thumb was caught between two roof bolts while installing monkey faces. 6 sutures were applied to a laceration and EE was diagnosed with a fractured right thumb.

EE was performing routine maintenance. EE was greasing the heavy media cyclone. EE picked up the pneumatic grease gun with right hand on the handle and left on the side of the pump. The pump cycled one last time catching EE's left hand index finger

Roof Bolter operator was injured when a glove was caught between the drill steel and the chuck of the drill pot while drilling. EE was examined by a physician and diagnosed with a fractured right thumb.

EE was operating a bolt machine when the canopy unexpectedly dropped 2-3 inches and hit their head.

Operator was moving forward in the dozer and dropped the blade causing them to come to a sudden stop that resulted in hitting their head against the window latch.

Continuous Miner operator had just began to load in the face of #9 when a rock fell off the continuous miner striking the EE in the right leg resulting in fractures to the right fibula. Lost workdays were a result of this injury.

Employee was attempting to put a Brookville mantrip that had derailed back on the track. The jack turned over causing the mantrip to land on a crib block. The crib flipped up hitting employee in the face.

while installing a roof bolt, a piece of drill steel came out of the tray and came in contact with the bolter arm as they were pushing the bolt to the top. the drill steel came out striking the employee above the right eye resulting in three stitches.

david was working on a broken conveyor chain in a shuttle car. EE was using a handheld grinder to remove metal clips that keeps the links together. the grinder kicked back striking EE's thumb on EE's left hand.

Employee stated EE was attempting to place EE's drill steel in the drill pot of the Roof Ranger RR2-13 machine, EE lost balance and fell into the controls which caused the drill pot to rotate. This caused hand to be twisted as the steel rotated. EE was diagnosed with fractured bone in right wrist.

Employee who is a welder stated they started feeling discomfort in right eye after work. Employee stated they started to have the same discomfort during the next workday and wanted to report it to mine management. Employee did seek further medical attention. Employee was diagnosed with Ultraviolet Keratitis. Employee was prescribed eye drops.

Employee was bolting in #5 entry on #1 section. After drilling with the 3' starter, EE inserted a 6' stack steel and used the fast feed to run the steel up the hole. EE let off the fast feed about 1 foot from the termination point but the fast feed stuck causing the steel to break striking EE on the right index finger resulting in 7 stitches.

Injured employee was drilling last hole of the cut up when a rock fell out striking their left hand resulting in lacerations to their left index finger requiring 7 stitches, middle finger requiring 4 stitches, and ring finger requiring 2 stitches.

Employee was bolting in 3 Left crosscut and had just finished drilling the hole and was in the process to install a bolt. As they pushed the install iii bolt up using the fast feed it stopped and they bumped the rotation. When they did it caused the 24" bolt wrench to break and strike them in the side of the face. Employee received 12 stitches.

EE was spot bolting in #6 entry on 2 section. EE was operating the 406 R/B while swinging the drill station out the drill steel caught the top and broke near the chuck. The steel sprung toward EE and struck EE on the left ring finger. EE transported self to plateau medical and was given 4 stitches in the finger, EE reported back to work on EE's next shift.

Employee felt a pop in EE's shoulder as EE was bending a 48" resin 3/4" roof bolt in the roof bolter machine bolt bender. The company was notified today (4/17/23) from the referred specialist that the employee has been taken off work due to EE's injury.

Injured employee was drilling a test hole when the steel became lodged in the top, while attempting to free the steel it fell out of the hole striking the left ring finger resulting in a laceration that required 10 stitches.

Employee stated they were assisting in the installation of a drainage ditch, positioned up hill to spot for the dozer. The dozer moved some material that had an old bent up section of gas line in it and when this happened, it caused the other end of the gas line to shift and it struck the employee across the back of their shoulders.

Employee stated EE had drilled EE's hole and was in the process of installing the bolt when EE felt EE's shoulder pop and start hurting. Being reported per citation #9559715 under protest to avoid any further action.

Employee was installing a 4 ft. roof strap when the strap fell off of the ATRS of the roof bolting machine, striking them in the left leg causing a laceration. Employee received six sutures to close the wound, received a full work release with no restrictions, and returned to work on their next regular scheduled shift.

A roof bolter operator was getting drill steel out of the hole when it fell, knocked off EE's glasses, and hit and scratched EE's left eye. A prescription medication was prescribed and used.

Employee was using a hand held grinder and cutting a 1.5" metal pipe and struck leg with the grinder. Sutures were required.

Employee stated they had drilled a hole and was in the process of installing a 6' bolt when a piece of rock fell that measured 2" x 18" wide x 2" thick striking them on the back. Employee missed on 07/22/2023 to seek medical observation. Being reported per citation #9559720 under protest to avoid any further action.

Employee stated EE was welding a slide in a Rock box and was reaching to finish welding and something popped in EE's right ankle

Employee was attempting to install a roof bolt into the hole EE just drilled. The bolt did not line up perfectly with the chuck of the roof bolting machine so EE bumped the rotation on the machine to try and get the bolt to go into the chuck. As EE bumped the rotation, EE's glove got caught around the bolt, wrapping EE's hand around the bolt, fracturing the left thumb. No Days Lost

Employee was drilling a hole in the mine roof when drill steel broke and hit EE's left forearm, resulting in a laceration requiring 10 staples. Employee returned to work their next scheduled shift with a full release.

Employee was cutting a tree. As the tree fell it slipped off the stump falling onto employees foot, resulting in a broken foot.

Injured employee was in the process of cutting expanded metal with a grinder to fabricate a guard for a belt head, when the grinder caught the expanded metal causing the disc to climb up the metal and contact employee's left index finger. Employee finished shift and sought medical treatment where employee received 7 sutures. Medical Only.

EE stated that EE was in the process of installing Roof Bolts in the #5 Left entry at 37 brk on the #1 working section. During the process of installing the corner bolts, EE stated EE's glove on EE's left hand got caught between the bolt lip and the chuck causing EE's left hand to wrap around the bolt EE was installing, dislocating EE's left middle finger.

Employee stated EE was bringing EE's stack steel out of the top the steel came apart from each other and struck EE in the left side hitting EE in the ribs, Employee left the mine site in an ambulance for treatment EE notified us there was no broken bones doctor told EE to take 2 days off.

Employee was installing a T-3 bent rib channel and caught EE's left index finger between the strap and the roof, sutures were required to repair the wound.

Employee was in the process of installing a .804 diameter bolt and while inserting the bolt into the drilled hole the bolt came to an abrupt stop which resulted in employee feeling a pop and a pain in EE's right shoulder. Employee later sought medical treatment and the results of an MRI were received on 10/5/2021 that reflected a torn longhead bicep muscle.

Employee Stated they had adjusted the drill pot on a DDr13 roof bolter and was watching it while bolt partner raised the drill pot and got the tip of left pinky finger caught between the longarm and the drill pot. Employee got medical treatment on 7/12/21 and was placed on restricted duty.

The Employee stated that they were bolting on 416 Fletcher walkthrough R/B, in 6 right on Unit 3, they had set an inside bolt and was swinging out to set to outside bolt, when a piece of the top fell and struck them on the left shoulder, left forearm, and left side of the face.

Employee was installing a 16' roof mat when it slid off the ATRS and cut EE's left wrist which resulted in a laceration that required sutures. No lost workdays.

After placing a jumper in the 4 mains belt EE started releasing the 3 ton chain hoist to let the pressure off the belt clamps. EE had worked the hoist 2 cranks when the belt broke outby the belt clamps allowing the clamps to fly forward knocking 2 top rollers loose on the belt pinning lower left leg between the belt structure.

While installing a cutting edge and corner bit on Caterpillar D-10 using 1 inch drive air gun, EE tore muscle in right arm.
