Employee, Airway Examiner, was removing block from a regulator to access a secondary travel way when an overhanging block fell approximately 3 feet, pinching EE's right pinky between the falling block and one EE was holding. EE sustained fractures to the right little and ring fingers and a laceration requiring 4 sutures. No Days Lost

EE was struck in the back by a falling timber, then fell forward into a scoop bucket. EE's face struck the scoop bucket. EE is off work awaiting an appointment with a specialist for a fracture in EE's lumbar spine.

EE was on one knee at the lolo and attempted to stand up. As EE placed EE's right hand on the lolo top rail to steady self, EE did not realize that the belt guide rail was so close to EE's hand. The belt guide wheel caught the thumb on EE's right hand. EE did seek medical treatment where EE received stitches in EE's thumb. EE has not missed any work.

EE was evaluated for Pneumoconiosis at the Occupational Lung Center in Charleston, WV. EE was diagnosed with a 15% pulmonary functional impairment.

Employee stated they had stagged up a set of loose forks off an IT to work on, when one of the employees bumped one of them and they fell over striking employee on the top of their right foot

Employee was sitting on a bucket using a torch to cut links out of a chain on the surface high wall miner. Employee stated they felt their foot get hot. Employee stated they had a small red spot on left foot big toe at the joint. Employee went to seek medical treatment 2 days after the incident.

The EE stated they had the right-side rear door opened on lube truck checking the heater. Working with my right hand and balancing myself with my left when, the door unexpectedly swung all the way open striking the tip of my middle finger on my left hand against the left-side door hinge. EE went to the hospital 2 days later and had finger lanced to relieve pressure.

An unintentional roof fall was discovered on 9 North Mains, in the Intake, 2 Breaks outby spad # 10634. The fall is approximately 30ft. long, 10-12 ft. thick, and 18 ft. wide. No injuries or entrapments occurred as a result of this incident. MSHA and WVMHST Officials have been notified.

An unintentional roof fall occurred at 3 South Mains, #8 Break, in the #7 Entry. The fall is approx. 5-6 ft high, 12 ft wide, and 15-20 ft long. No injuries, entrapments, ventilation disruption, or blocked travelways occurred. The area was dangered off and the lifeline rerouted.

Employee stated they had just moved up to the next row of bolts and set ATRS when they stepped under the canopy a rock fell out between the bolts, they had just installed hilling the canopy bouncing off hitting employee in the right thigh. Employee worked until 07/04/2026 went to the doctor and began missing work on 07/06/2026

EE had a piece of metal fall on EE's right ring finger while working on a continuous miner. EE received 3 stitches and was diagnosed with a fracture to EE's right ring finger.

Employee was roof bolting in #2 Entry when a rock dislodged from the mine roof while drilling, struck EE's hand, and caught EE's left ring finger between the rock and drill pot. Employee was evaluated at Boone Memorial, diagnosed with a fracture, and returned to work 6/4/2026 with no restrictions. No Days Lost

An unplanned roof fall occurred on the #1 section in #40 crosscut, #4 entry. The fall was not above permanent roof support, but permanent roof support was damaged. The rock measured 3' thick, 15' long, and 20' wide.

Employee stated they were changing the AC accumulator when their wrench slipped and as they reached to catch themselves an access lid fell hitting their left thumb.

EE and another employee were pulling the engine and removing nose cone on the radiator. EE was on the bottom under the 992g wheel loader. The other employee was on top of the machine. EE was taking the radiator nose off, accidentally took bolts out of counter weights and it fell on EE. Resulting in a fatality.

Employee was injured when EE's left foot was struck by a rock that dislodged from the mine rib. EE was diagnosed with a fractured metatarsal and EE's foot was temporarily immobilized.

Employee diagnosed with Occupational Pneumoconiosis with a 10% pulmonary function impairment after evaluation.

Employee was mounting a CAT 980 Loader when EE's foot slipped on the the step causing EE to catch self with EE's left arm. EE was diagnosed with a torn tendon and muscle and had to have surgery on 5/14/2026. EE's first day missing work was 5/14/2026.

An unplanned roof fall occurred outby the section loading point above anchorage. The fall is located at #16 crosscut from the #7-#8 entries measuring approximately 90 feet long by 10 foot thick. Supplemental roof support was added in the adjacent entries, and the fall has been breakered off.

Employee stated as EE was bolting in 3 left crosscut a rock fell from the top hitting EE in the back.

An unplanned roof fall occurred on 3/24/2026 at approximately 7:40 am on the slope. The fall occurred between crosscut No. 0 and 1 and was approximately 50' long x 20' wide x 5' thick.

Based on audiometric evidence and review on March 13, 2026, the employee meets the reportable hearing loss threshold under 30 CFR Part 62. The age-corrected Standard Threshold Shift was 25.6 dB in the right ear, exceeding the 25 dB average at 2000, 3000, and 4000 Hz from the employee's baseline/revised baseline audiogram. All 30 CFR Part 62 requirements were followed.

EE exited EE's carrier to attempt to dislodge a rock form the dolly on the back bridge. EE was pushing down on the slate bar EE was using to dislodge the rock and felt pain in EE's left shoulder. EE has continued to work until 05/07/2026 when EE was taken off work.

As the airway examiner was examine the weekly intake airway a small 10'x10' roof fall that pulled 4 roof bolts was found just inby the fan house, the lifeline was rerouted to another entry, we submitted a plan to clean and rebolt the area that has been completed.

Employee was helping remove a spindle group from a front suspension cylinder of a haul truck. When the spindle came loose it dropped down striking the employee's arm and caught it between the spindle and the steering link of the haul truck resulting in a deep laceration to the left forearm.

Employee was repositioning a hydraulic jack under the right front of a 777-haul truck. While positioning the jack to the correct spot employees left thumb got caught between the jack handle and the tow hitch of the haul truck. Resulting in a fracture and laceration of the left thumb.

A co-worker observed EE's 2022 Chevrolet 2500 truck had traveled over a berm and came to a stop on Coal Fork Road. The co-worker found EE in the driver's seat. EE was unresponsive and had no pulse. Co-worker began CPR. EE was transported via ambulance to Charleston Memorial Hospital and pronounced dead at 9:30am.

Employee stated EE noticed some push that needed pulled up the hill from the refuse, EE stated EE got the 325-cat excavator to remove the brush and has EE was doing so EE pulled a limb into the front glass of the excavator hitting employee on the right leg.

EE's rail motor EE was operating had de-railed. EE used a jack and a bar to put the ride back on the track. Upon doing so, the jack turned over causing the bar to strike EE on the right cheek area of EE's face. The ride went back on the track at that time and EE gathered up all EE's tools and went to the outside to be checked out for precaution.

Employee was pulling ventilation curtain from beneath the continuous miner pan when the tripped over a hydraulic oil can concealed under the curtain and twisted a foot/ankle. Employee was diagnosed with a Jones fracture of the left 5th metatarsal and underwent surgical repair on January 23, 2026. The employee is expected to remain off work until April 17, 2026 (Lost Work Days).

EE was evaluated for Pneumoconiosis. They were diagnosed with a 10% pulmonary function impairment on 12/19/2025.

Employee installed inside roof bolt with 54"x12" strap and swung out to drill outside hole. While lining strap hole with drill steel they stepped on loose rock, slipped and struck right wrist on strap edge between glove cuff and Kevlar sleeve, causing laceration. Treated with 7 sutures. Full release, no restrictions; returned 12/12/2025 to normal duties. Medical Treatment Only.

EE was found to have 10% pulmonary function impairment.

Employee stated EE was tightening a bolt on a belt clamp with channel locks, the bolt broke as EE was pulling it towards EE causing EE to hit EE's mouth losing EE's tooth. Employee had a root canal on EE's tooth on 03/16/2026

On November 24, 2025, the operator was notified by MSHA that the employee has a sufficient degree of pneumoconiosis.

Clear Cutting, a lot of the trees has grape vines in the tops, while cutting one tree when cut the grape vines pulled the rotted top down and hit my employee in the head

Employee was installing PTO driveline when it fell down striking the employee in EE's mouth area resulting in a chipped right front tooth.

A non-injury roof fall occurred in the primary escapeway on 4 belt, entry 1, just inby crosscut 10. The fall was approximately 25'x15'x5-6'.

While walking down an incline to clean out an obstruction in a culvert caused by beaver activity, EE took a step and felt a burning sensation in EE's left knee area. EE has worked every day since and had a procedure on 02/27/2026. EE returned to work on EE's next scheduled shift.

Employee was bolting in the #1 Entry when, while checking hole depth with a 6-ft., 0.804 diameter bolt, it slipped from left hand and smashed right ring finger between the bolt head and drill chuck. Employee self-transported to CAMC General Hospital, received four sutures, and returned to full duty on 11/13/2025.

Employee was lifting a fire extinguisher up on a dozer with their right arm they swung it up onto the track then up on the hood. They completed the shift, went home and stated that they then noticed their arm didn't look right but had no pain. They reported it the next day but continued to work until 12/18/2025. They were then was taken off work to repair an upper arm injury.

Employee was hauling supplies into the mine. They encountered a hill on 3 belt track entry. They derailed near the bottom of the hill and their arm struck the canopy post in the cab.

An unplanned roof fall occurred on 10/18/2025 at approximately 1:16 AM on the 4 right panel of Unit 1. The fall occurred in the intersection of No. 8 entry, crosscut No. 17 and was approximately 20ft X 20ft X 10ft thick.

EE was in the process of removing a J-Bolt from a bottom hanger using a 1/2-inch impact to remove the loop nut. J-Bolt swung around and hit EE's finger.
