While performing overhead torch work to cut bolts, the miner was struck by hot slag. The slag traveled between the face shield and safety glasses and contacted the eyelid. The miner was evaluated by an ophthalmologist, where a small foreign body was removed and the burn was treated with prescribed ointment.

Coworker was replacing a cutting edge on a front-end loader.  While tightening the new bolts, coworker bent over to move the 1" air impact gun from one bolt to the next and felt a "pop" in EE's right shoulder.

EE was operating D8 bulldozer when EE ran over a large rock. The machine was jarred causing dust to fall off of the roof of the cab. A piece of dust got into EE's eye, causing a scratch to EE's eye.

EE was moving material from a bench to be loaded onto trucks. EE had a full bucket and swung to the left rapidly, causing the rear to lift. EE tried to correct the instability by swinging back to the right, but the machine was too far off balance causing it to overturn on the cab side. In the process of the rescue the ee pulled a muscle in EE's back.

While using a compressed air line with nozzle attached, it slipped from EE's gloved hands striking EE on the lens of the safety glasses and EE's right cheek. The impact caused bruising of EE's left eye.

EE was replacing a relay for the fuel pump truck. EE tried to drill out the new electrical terminal to fit on the existing stud using the drill press. The bit grabbed and spun the terminal, pulling EE's hand into the bit, causing a fracture and a laceration on the tip of EE's left index finger. EE received 12 sutures.

EE was removing a rock from a de-energized machine, stored tension in the v belts caused the unit lurch forward when the rock was removed. This severed the EE's fingertips on EE's right hand including the pinkie and ring finger to the distal knuckle.

While operating the dozer in reverse, the EE had head turned and struck a rock which jarred their neck.

EE was standing on the ground welding at face level wearing safety glasses, a face shield, and mask. EE stopped grinding lifted EE's face shield to speak to another EE. EE felt dust enter EE's eyes. EE immediately used water to flush the eye, Next day EE's eye felt fine. A few days later it became irritated. The EE did receive medical treatment to have a piece of old steel removed.

Employee was welding underneath truck with welding helmet and safety glasses on when buildup dirt fell from the bottom of the frame and dropped down onto the shield and scratched their left cornea.

Employee sustained a crush injury to the left middle finger while removing hardware that secures the top of the crusher to the base of the crusher.

Employee was using a torch to remove rusted bolts on the middle screen deck when hot slag entered left ear causing a perforated ear drum. Employee did NOT seek medical treatment until April 10, 2019.

Employee's were attempting to empty a small metal scrap bin into a dumpster. They were using an overhead hoist to raise the bin. Upon turning the bin over to empty it out, one employee got hand pinched between the bin and dumpster, causing a contusion to right hand.

The injured employee was helping change screens on the tertiary plant. While starting the nuts on the side rail a second employee on the inside of the screen began to tighten the bolt with a cordless impact wrench and caught the employee's left thumb between the washer and side rail.

Employee was on ground when a hoist began lifting a screen cloth up to the screen. The cloth became detached from the lifting hook and bounced around when it struck the ground. It hit the employee's hand. It barely made a mark and the employee worked the rest of the week. Over the weekend EE went to the ER and was told EE had a broken bone in left hand.

Employee was holding the head of a bolt while turning the nut with an impact gun causing the bolt head to lacerate his left palm, requiring 40 sutures.

Employee was welding cracks on a bucket when a piece of metal went into his eye.

As the EE was cutting through pipe the portable band saw slipped and lacerated his R hand.

Left Ankle/Foot - Loading a walk behind saw onto it's trailer, it reared up hitting ankle/foot causing ankle fracture

emp was using a skilsaw when it jumped and cut his left little finger. emp received 7 sutures

While using a cutting torch a piece of slag entered his L eye. This was not reported earlier because the prescription that was given to the EE was not clear on the paperwork received from the Doctor.

As the EE was tighting a plow bolt on a quarry loader his L hand came into contact with the bolt head and the impact wrench caused it to spin and lacerate his L palm.

EMPLOYEE WAS OPERATING A HAND GRINDER WHEN THE CUTTING WHEEL SHATTERED. THE GRINDER WAS KICKED OUT OF HIS HAND, CUTTING THE 4TH FINGER OF HIS LEFT HAND. THREE SUTURES WERE REQUIRED TO CLOSE LACERATION.

Miner was grinding steel and F.B. entered Lt eye under safety glasses-required follow up treatment after continued irritation.

As EE was tightening the bolts on the pay loader bucket, the 1" air wrench slipped off one of the bolts striking his right hand between the air wrench and pay loader bucket. Bruising the tendon of the second finger in the right hand .

AS A WELDER WAS WELDING AND REPAIRING PL-74. HE WAS GRINDING A WELD WHEN THE 4" GRINDER KICKED BACK AND MADE A LACERATION TO HIS RIGHT CHEEK ON HIS FACE.

Employees were flipping hammers at a horizontal crusher - STEDMAN CRUSHER. While flipping hammers, you have to move them to align them up. Employee caught middle finger under hammer with gloves on. Employee never looked till break time at approximately 9:00 to check out his finger. Employee reported right away.

While fastening a slat while using an impact wrench a foreign object entered the Left eye.

Employee was using a grinder, when the cut off wheel got caught, flipped the grinder and cut his hand.

Miner was using an impact wrench backing a bolt out of a screen deck when the length of the bolt caused the left hand of the miner to be pinched against the top area of the screen deck where he was working.

Left side groin area started hurting while flipping a headnut for a crusher.

Employee was using a drill to make bolt holes to mount an air conditioner unit. While drilling into the metal, a sliver of metal went into his right eye.

Employee was assisting with the move of a portable crusher when something thought to be a spalled piece of metal hit him in the face - lip/chin area.

Employee did not have eye protection on and suffered welder's flash.

ACCUMULATED EFFECTS FROM USING THE BREAKER MACHINE (EXCAVATOR) THE SEAT HAS A HARD BOTTOM AND THERE IS NOT MUCH GIVE WITH THE CONTINUAL JARRING.

Tightening a spring bolt and was using an air wrench, slipped and cut some of his pinky finger off.

EE had a wrench and was tighting up a bolt on a crusher in the shop and there was no room for a air wrench to get into the space, he was pulling on the wrench and something popped in his neck.

WELDING A BUCKET WHEN DIRT GO IN HIS EYE

mechanic started acetylene/oxygen blow torch, heard loud popping sound, turned it off, it "blew up" near hose. his left palm, side and back of hand was burnt. He performed first aid on himself ,we sent him to Tyrone Hospital where they prescribed him pain relievers, antibiotics and wrapped his hand. torch was rebuilt approx. 2 years ago by National Valley Gases.

Employee was cutting trees when the chainsaw he was using slipped and cut his left knee. He then went to Nason Occupational Health where he got two stitches.

EE WAS WORKING NEXT TO WELDER. THE WELDER WAS USING A CARBON ARC TORCH, CUTTING WELDS APART & APPARENTLY THE EE WAS CLOSE ENOUGH TO THE ELECTRIC ARC TO RECEIVE FLASH BURNS.

WALKING DOWN CONVEYOR AND SLIPPED ON ICE, CAUSING CUT TO RT. HAND.

WHILE CUTTING A STEEL PLATE WITH A TORCH, A PIECE OF THE BURNING STEEL FLEW UP FROM UNDER EE'S SAFETY GLASSES CAUSING A POSSIBLE ABRASION TO HIS RT. CORNEA OF HIS EYE AND ALSO BURNING TO HIS RT. EYELID.

PULLING STEAM JENNY. JENNY RAN OVER RIGHT ANKLE.

INJURED WAS OPERATING 235 CAT HOE WHEN RIGHT LIFT JACK BUSTED CAUSING THE SIDE WINDOW TO BREAK - GLASS & OIL HIT RIGHT SIDE OF BODY.

WORKING ON BLADE OF GRADER, STOOPING DOWN ON RIGHT KNEE, STRAINED KNEE WHILE WORKING.

CUTTING TREES TO CLEAR FOR STRIPPING HE CHAINED LOG TO 7 STEPPED BACK AND WHEN PULLED LOG KICKED BACK HITTING HIM AND LANDING ON HIM.

INSTALLATION OF NEW EQUIPMENT INSIDE BUILDING INADEQUATE VENTILATION IN BUILDING TEMPORARY LIGHTHEADEDNESS,WEAKNESS.

WHILE WELDING, CUTTING STEEL, ANOTHER EMPLOYEE WAS RUNNING A WIRE WHEEL ON A HAND GRINDER AND A PIECE FLEW OFF HITTING IN EYE FROM SIDE OF SHIELD (LEFT EYE).

EMPLOYEE WAS TAKING CRUSHER APART WAS IN CAGE OF CRANE HAD DUST PIPE HOOKED IN CAGE WHILE BEING LOWERED TO THE GROUND IN CAGE DUST PIPE BECAME CONNECTED WITH CRANE HOOK & UNHOOKED CAGE FROM CR ANE EMPLOYEE FELL APPROX 10 TO 15FEET
