Mostrando los 13Individual sustained a 5cm laceration to left forearm while performing maintenance with a 4.5" grinder. Grinding wheel became bound in material while individual was cutting a weld, causing the grinder to accelerate forward into their arm.
Miner entered HP400 cone crusher after performing lock out/tag out to clear a large rock stuck between cone and mantel. Miner failed to recognize the stored energy in the cone being suspended by the rock. When dislodging the rock, the weight of the cone came down trapping both feet between the cone and mantel.
DUMP BOX OF HAUL TRUCK BEING OPERATED BY MINER WAS JOSTLED BY EXCIVATOR BUCKET WHILE LOADING MATERIAL. MINER REPORTS NECK AND BACK PAIN.
Team members were picking up a sand screw equipment off a trailer with a crane. One of the rear rigging legs was hung up on a bolt/stud. Team member went to correct the rigging. As EE's hand got near the chain it released tension, taking EE's hand with it towards the sand screw frame rail, pinning it between the chain and the frame. Tip of right index finger was fractured.
WHILE PERFORMING MAINTENANCE ACTIVITIES, PNEUMATIC HOSE ATTACHED TO IMPACT GUN SEPARATED AT UNION. COUPLER AT END OF HOSE STRUCK TM IN RIGHT CHEEK AND MOVE BETWEEN EYE AND EYE PROTECTION. TM WAS EVALUATED AND PRESCIRBED MEDICATION
Miner was operating long stick excavator over a period of time and felt weakness, muscle burning and loss of feeling in right shoulder and arm. Miner lists the date of injury as early September but did not report it to employer until 12/05/2018 and first sought medical treatment on 12/15/2018. Attending physician has listed injury as right arm pain and hand numbness.
ON 8/16/18, MINER REPORTED EXPERIENCING SEVERE BACK PAIN AFTER SITTING IN A CHAIR WHILE OPERATING THE DREDGE ON 8/14/18, FOLLOWED BY A 12 HR SHIFT PERFORMING MAINTENANCE ON 8/15/18. MINER INITIALLY REPORTED TO SUPERVISOR AND A GROUP OF MORE THAN FIVE OTHER MINERS THAT BACK PAIN WAS NOT RELATED TO WORK CONDITIONS.
The employee attempted to stop a pulley and belt system with employee's left hand.
Running a half inch Drill Motor the bit caught and the drill twisted around and fractured the employees right hand,
Mine operator sitting in seat of excavator turned to see what other miner wanted as he had yelled. When he turned in the seat coat sleeve caught swing lever and swung excavator into other operator's foot.
Employee was adding a drill rod onto the rock drill and it fell off and smashed his foot breaking the toe.
WHILE GRINDING A PIECE OF METAL WITH A HAND HELD AIRDISC GRINDER, DISC CAME APART WHEN IT CAME IN CONTACT WITH METAL CUTTING LEFT ARM.
