Team member was making a squatting motion on level dry ground when what EE claimed to be EE's knee gave out and EE fell down to the ground. Outside conditions at the time were clear and sunny.

A haul truck was driving erratically up a ramp. It stopped near the top, rolled back down into the left repose and tipped onto its side. The operator was found unresponsive, removed from the truck, and airlifted to a Medical Center. Driver died the following day from an apparent medical condition, as no injuries were found.

Employee alleges employee was injured at work on 09/30/2023. Employee did not report nor notify anyone of an alleged incident until employee sought medical care on the evening of 10/1/2023. Employee has not identified how employee was injured and is unable to provide any details of the alleged incident.

Employee was examining the belt line when EE stopped to sign a date board. As EE went to step away, EE felt a pop in the left knee. Employee continued to work until EE received surgery on 10/9/2023.

The technician was working outside when dust/dirt blew into their left eye. After removing the dirt particle, the Doctor prescribed an antibiotic for the tech's eye.

Team member suffered heart attack as they were coming in to work, Team member had not yet started their shift and was walking to the office when they collapsed.

The employee says: felt knee pop while walking around haul truck. The employee continued to work and did not want medical care until 05/03/2021. Employee was then evaluated and scheduled for arthroscopic surgery. Employee had surgery on 07/06/2021 and became reportable and began missing work shifts on 07/09/2021.

While passing through a mandoor the door closed catching the end of EE's left index finger between the door and the door frame resulting in an avulsion requiring sutures and a fracture to the left index finger. The employee was treated at a hospital on 9-29-20 and following a visit with an ortho specialist on the same day, released to work full duty.

Employee was helping tram a mobile feeder from the 5 Right section to outby. The other employees were getting ready to pull cable slack when the employee stepped back to get out of the way. The employee felt discomfort in the back of the right knee. Employee continued to work until 10/2/20 when surgery was needed to correct the injury.

The delivery driver was found unresponsive laying on the ground near the cab of the truck. CPR and an AED were administered by mine rescue personnel. The victim was immediately transported by ambulance and turned over to local EMS crew. This is a non-chargeable medical event.

A haul truck operator began feeling ill with chest pains and shortness of breath. EE was transported to a local hospital where diagnosed with having had a mild heart attack and a blood clot. Reporting Non Chargeable.

The electrician had been troubleshooting a misalignment switch at the bottom of the conveyor. When walking up the incline to exit the area they began experiencing chest pains. Electrician was transported to the ER where they were diagnosed with having had a mild heart attack.

Descending from haul truck caught foot on water pump fitting while twisting body. Felt pain in left knee. Employee continue to work until recent surgery resulting in injury becoming recordable.

EE was setting at rest in a covered minetrip when another employee reacted to a loud noise and swung and struck EE in the mouth with the hand held radio they had in their hand, resulting in 2 broken teeth to the EE which had to have dental repair. The incident was not reported to mine management until 11-29-2017.

Person was weed eating around office building and was found unresponsive and was taken to Logan General Hospital where ee was pronounced dead. Probable cause was natural death. We have not received the autopsy report as of yet.

Employee was traveling from the track entry to the Return entry, as employee cleared the mandoor employee slipped on some ground rock and the air pressure caught the mandoor closing it, the employee was struck in the back by the mandoor handle as the door closed.

A millwright stepped up on a tub cable hook to connect the ground lead from welding machine to the revolving frame of the dragline. When EE stepped up EE felt pain in right knee. On August 18, 2016 EE had a medical procedure to knee that makes this incident reportable.

While exiting the plant due to an impending thunderstorm the employee was struck on the left cheek by an airborne piece of broken fiberglass siding. The impact left a laceration that required sutures to close. This was a weather related incident and act of God. EE was released to unrestricted duty.

While the injured was walking the alternate escape way (belt entry) out of the mine as part of the quarterly escape way drill, EE felt/heard a "pop" in EE's left knee. EE continued to work up until 7-27-16 when EE had surgery to repair a torn meniscus.

Employee was walking when he felt a sharp pain in the back of his right ankle area. There was no slip or any incident that would contribute to the injury. After the results of an MRI it appears that the employee has a tear in his tendon. After our investigation that concluded on 12/07/2015 we are reporting this as a Reportable Injury.

A mayday was called for an unresponsive haul truck operator at the SH22. A loader operator boarded the haul truck and confirmed there was a medical emergency. He began chest compressions until mine rescue personnel arrived. The patient was then transferred to the public ambulance service. This was a medical emergency and not the result of an accident.

Heavy gust of wind blew dust and foreign material into eye requiring removal of material by optometrist.

Due to horseplay, injured EE was pushed, lost his balance & twisted his shoulder as he caught himself. Dr. determined EE had strained shoulder muscle which resulted in 1st aid incident. He was revaluated by an orthopedic surgeon & determined to have a torn rotator cuff. Surgery conducted on 1/17/13. At a follow up on 1/21/13 the dr wouldn't release back to modified work.

The haul truck operator was assigned to conduct safety observations of various jobs throughout his shift. The next day he was diagnosed with actinic conjunctivitis of both eyes.

EE felt sick at work and wanted to go home. His daughter came and picked him up and took him to the doctor. Drs at hospital thought he had a mild heart attack. At that time company reported to MSHA. The following day it was determined that it was not a heart attack but his gallbladder.

The employee bent over to pick up and get out the way a section of miner water line. When he bent over he stated that he felt a pop in his back.

Standing beside coal truck while mechanic put air in tire and was stung by a bee.

While working on a track dozer, the technician stood up from a squatting position on the concrete shop floor and turned to his left. The twisting motion fractured the fibula just above the right ankle.

EE was kneeling down removing a guard from the side frame on a shovel. When the EE stood up and started walking, a pain went through his left knee at that point. He did not trip or slip on any foreign objects.

The technician was found slumped over, but conscious, in the seat of his pick-up. The technician was driven to the first aid station where he was transferred to an ambulance. The technician was pronounced dead on arrival at the hospital.

At the time of the incident the tool room leadman was standing on the lower step of a 4-foot step ladder that was set up parallel to a storage rack system. As the leadman reached back to retrieve a part that was located slightly behind him and to his right, he felt a sharp pain in his lower back.

An employee suffered a fatal heart attack while standing beside a Caterpiller D-10 dozer. Employee was found on the ground next to the track of the dozer. He was found at approximately 3:30 pm. The actual time of the heart attack is unknown but was probably within 15 to 20 minutes of the attack.

On 10/3/07, an EE with Express Shuttle, who is contracted by Burlington Northern Railroad, passed away due to an apparent heart attack on the Black Thunder Mine site. The cause of death has not yet been determined by the coroner.

EE saw inj EE walking up north side of 2 entry to check steering jack on bt4. EE turned again & saw inj EE collapse on floor. When they got to him he looked like he was having a seizure.

The employee was entering the door to the plant, when it was caught by the wind catching the top of his right middle finger between door & frame.

EE was preparing to go home after shift ended, when he experienced chest pain. He was transported to Sevier Valley Hospital by ambulance, where treatment continued. Life flight helicopter transported EE to Utah Valley Hospital. EE passed away either in helicoper or at hospital, due to heart condition. EE's shift started at 2:30pm on 12/12/06.

An employee was taking a break in the lunch room and felt some irritation in her left eye. She was checked by a rescue team member and then taken to the eye doctor, where she was diagnosed with a corneal abrasion. She was given an antibiotic ointment to prevent any infection. The employee could not recall any incident that would have caused this injury.

Employee was picking up ballast when the wind blew a metallic foreign body into his (L) eye. Particle was embedded. He returned to regular duty on 8/2/06.

Employee was gathering cleaning supplies from a pickup; and she felt an itching sensation on the back of her left hand. She continued to work, and noticed pain and swelling progressing over the next several hours. She was taken to the hospital ER, and the attending physician said her symptoms indicated she had been bitten and subsequently poisoned by a black widow spider.

This EE contacted Legionnaires Disease that was determined to have come while working at this site.

The technician walked approximately 20 feet from his equipment to look the work area over. While walking back to the equipment, he experienced pain on his right knee cap from repetitive motion.

A contractor for AEP,was operating a small dozer & felt pain in his chest. He stopped & rested a little while and had a heart attack.

A particle flew into the eye and the employee tried to flush with eye wash with no success.(corneal abrasion)

Employee felt severe lower back pain when he finished his shift

HE WAS WALKING IN A STOOPED MANNER DUE TO HEIGHT RESTRICTIONS AND FELT PAIN IN HIS BACK.

EE was operating dozer, clearing trees, and messed with beehive - stung by bees 4 or 5 times. He is allergic.

EE STATES WHILE MOVING PORTABLE HEATER HE FELT PAIN IN RIGHT SIDE OF CHEST. WE DO NOT FEEL THIS IS A WORK-RELATED INCIDENT. (A-FIBRILLATION; BLOOD CLOT ON HEART.)

EE STOOD UP AFTER EATING LUNCH AND AS HE WAS TURNING AROUND TO LEAVE, A PAIN IN HIS RIGHT LEG CAUSED HIM TO SIT BACK DOWN.

EE STATES WHILE PICKING UP TRASH, HE BEGAN HAVING CHEST PAINS. EE WAS TAKEN TO THE HOSPITAL AND IS SCHEDULED TO UNDERGO BYPASS SURGERY. WE DONT FEET THIS IS A WORK RELATED INJURY OR ILLNESS.
