An electrician was walking down the stairsteps from the #3 pump house and felt pain in their right ankle. The stairway was clear and safe for travel.
The C/M operator was mining 4 right crosscut into #5 heading when car driver found injured employee at the boom of the miner suffering from head and face trauma. The exact cause of the trauma is under investigation.
An employee was working in their office when they experienced left eye irritation.
While employee was exiting 33 Shovel, a gust of wind caught the door pinching left hand below the wrist in the first group of knuckles between door and door frame. Fractured finger.
Employee felt discomfort in their back and was diagnosed with lumbosacral radiculopathy.
EE raised from a squatting position on 3/24/2021 and felt pain in EE's left knee. On 9/23/2021, EE had an out-Patient procedure from that March Injury. EE has returned to work and report is Medical only.
Alleged occupational illness. After shift was completed, supervisor recognized employee walking with a slight limp. Supervisor asked employee if they were ok. Employee stated that was walking and felt a popping in knee, causing soreness. Employee has a history of non-occupational knee issues.
Employee stated walking behind haul-ruck, in fuel building felt back pain. Employee seen by responder, taken to hospital. Made no reference to work relatedness. Stated sought chiropractic care previous day. Sept. 5th, its reported employee stated had been performing several tasks that shift; shoveling, walking, placing wheel chocks for haul trucks, pulling fuel hose, greasing etc.
As the EE bent over to pick up a two wheeled cart EE felt pain in EE's back. The mechanism of injury was idiopathic in nature and due to a chronic condition. The EE was seen by a physician and placed on restrictions.
EMPLOYEE WAS STEPPING BACK AND ALL EMPLOYEE'S WEIGHT WAS ON LEFT LEG AND EMPLOYEE'S ANKLE GAVE WAY.
A shovel operator felt some discomfort in lower back when doing the pre-shift inspection. As the shift continued, the pain increased and EE sought medical attention after the shift.
Ee was squatting down inspecting a self aligning return roller assembly, as EE squatted EE felt something in EE's right knee slip. EE could not put any weight on it. EE was transported to the E.R. and taken off work for knee pain. This is a idiopathic incident, no event other than squatting.
Crusher employee was opening overhead door. Wind blew dust and debris into the shop area. Employee got dust in eyes. Flushed eyes with eye wash solution. Left eye still felt sore, safety rep brought employee to the eye dr. There was a small foreign object in employees left eye. It was removed and antibiotics were applied. Employee returned to work with no restrictions.
While working in the pellet plant the EE got dust in eye and later had eye flushed and was prescribed antibiotic drops.
While loading the 520 haul truck, the 102 shovel operator suffered an apparent heart attack. First responders were dispatched to the scene and EMT's began to administer CPR. The operation technician was taken by ambulance to the local hospital and was pronounced dead shortly after his arrival.
Employee was walking from job trailer to company truck without safety glasses when dust/dirt blew into his right eye. Employee did not report eye irritation until the start of his shift the next morning 09/06/13. Employee was taken to Eye Dr. where foreign body (dirt) was flushed out and given an antibiotic prescription as a precaution for his eye.
Employee states that when he bent over to pick up his gloves his knee twisted and locked up. He was unable to straighten it out.

The citation is being contested and this report is submitted under protest.**Employee strained back and felt a pop in right hip while stepping down into work box to work on a lower crusher screen deck. No Conditions identified that contributed to the incident.
Employee was crossing under a conveyor belt, a belt string entangled his earring and pulled the earring out of his ear.

The citation is being contested and this report is submitted under protest.**Employee felt a twinge in her back when she got out of her desk chair. Later felt pain and sought medical attention. No conditions identified that contributed to the event.
EE was exiting the porta john and slung the door hitting something. EE discovered a raccoon hissing at him. EE right leg was scratched by the raccoon. Freeing himself from the raccoon ee hit the raccoon on the head with a board killing it. EE was taken for medical treatment and released to return to work same shift. The raccoon was sent to Jefferson County for testing.
The injured employee was working in the pit on a windy day. He was wearing non prescription safety glasses while doing maintenance work when a gust of wind occurred. The felt a particle enter his eye, his eye was washed out but still felt some irritation so he was sent to the clinic where they diagnosed him with a corneal abrasion and prescribed antibiotic drops.
Employee got dirt in his eyes while watching a liner being lowered by the crane in our crusher. Incident was reported and employee flushed his eyes. On 3/31/2011 employee stated eye was still sore and went to clinic. Rust was removed and antibiotics were administered.
EE was at the bottom of the slope and said his knee gave out and he was complaining of having trouble breathing. EE was brought back outside and a ambulance was called.
Employee stated he twisted upper back while exiting under #2 conveyor after changing return rollers. He turned to exit from under the conveyor and twisted, causing back pain. Intial clinc treatment was fist aid only. Received notes from clinic on 6/12 stating employee will be attending physical theraphy.
Injured was walking by the lights for the #23 Drail on 8Q when the light exploded. The glass made several puncture wounds to the right ear area. Injured was taken to medical facility. Investigating why the light exploded.
Employee states that while walking along side a suspended load, he felt something pull in his calf muscle. He noted that the leg muscle had been "tight" for 2 days prior. There was no specific incident that he can attribute the pain to.
