Mostrando los 39The right side pinner was moving from #7 to #8 entry. The Ram Car was setting idle with the pump motor on waiting for the other car to come out of #9 entry. Employee was moving cable out of the way for the pinner to pull into face of #8. EE was standing behind ram car when the car released brake causing ee to rollback striking employee in back of knee.
Employee was operating a Diesel Scoop about 30' from the top of the slope. The scoop died and rolled down the entire slope and hit a concrete wall at the bottom. The employee had a bump on head and was complaining of back pain. Employee was taken to hospital seen and released. This accident did not become a lost time until 11/28/17 when Doctor took employee off work.
It had been raining that morning and haul road was wet and slick. The grader had not graded that section of haul road prior to the accident. As EE went around curve, the truck started to slide in the mud. Truck slid causing it to overturn.
On 08/31/2017 employee was operating an end loader and ran over a rock jarring their lower back. Employee continued to work and on 10/25/2017 they went to the Doctor for treatment.
The employee was lapping up old belt, when the belt lapper that the employee was using slipped off of the belt hitting employee in the groin
Driver went into a curve and turned tractor/trailer over. Driver says does not know what happened.
Road was wet. Driver said that EE looked down and when EE looked back up EE was at the edge of the road and ran up on the side of the safety berm, which knocked the wheel back into fuel tank. Driver sustained cut on head from hitting light and bruised ribs.
Employee was driving a scoop and ran over a rock, jamming employee head into the canopy of the scoop injuring neck.
The employee was shoveling rockdust into a bantam duster with back turned away from an oncoming examiner that was traveling toward the employee in low visibility. The examiner bumped the employee's leg and pushed it into the rockduster.
EE traveling out on damascus mac 10 mantrip employee failed to maintain EE's entire leg inside mantrip while traveling through airlock door and was struck by door causing seven stitches to left leg (shin).
Employee was driving to work in his personal vehicle on access road leading to mine. He ran into the mine's water truck on the access road. Employee suffered two cracked vertebrae in his neck.

EE was being transported for Random drug test, after taking random he was being transported back to his equipment by Foreman in a Ford F150 when he drove around parking ditch and hit a hole causing him to bounce. EE finished his shift and didn't report injury until 9/8/15. He missed first scheduled work day on 9/8/15. On 9/11/15 ee was terminated for failing Random Drug Test

Employee was operating a 773F haul truck and stated the bed was bouncing up and down when empty, causing him to injure his lower back. Employee did not Communicate to anyone that he was injured until 04/29/2014.
Employee was operating a 777F Haul Truck, and sometime during the shift employee injured his back, employee did not report the incident until 3/1/14. Employee stated the incident actually occurred on 2/14/14.
Employee was tramming a Ram Car back to the Continuous Miner. The Continuous Miner has just moved to a new location between crosscuts #7 and #8 entry when the Ram Car struck the tail of the miner causing his head to strike the canopy of the Ram Car.
Employee states drove over something in road causing him to strike head on top of Ram car canopy. Employee was off with medical issues I didn't know he was off concerning the report he filed in June of 2013. Had surgery in Dec. 2013 concerning neck injury!

Employee was traveling underneath a belt line guard located on the 2nd Main Belt Line at crosscut 7+40 and stuck his head on the guard causing an injury to the back of his neck.

While employee was taking lil-mac down slope it picked up speed and he couldn't slow it down so he tried to get off. He lost control and fell out on chest.
Injured employee was standing in between two (2) mobile rides, when one (1) of the rides was struck by a scoop, knocking the rides into each other, pinning the employee causing a fracture of the right lower leg.
A rock (4' x 3'x 3" thick) slid from atop the loading section of a ram car landing on top of the injured employee's left foot.
EE was in the process of changing a flat tire on a 3-wheeler transport ride underground on #2 unit. He had hooked a chain to the front of the 3-wheeler and was in the process of raising the ride about a foot and a half, when the chain came off, causing the ride to hit him on the left knee. Because of low coal, he was on both knees. He was taken to a local hospital for cut to knee.
Employee was in the process of reclaiming belt structure and was loading the structure in a scoop bucket. While doing so, he accidentally placed his left foot underneath the bucket as it was being lowered to the floor. As the bucket came down, it pressed down on top of the foot and fractured the tarsal bone. NOTE: He was wearing metatarsal boots.
EE was in the process of doing maintenance on the E. underflow pump located in prep. plant. Before it was started, power to the motor was to be knocked, locked, tagged out. He removed the belt guard from the underflow pump; grabbed the motor pulley with his right hand while it was still running causing the r. thumb to be amputated below upper knuckle.
EE stated that while operating a ramcar on the section, he dipped down in a hole & then bounced up throwing him against the roof of the canopy. The impact caused him to hit his head against the canopy. He was semi-unconscious immediately afterwards & then responsive. Taken to the hospital, treated & released. On 7/28/08 he went to a chiropractor, & now has begun missing work.
