A 966 loader was loading from a stockpile and struck a large rock that was buried beneath the stockpile. The impact injured the wrist of the operator.

Supervisor was sitting in parked vehicle, when he was struck by employee personal vehicle head on going about 15-20 mph at the time of incident. Supervisor sustained injury to back.
Employee was in process of being task trained to operator 330 Articulation truck and was a passenager, loader 950 got into soft spot and loaded landed on its side at the loading area. The loader bucket struck the bumper of truck.
Employee stated that he was operating a battery mantrip. Employee felt something "pop" in his right shoulder when the brake lever was pulled. Employee was placed on Restricted Duty on 4/8/2011
Employee was operating a fuel truck when the operator drove through a drainage ditch going across a haul road. The employee fractured two lower vertebrae's as a result of the impact.
Employees were tasked to bring an electric rectifier from the surface to the haulage. During transport, the rectifier became wedged between the ribs of the drift. A 6" x 8" x 24" timber between the battery loci and rectifier was a brace for the loci to push against. When the loci contacted the timber, the timber twisted and the movement caught the employee's pinky finger.
Employee stated that he was working on the feeder when a ram car trammed near him. The oil compartment caught his foot between the compartment and the coal floor. His FLWD did not occur until 03/17/2010 due to surgery.
The EE drove into a pot hole, the vehicle bounced, and he hit the left top door/cab with his hard hat. He was wearing his seat belt. First aid provided and transported to RMCH ER. Initial evaluation was a chipped back top tooth on right side. He was released back to work without any restrictions and referred to his personal dentist for further evaluation and treatment.
Acceptable conditions- Employee was removing equipment from belt structure and got his finger caught between two pieces of equipment causing a fracture to the left pinky finger.
Employee was operating Loci when he jolted his back in the seat as the loci & train transitioned from rail to pocket rollers. He RTW & continued assigned duties, no day missed. No new event, inj. or work cond. aggravated injury. Became DAFW when doctor took him off work due to employee complaints of cont. pain. Appt to consult for surgery - June, 2010. First day missed 5-12-10.
Haulage train, powered by Trolley #685, loading ore cars on the haulage level was struck from behind by a second train entering the same loading panel. No damage or injuries were reported at the time. Several weeks later, employee in Trolley #685 complained of back pain associated with the incident.
Employee was mucking in a line with a LHD with a cab. As the employee moved forward a rock came around the front of the bucket. The LHD wheel ran over the rock and jarred the employee resulting in a back injury.
Employee was operating a 1350 LeTourneau loader when the right lift arm ball broke causing a major hydraulic line to get stretch and come loose and whip around hitting the operator's cab on right side. The impact of the hydraulic line broke the cab window and sprayed employee with splinters of glass, causing a cut on employee's right ear and shoulder which required sutures.
As employee was riding mantrip, he raised his left arm up to shield himself from the run through track drops. As mantrip travelled through, his left arm was pulled back over his head.
Employees were working on the haulage level pulling muck. Injured was in the process of placing a 4 x 4 rock remover in the chute to remove rock that wedged in the chute. Co-worker was operating the chute door upward slowly & paying attention to the remover & chute door while injured was guiding remover into position to push rock when his finger rock or the chute.
Employee in an attempt to locate hole under conveyor, ran his hand under conveyor pan. Employee's glove and right middle finger were pulled toward the screw conveyor and golve was pulled off his hand. Resulting injury to finger nail/nailbed required 3 sutures.
The employee stopped at an intersection and did not have a good visual because of the loaded bucket. He placed his hand on the door to raise himself up to get a better look. As he did this either his elbow or his leg contacted the control causing the machine to pivot pinching his right ring finger. The finger was also fractured. The laceration took 4 sutures to close.
The employee was riding in a mantrip as they exited the mine at shift end. At an intersection, the mantrip hit a rib with the right front end causing employees head to snap back and forth hurting his neck. He suffered a Whiplash that was too sore to return to work.
Employee stated while driving a loaded coal haul truck she hit a bump in the haul road which caused pain in her lower lumbar area.
Employee was swapping out supply cars. He was standing near the rail to uncouple the cars. When the cars separated, the car rolled forward taking the slack out of the chain which had the cars secured and caught the employee's left foot. He was placed on restricted duty 7/24/07.
At 1300 incline station, employee was changing rollers from LHD bucket. LHD operator prepared to dismount and install chocks. LHD rolled several feet striking a Kubota which struck the employee. Taken to doctor initially, doctor found only bruising. After employees normal 5 days off, went to another doctor who gave restricted duty. Restricted duty started 4/20/07
EE stated that while troubleshooting the forklift begam to tram. It spun to the left catching his foot between the forklifts frame & ground. His first lost work day did not occur until 4/27/06.
Employee stated that he was moving a pile of "gob". A 3 inch pipe was in gob & was caught in a bind. The pipe turned loose & struck the employee in the jaw.
EMPLOYEE STATED THAT HE WAS OPERATING PARTS JEEP ON L-10 TRACK DELIVERING PARTS TO THE LONGWALL. AS HE WAS DRIVING THROUGH THE INBY DROP CURTAIN, HIS ARM GOT CAUGHT IN THE CURTAIN. HE WAS DIAGNOSED WITH A ROTATOR CUFF TEAR TO HIS RIGHT SHOULDER. HIS FLWD DID NOT OCCUR UNTIL 11/17/05.
While operating 980 loader, loader rolled over on its side and seat belt came loose, employee landed on his head and back.
Employee was operating cable peeler picking up trail cable behind a shovel. Employee had the reel too high and backed up an incline causing the reeler to tip on its side.
Returning to pit, right front wheel left haul road and struck culvert and embankment at intersection in haul road. Truck jacknifed, turned sharply to left and struck berm. Employee received laceration to left side of head, requiring stitches. Window and steering cylinder damage to haul truck.
WAS LEAVING WORK, HE WAS DRIVING HIS PERSONAL VEHICLE & DRIVING INTO THE SUN. HIS VEHICLE STRUCK A RED DOG PILE, AT A GLANCING BLOW. HE WAS WEARING HIS SEAT BELT. THERE WAS DAMAGE TO THE FRONT OF HIS CAR. HE ALSO RECEIVED A VERY MILD COMPRESSION FRACTURE TO HIS T-12 & L-5 VERTABRAES.
EMPLOYEE WAS RIDING MANTRIP TO SECTION AND HAD HIS ARM LAYING ON THE BACK OF THE SEAT WHEN THE MANTRIP WRECKED AND THE BATTERY COVER MOVED FORWARD PINCHING HIS ARM BETWEEN THE COVER AND MANTRI P FRAME RESULTING IN A LACERATION.
WHILE WORKING AT THE PREPARATION PLANT, UNDER A CONVEYER BELT, APPROX 15 FT ABOVE HIM, A ROCK WEIGHING APPROX 2 POUNDS STRUCK EE ON HIS UPPER MIDDLE BACK, WHICH CAUSED A CONTUSION.
EE WAS LOADING 777 CAT END DUMP TRK WITH A 992 LOADER(60053)AT THE PROSPECT AREA WITH ROCK & DIRT. THE EE ALLEGEDLY TWISTED & JERKED LOWER BACK WHEN A ROCK ROLLED UNDER THE LOADER FRONT TIRE WITH THE BUCKET APPROX 2/3 FULL IN THE RAISED LOADING POSITION. THE LOADER LEANED SEVERLY TO THE LEFT (SOUTH) CAUSING OPERATOR OF THE LOADER TO QUICKLY TURN THE LOADER TO THE RIGHT.
EMPLOYEE WAS DRIVING FUEL TRUCK WHEN HE HIT AN ICY PATCH OF ROAD CAUSING THE TRUCK TO GO INTO A SKID. WHILE TRUCK WAS SLIDING IT HIT A DRY SECTION OF ROAD AND THE FUEL TRUCK TURNED OVER ON ITS SIDE.
TWO EMPLOYEES WERE CAR POOLING TO WORK IN A PERSONAL PICKUP. THEY ARRIVED AT THE PUBLIC PARKING LOT AND ONE GOT OUT. THE OTHER TURNED THE CAR SHARPLY AND KNOCKED HIM DOWN. THE RIGHT REAR TIRE PICKUP KNOCKED HIM DOWN & RIGHT REAR TIRE RAN OVER HIS ANKLE & LOWER LEG CAUSING SEVERE BRUISING
EE WAS OPERATING MANBUS EXITING SECTION WHEN THEBUS WRECKED, THROWING HIM FORWARD, STRIKING HIS MOUTH ON THE CAB OF THE MANBUS. THIS RESULTED IN2 CHIPPED TEETH ON RIGHT SIDE OF HIS MOUTH.
EE WAS OPERATING A CAT 777 TRUCK AND INJURED HIS LOWER BACK WHEN HIS TRUCK WAS STRUCK BY A LOADER. HE WAS SEEN BY A PHYSICIAN THE FOLLOWING DAY AND WAS UNABLE TO REPORT TO HIS NEXT SCHEDULED S HIFT. THE LOADER STRUCK THE TRUCK WHILE LOADING.
TRUCK DRIVER DRIVING A LOADED 160 TON RIMPULL TRUCK UP A RAMP STALLED OUT WHEN IT SPUN OUT ON A MUDDY SPOT IN THE RAMP. THE TRUCK ROLLED BACKWARDS AND JACKKNIFED, CAUSING THE TRACTOR TO ROLL ON ITS SIDE. DRIVER DID NOT START MISSING WORK UNTIL 9/7/00.
WHILE EMPLOYEE AND COWORKER WERE PREPARING TO INSTALL TROLLEY BRACKETS, A TRAIN STRUCK THE BATTERY FLATCARS THEY WERE USING, CAUSING EMPLOYEE TO FALL ON HIS BACK AND STRIKE HIS ELBOW. SUUFERED A BONE CHIP ON HIS LEFT ELBOW AND HAD COMPLAINTS THE FOLLOWING DAY OF NECK AND BACK PAIN.
EMPLOYEE CLAIMS HE FELT LOWER NECK AFTER AN INCIDENT THAT OCCURED ON THE PREVIOUS SHIFT INVOLVING AN LHD LOADER FALLING INTO HOLE.
EMPLOYEE CLIMBED ONTO RAILROAD CARS TO GET A BRAKE. AFTER SETTING BRAKE-AND WHILE HE WAS STANDING ON COUPLER OF RAILROAD CARS-THE MOVING CARS COUPLED INTO STATIONARY CARS ON LOWLINE, CAUSING C OUPLER TO COMPRESS AND INJURING EMPLOYEE'S FOOT.
EE CLAIMS HE WAS IN THE PROCESS OF PULLING THE AIR DOOR OPEN WITH A CHOKER ATTACHED TO FLAT CAR, HE WAS PINNED BETWEEN DOOR AND FLAT CAR COUPLING.
EE AND ANOTHER OPERATOR WERE ASSIGNED TO PACKAGING THE MILL. EE WENT TO ADJUST THE FORKS SO THEY FIT INTO THE BAG PALLET. USING A FORKLIFT. THE OTHER OPERATOR REMAINED ON THE FORKLIFT WHEN HE THOUGHT THE OTHER OPERATOR WAS HAVING DIFFICULT THE DISMOUNTED THE FORKLIFT BRUSHING THE FORK CONTROL EVEN WITH HIS RT LEG CAUSING TO OPEN.
EE STATED HE REACHED INSIDE OF THE #1 MBC TO START THE MACHINE FROM OUTSIDE THE CAB HE ACCIDENTALLY HIT THE TRAM LEVER CAUSING THE MACHINE TO MOVE CRUSHING HIM BETWEEN THE MBC AND THE LOW FRAM E STRUCTURE.
HAUL ROAD DUSTY FROM PREP BY MOTORGRADER FOR MGCL2 APPLICAITON. PICKUP DRIVEN BY MECHANIC WAS AHEAD OF EMP. EMP DROVE INTO HEAVY DUST, OFFERING LITTLE VISIBILITY. PICKUP COLLIDED WITH RIPPER A SSEMBLY ON BACK OF MG WHICH WAS BLADING THE ROAD. WATER TRUCK WAS WATERING THIS AREA.

EE CLAIMS HE HAD DUMPED AT THE G WEST GRIZZLY HE BACKED THE LOADER FROM THE GRIZZLY AND STARTED MOVING EAST, A ROCK HAD COME OFF THE BUCKET WHEN DUMPING AND LANDED ON THE ROADWAY. HE RAN OVER THE ROCK STRAINING HIS BACK.
EE STATED HE WAS TRAMMING MOBILE BRIDGE, WAS INATTENTIVE TO DETAILS OF JOB, THE CANOPY HUNG ON A ROOF BOLT CAUSING THE MACHINE TO SUDDENLY DROP, THE CANOPY STRUCK HIM ON THE HEAD CAUSIGN NECK PAIN.
EMPLOYEE WAS BEHIND THE WING CURTAIN IN #3 ENTRY, PULLLING THE MINER CABLE BACKWARDS WHEN THE MBC STRUCK HIM CAUSING HIM TO BE LODGED BETWEEN THE DUCKBILL OF THE BRIDGE AND THE RIGHT RIB.
EE STATES HE WAS CLIMBING OUT OF SCOOP. THE BACK OF THE SEAT BROKE CAUSING HIM TO FALL OUT. HIS L KNEE CAUGHT ON CONTROLS.
