Was putting cable behind anchor. The shuttle car lost power. Went and put power back in, and the stop, start switch was stuck and the cable started taking up and hit employee on the back of the legs and flipped employee. Employee landed on left wrist and head bounced off the bottom of the mine floor.

Allegedly the hydraulics locked up on the machine causing it to raise off the ground and drop.

While edging up coal pit the loader bucket allegedly dropped off the coal and hit the pit floor

Operator was cleaning top of Powelton Coal when allegedly was stopped by a rock and was jarred.

Allegedly when backing under loader a rock was dumped on the tail shoot and jarred the driver.

Employee alleges that the boom of the loader stuck in the up position and when EE went forward the boom dropped causing the rear of loader to raise and upon the rear setting back down was jarred. Employee did not report the incident to management till 8/1/2016 and did not miss work until 8/11/16 due to the alleged injury.

Was jarred when the truck was being loaded.

A second EE backed his hauler into the hauler injured EE was sitting in waiting to get loaded. Proper safety procedures were not used in this incident. Both men said they were OK & did not need medical attention. On 12-15-15 injured EE came into work and said his back was hurting and wanted to get checked out. EE went to ER & was told to follow up with his Dr.

The hauler was on a slick incline and could not pull up the hill the hauler driver requested that the dozer give him assistance, the hauler tires were spinning and when the tires got traction, the hauler jerked, causing it to jerk the operator, hurting his back.

Employee was using a 980H loader to bust up coal, when he said he felt a sharp pain in his neck. (Back of neck).

Employee was operating a CAT-777D Hauler. He stated the bed of the truck came down & hit the frame of the truck hard, causing it to shake him.

Employee was working on a service truck while laying under it the truck rolled back and ran onto the employees arm and chest.

Employee alleges injury after being laid off at the end of his 12/5/2013 shift but never reported to management. Management was notified on 12/20/2013 by Workers Comp. Claims neck and back injury after running his rock truck into another rock truck.

Employee alleges hitting his knee on bucket lever of Loader.

Operator says he jammed neck into cab protector when he hit a dip in the road.

EE alleged that the bed of the Cat 789C hauler free fell and undestinational distance, causing an injury to his back. EE said he raised the bed of the hauler & then as he was lowering it the bed stopped, he pushed the down lever & the bed of the hauler; FREE FELL down against the frame. The hauler was checked by the mechanic & no problems were found.

Employee alleges seat bottomed out on rock truck causing back and hip pain.

IEE stated he was guilding a wire rope through the scrapper which was pulling belt through it.EE stated he had his right hand on motor and it slipped off causing his finger to get caught in belt pulley. He tried to free his right hand with his left hand and got his left (tip end) finger caught also. (index finger)

EE was using a Johnson Ride to pull a pull behind rock duster he said he was backing up and hit the Head Drive #4 said it made him hit his back against ride he was operating. EE stated foot switch stuck on him. (maint. checked foot switch and found no problems)

EE was attempting to unlatch his tailgate when the tailgate released and the truck moved backing onto the EE's foot, causing injury to his right foot.

While using a manlift to attach plywood to spirals, employee states that he unknowingly lowered the manlift boom on the corner of the opposing spiral frame. When he swung the boom out, it dropped off the frame causing the employee to hit his rib/chest area on the rail of the manlift basket. The employee received medical treatment and was placed on modified duty.

EE stated that he lost his brakes on scoop and hit rib.

EE was operating 2-man Johnson - stinger ride when he hit a hole in the bottom, causing the steering wheel to turn quickly. The ball on the steering wheel hit him on the back of his hand, breaking a bone.

EE stated he got off ride to open door and man trip (2 person) rolled and hit him in lower back. EE was checked no cuts, scratches or bruising, didn't want to go to Dr. or accident report filled out at time was let go four weeks later. Just informed us he wants accident report filled out 4/15/12.

Helping mechanics work on steering on RT#099. Mechanic was in truck and employee was at left side. As mechanic went to check steering, victim went behind left front tire and was pushed into housing. Fractured facial bones on left side and had bloody nose.

Employee was scooping the No. 1 heading when he ran over a rock, bouncing him in deck of scoop, causing injury to his back.

#4 belt slipped in the head drive, causing minor smoke, before the drive shut off. EE stated that the smell caused him to be sick to his stomach. EE worked the remainder of his shift and went to the doctor the following morning.

Hauling load in 777C and went from wet to dry conditions while coming down embankment. Appears tires caught while getting ready to dump and truck tipped over onto passenger side. EE was sent to hospital with abrasions and released 11/09/2011.

EE was climbing onto the rock truck at beginning of shift when his back popped. Later while being loaded his truck was shook by a large rock and his back popped again. Taken to emergency room, cat scan, given pain medication and off work for 2 days. Returned to work with no restrictions on 10-17-11.

The victim was operating a rock truck. When the shovel was loading the truck, a rock shook the truck and jarred the operator. He began experiencing pain in his neck area. Diagnosed as bruised cervical. Returned to work on 10/17/11 with no restrictions.

This is an alleged claim, nothing was reported to management about an accident nor an injury. It is alleged that in the process of removing a large rock with his loader he jarred himself.

He was on the man trip preparing to go underground. The man trip did not go into reverse but stuck in forward. It was parked in front of the maintenance trailer. When the man trip moved it struck the towing tongue of the trailer, the sudden stop caused him to twist forcefully. His left ankle started bothering him 4 hours later, diagnosed with Fx of lower left fibula.

Victim was driving a Caterpillar 785 rock truck and hit a hole in the road. The sudden jolt jerked his arm out straight and pulled a muscle in his right shoulder. Treated & released at the hospital. Was on light duty one day, no lost time.

Victim was hauling shot rock with a 777 Caterpillar truck. He hit a depression and it bottomed out the seat and his back started hurting. He went to Pikeville Medical Center ER later that day. Employee still off work.

EE was assisting in moving a section water pressure pump. He was securing a chain on a hook that was mounted on the ram of a scoop. As the ram retracted, the employees hand slipped behind the ram and was caught between the ram and the bucket frame. X-rays reveled in-line fractures to the right little finger and ring finger.

Drove truck on berm for 90'. Truck then went over berm & down hill. It rested 100' from top of bank.

Victim was operating a 992D loader in shot material, a rock fell off the highwall and struck the loader bucket boom, causing him to be shaken in the seat. He started having pain and burning in his neck . He worked until the 21st when he went to the doctor. They did a catscan, gave him pain medicine and a pain shot. He returned to work & no time was lost.

Employee was operating a loader when the bucket struck a solid place and bounced operator in his seat causing him to strike his head against the headrest of the seat.

EE was cleaning raw coal crusher using a walk behind BobCat. EE picked up a 5 gal. bucket of mud and strted backing up a steep grade. The BobCat moved forward the EE slipped and fell. his left foot ended up under the BobCat.

Driver was driving tandem coal truck from underground mine to prep plant on the normal haul road, when he got too close to the ditch in a curve and started spinning in the mud on the side of the road and slid into the ditch, causing the truck to lay on its side. The driver hit his head on something inside the cab, giving it a small cut.

The coal truck driver was leaving the mine pit area which had more coal accumulated than normal and the pit area was tight. The loader man directed the truck driver on where to back in, loaded him and then told him to pull forward. He pulled forward approx. 25 feet and turned over on the right side.

EE was not wearing a seat belt while loading rock into a haulback truck. As he tried to lift a rock, the teeth on the bucket slipped, causing EE to hit his head against the air conditioner. The knob on the air conditioner caused a laceration on his head that required 5 staples. He was given a full release for the following day. No lost time.

THE WATER TRUCK DRIVER WAS TAKING A LOAD OF WATER TO THE STRIP MINE ROAD WHEN HE CAME DOWN A HILL TOO FAST AND RAN INTO A SMALL STOCKPILE OF GRAVEL. HE THEN BACKED UP AND CONTINUED ON TO PUT THE LOAD OF WATER ON THE ROAD.

EE stated that while the 777 Truck he was operating was being loaded, the 9926 Loader bucket struck the truck and jarred his back. EE did not miss work until 4/20/09 and returned to work on 4/23/09.

A 992 Endloader bucket was being lowered to dump load in a 777 rock truck. During this action the boom began to free fall striking rock truck.

Employee alleged that he was lifting a rock with a 992G loader when the rock slipped out of the bucket causing the loader to rock throwing him out of his seat hurting his back. Dr. diagnosed strained back. Seat belt was in good working condition but was not being used.

Employee was operating a Cat 777 rock truck. While being loaded, the end loader operator dropped a rock out of the loader bucket. This rock hit the bed of the truck and jarred the employee.

End loader bucket struck the side of rock truck bed while truck was being loaded. Note: Injured did not seek medical treatment until 2/13/09.

Employees foot was caught between the metal frame of a airlock door and a rubber tired pers. carrier.
