A supply trip left the 60# spur at Sugar Run, and was moving towards the slope. The injured was moving mantrip to switch out with two other motors, when the supply cars came uncoupled and struck the injured's mantrip. The employee did not sustain any broken bones, but is off work to see a neck specialist.

Injured EE was operating a shuttle car on the 3W CM Section. As EE was driving into the #3 entry they hit a "pot hole" causing the shuttle car to bounce & the seat became dislodged from the mounting bracket. EE fell onto the floor of the operator's deck. Suffered contusions & a shoulder & neck/back strain. Employee has training & experience and was wearing required PPE.

Employee was hanging 350 MCM cable to the mine roof in the 7th North set-up. A Wagner 25X was being used to lift the cable. The employee inadvertently placed EE's right hand on top of a pivot point resulting in EE's fingers being smashed. Laceration requiring stitches.

Injured employee was operating a diesel locomotive on the 3 West Haulage pulling 5 empty shield carriers and 1 rockdust supply car when they lost control of the trip and began sliding. Slid down the 3 West Haulage to the 8 blk. track spur on the 4 West Haulage where it derailed the locomotive and trip and hit the corner of the coal rib straining neck/back.

IE was standing at the edge of a crosscut when a scoop was passing. The scoop caught a feeder cable causing it and an emulsion line to fall striking the IE in the head. IE was taken to the hospital and released the same morning. MSHA hotline was immediately called. MSHA inspector investigated the incident.

Injured employee was driving a 745 Haul Truck on a haul road from the Prep. Plant to the slurry cell when EE hit a rut in the road and felt pain in neck. Injured employee continued to work for 4 months but sought medical attention on 8-31-20 and began missing work at that time due to this injury.

Injured employee unexpectedly drove a diesel mantrip into a track spur and collided with a parked mantrip. The spur switch was in the open position and employee admittedly failed to check the alignment indicator as EE approached the switch. Also failed to slow down for the switch as EE approached. Employee suffered 2 broken ribs as a result of EE's collision.

IE was running a rock truck. IE went over a rut and bounced up hitting head on the top of the truck. IE finished shift and waited until 3-31-2020 to be checked out by a Dr.

IE was operating a locomotive and derailed at a switch. IE bumped an elbow against a fire extinguisher. IE decided to seek medical attention. IE did not report back to work for 2 days.

IE was traveling up the track with a locomotive pulling a trash car. As the IE crossed a track switch the curved rail broke and came up into the trash car. The sudden stop caused the IE to bump left hand and knee inside the operators compartment. The IE was brought out and went to the doctor to be checked out.

EE was unspooling 2" plastic slick line. The scoop started pulling the line to stretch it out, the end whipped off the spool and struck the EE in left leg. The EE worked until 2/20/20 until EE went back to the ER and was given off until 2/24/20.

A fully trained miner was shoveling under the belt when the shovel came into contact with the belt causing it to come back and hit the miner in the cheek. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was tramming a scoop when the scoop hit the coal rib and jarred EE's back. The injury was not the result of any compliance issues nor were any protective guards involved.

IE went to check on the hyd.functions of a belt clamp for the slope belt. Noticed a lump of coal at the clamping part and went to push it away when the clamps set. EE got the tip of middle finger pinched. Cause of this injury was safety awareness of this employee. Compliance with rules/regs,PPE,mining systems not a factor, miner proficiency may be a factor.

Employee was looking for a damaged top roller that EE was going to replace during a belt idle period. EE's shirt sleeve was caught by a top roller and EE's arm was pulled between the top belt and a roller causing abrasions to EE's left arm. It did not include any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

A fully trained miner was attempting to pull cable slack with a loader. The miner instructed the loader operator to pull forward as miner was standing behind the machine. Miner had their arm to the left of the boom and when the machine moved forward it went to the right and the boom caught miner's arm between the boom and rib.

Employee was operating a rubber tired mantrip when the front wheel caught a track tie causing the steering wheel to spin quickly causing a pain in EE's right shoulder.

A fully trained UMWA miner was operating a mantrip when the jack walker bled off causing the mantrip to abruptly come to a stop. The passenger of the mantrip was jarred into the passenger compartment. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee was operating a scoop loaded with a LW shield. EE lowered the bill but failed to release the winch. This caused the shield tip to raise. The shield tip caught the scoop canopy, caused it to raise and slide off the side of the scoop. The light caught the employee's hardhat forcing their head down and backwards. No compliance, PPE or protective device issues.

Employee was operating a 2 man personnel carrier when the axle broke causing the employee to lose control of the vehicle causing it to wreck. The employee suffered from a laceration and contusions.

IE was driving a manbus around the corner from EM track to the MN 4 track. As IE rounded the corner 2 locomotives with a shield on a carrier started sliding outby. As IE tried to drive outby away from the locomotives, IE rounded the corner, the 2 locomotives with the shield on the carrier collided into the manbus IE was driving.

IE was on a manbus coming around the corner off of EM track onto MN4 track when 2 locomotives with a shield loaded on a trailer lost traction and started sliding outby towards the manbus IE was on. The manbus headed outby but as they entered into the turn, the locomotives and shield on the carrier collided into the manbus.

The Employee was at the end of the rail on the 1 West Tailgate section. EE was in the process of tramming the scoop down the track entry, outby 2blks to load supplies. The scoop tires slid off the rail causing the scoop to slam down on the rail, and strain EE's neck. The employee was complying with all laws, company policies, and was wearing all appropriate PPE.

A fully trained miner was running the conveyor of EE's shuttle car when a rock bounced out of the conveyor into the operator compartment and struck EE in the head. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was filling a stone car at the stone dump when a piece of stone bounced off the car and struck EE above the right eye. The injury was not the result of any compliance issues nor were any protective guards involved.

IE was operating the trailing locomotive traveling down the MN4 track. As the lead locomotive went into the turn & derailed which caused IE's loco to wreck as well. IE's body slid against the wall of the compartment of the loco & hit hip against IE's SCSR. IE was on vacation the next week & didn't miss any work until 7/12/19 due to this incident.

IE hit head in canopy of scoop when they trammed it over obstacle in roadway.

Employee was tramming scoop, while turning scoop employee caught finger between stop block of machine and operators kitchen. Cause of this incident was lack of employee concentration. PPE was not a factor, mining equipment was a factor, mining systems was not a factor, compliance with rules and regulations was not a factor, miner proficiency was a factor.

A fully trained miner was a passenger in a mantrip that was struck by another mantrip that slid into them.

Employee was operating a scoop that had traveled over top of rock laying in tram road. When scoop came off of rock it bounced the operator in the operator's compartment causing EE to hit head on the top of the operator's compartment. Compliance with rules/reg, PPE, mining equipment/systems not a factor. Miner proficiency may be a factor.

EE driving a 7 person diesel vehicle on the 6 North Track. Communicating with another operator of a 4 person diesel vehicle outby to drive toward each other to talk. EE parked vehicle at 45 blk to allow the other to approach. As the outby vehicle approached and began to slow, it slid into the parked vehicle causing EE to bump their knee against the throttle cylinder.

The employee was sitting inside the cab of a company motor 4B when another motor and dust pod car became uncoupled rolling back and striking employee's motor. When the cars collided an object flew off one of the cars striking the employee on the right side of head causing a laceration and a concussion. The employee was wearing all the appropriate PPE.

IE was riding on a track mounted manbus off of the section at the end of the shift. As they came up to a switch, there was a locomotive sitting in the track trying to get into the switch. As the operator tried to stop, the manbus made contact with the locomotive & jarred the manbus. IE's ribs struck the side plate & bruised ribs. IE is supposed to return to work on 5/15/19.

A fully trained miner was getting a dust hose off a scoop when the bucket was lowered and EE's foot was caught under the bucket. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was injured when they were operating a continuous miner. A rock came out of the conveyor landing on their right leg and breaking it in two places. The accident was not the result of any compliance issues nor were any protective guards involved.

As IE was attempting to remove a dropped bottom cradle from underneath the belt, the cradle caught the belt and struck IE in chest.

Employee was tramming scoop out of x-cut when EE encountered a ledge that cause the scoop to drop resulting in EE jamming EE's neck. Cause of this incident was EE not paying attention to surroundings. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor, miner proficiency may have been a factor.

A fully trained miner was a passenger in a rail mounted mantrip when the operator slid on wet rail and struck another mantrip causing the passenger to hit eye on the metal frame resulting in a laceration. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee was rock dusting along the belt, near the transfer, when a rock bounced off the belt and struck employee in the lower left leg. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. The employee began missing work due to the injury on 04/10/2019.

A fully trained miner was struck in the nose with a hook when the hook came free from its anchor point on a supply car. The injury was not the result of any compliance issues nor were any protective guards involved.

The employee was traveling on 9 South Mainline Haulage in #70 7man diesel mantrip. The mantrip struck a broken rail at 146block 9 south haulage, causing the employee to strike multiple parts of body against the mantrip, neck, back, knee, and shoulders. The employee didn't violate any laws or company polices, and was properly trained.

A fully trained miner was a passenger on a rail mounted mantrip when EE claims that EE's neck was jarred. EE worked 3/4/2019 and on 3/5/2019 decided to seek medical treatment. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was operating a diesel motor when ee struck a supply car causing ee to lunge into the side of the operator's compartment. The injury was not the result of any compliance issues nor were any protective guards involved.

EE was using handle on coupler to unhook motor from shield car, due to slack in the rope the cars inadvertently drifted ahead coming into contact with EE left foot. Cause of this incident was lack of EE concentration. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency was a factor.

A fully trained miner was driving a supply motor when ee hit head on a roof bolt. The injury was not the result of any compliance issues nor were any protective guards involved.

EE was laying coal onto a rough area of stockpile, when they started to bounce and failed to maintain control of the Pan. As they bounced they struck their head on the steering wheel, causing a laceration and a fracture to the nose. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

A fully trained miner was riding in a man trip when it went into a switch and hit another man trip causing EE to jar back. The injury was not the result of any compliance issues nor were any protective guards involved.

The employee was riding in a mantrip along 9 South Haulage. The mantrip picked 16D switch, went into the spur and struck a parked ride before stopping. The employee was thrown back against the seat and struck neck area on the SCSR storage box. The employee didn't violate and law or company polices, and was wearing all the appropriate PPE.

Employee was positioned back in a crosscut waiting for locomotives to pass. When the locomotive passed, it caught a power cable causing the cable to strike the employee on the left side of the face. The injury resulted in a fracture to the left orbital bone. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency was not a factor.

The ee was operating a CAT 740B Articulating truck when the bottom center pin sheared off causing the cab of the truck to tilt forward jolting the operator. The ee stated ee had pain in lower back and left leg numbness. The ee did not violate any laws or company policies and was wearing seat belt. The ee pre-op-ed equipment prior to operation.
