Employee was being loaded by loader when the loader dropped a large rock into the truck. The rock jarred employee, injuring shoulder.
Employee was driving mantrip when tire slipped and lost traction. Backed down to set over, brake line busted. No way to stop & control speed so, employee hit arches to keep from going all the way down the hill. Struck left shoulder. Waited on Dr's report before filing. First day of disability was 06/17/04.
EE WAS TROUBLESHOOTING ALT. SYSTEM ON A WATER TRUCK. TWO OF HER FINGERS CAME IN CONTACT WITH THE BELT AND SHEAVE, CAUSING LACERATIONS AND HAIRLINE FRACTURE.
Injured driving Mac 8, hit rock and lost grip of steering wheel. The knob spun around striking left hand.
Mantrip was struck in belt entry near feeder. as coal hauler approached feeder to discharge load of coal, hauler operator did not see mantrip. Tailgate of hauler made contact with mantrip causing injury to rt foot/toes when they were caught between two pieces of equipment. Injured was a passenger in mantrip.
2 CRANES & WINCH TRUCK HAD TURNED HAUL TRUCK BED UPSIDE DOWN, THEN 2 CRANES LOADED THE OVERTURNED BOX ON FLATBED TRAILER. INJURED EE THEN WAS CHECKING TO SEE IF THE TRUCK BED WAS CENTERED PROPERLY ON TRAILER. A LODGED PIECE OF EARTHEN MATERIAL CAME LOOSE COMING DOWN & STRIKING EE.
EE WAS UNDER WATER TRUCK REMOVING THE DRIVE SHAFT WHEN A LOADER BACKED INTO THE REAR OF WATER TRUCK, PUSHING IT, CATCHING HIS SHOULDER & BACK ON REAR DIFFERENTIAL
EMPLOYEE STATES THE SHUTTEL CAR SLID DOWN INTO LOW RIB AND PINNED HIM BETWEEN THE SHUTTLE CAR BUMPER AND RIB. EMPLOYEE RECEIVED FRACTURED ANKLE AND REQUIRED SURGERY.
EE STATES HE STRAINED HIS NECK FROM BOUNCING UP AND HITTING HIS HEAD AGAINST THE CAB OF HIS SHUTTLE CAR.
THERE WAS A 15 TON MOTOR PARKED INBY MANTRIP. EMPLOYEE GOT IN MANTRIP, PULLED IT DOWN 12 BRKS AND SLOWED DOWN FOR ANOTHER WRECKED MANTRIP. MOTOR ROLLED OFF 12 BRKS AND STRUCK MANBUS, BRUISING RIBS OF EMPLOYEE.
EMPLOYEE WAS RUNNING CAR WHEN SHE ALLEDGED THAT SHE HIT A HOLE IN THE ROAD WHILE HAULING COAL. FELT PAIN IN LOWER BACK. ROADWAYS ARE MUDDY ON THIS UNIT.
EE WAS CLEANING THE ENTRY AT THE SEALS WITH A SCOOP. PIECE OF 4"PLASTIC WATER LINE FLIPPED UP INTO THE CAB AND STRUCK RIGHT RIB CAGE. FRACTURE OF 7TH & 8TH RIB.
WAS STANDING ON SCOOP BUCKET DUCKBILL TYPE SHOVELING OUT LOOSE MATERIAL. HE SLIPPED ONT HE WET METAL SURFACE AND TWISTED HIS KNEE LEFT. BEGAN TO LOSE TIME ON 11-7-03.
RIDING AS A PASSENGER IN A 9 TON GOODMAN MANTRIP AS THE MANTRIP WAS BEING OPERATED INTO A SWITCH THE OUTBY WHEELS STARTED TO CLIMB ON THE GUARD RAIL. THE MOTOR CAME DOWN ON THE RAIL AND THE IN JURED WAS JOSTLED, STRAINING HIS NECK.
UNIT SUPERVISOR WAS MARKING FACE. SCOOP OPR. WAS ASSIGNED TO CLEAN #5 ENTRY. OPR WENT THRU CURTAIN 80'OUTBY ACCIDENT AREA & WAS PREPARING TO DUMP BUCKET LOAD AT FACE OF #5 WHEN HE HEARD SOMEON E YELL. OPR IMMEDIATELY BACKED UP WHEN HE DISCOVERED HE HAD STRUCK SOMEONE W/BUCKET. SUPERVISOR SUSTAINED INJURIES TO BOTH LEGS BELOW THE KNEES.
WHILE OPERATING A JEEP ON THE HAULAGE THE JEEP DE-RAILED. EE WAS THROWN AGAINST THE SANDING DEVICE HANDLE BRUISING HIS RIBS. HE CONTINUED TO WORK UNTIL 10/14/03 WHEN HE WAS TAKEN OFF WORK FOR SEVERE BACK SPASMS. IT IS BELEIVED THAT HE MAY HAVE A CONPRESSION FRACTURE OF A VERTABRAE.
EMPLOYEE WAS TAKING MANTRIP INTO MDR UNIT WHEN HE HIT AN UNDERPASS AT MOUTH OF UNIT WITH HEAD. INJURIES TO NECK, BACK AND HEAD. HE DID NOT LOSE ANY TIME UNTIL 1/20/40 FOR SURGERY.
HIT A HOLE IN ROADWAY WITH S/C AND HIS LOWER BACK WAS STRUCK BY A PROTRUDING PIECE OF METAL IN HIS S/C SEAT.
EE WAS STANDING APPROX 6' FROM RIDE PUTTING TOOLS IN BOX AT THE END OF SHIFT. ANOTHER EE SAT DOWN ON RIDE, SAID THAT THE RIDE JUMPED BACKWARDS HITTING INJURED IN THE BACK OF HIS LEFT KNEE.
EE STATES HE WAS OPERATING A SCOOP. HE HIT A POTHOLE, BOUNCED AND HIT HIS HEAD ON THE TOP OF THE CAB CAUSING PAIN TO HIS NECK AND UPPER BACK. EE REQUESTED MEDICAL TREATMENT ON 6/9/03 AND WAS P LACED ON RESTRICTED WOTK ACTIVITY FOR A COUPLE OF WEEKS.
WAS RIDING IN A JEEP, WHEN JEEP WENT INTO SWITCH IT DERAILED AND INJURED BRUISED HIS RIBS.
EE WAS OPERATING A 10 JOY S/C. WHILE BACKING UP THE S/C THE EE HAD HIS LEFT HAND ON THE OUTSIDE OF THE STEEL CANOPY. HIS LEFT HAND WAS CAUGHT BETWEEN THE STEEL CANOPY AND THE COAL RIB. THIS AC CIDENT RESULTED IN A BROKEN LITTLE FINGER AND A LACERATION TO THE INSIDE OF THE LEFT HAND.
EE SAID THAT HE HIT A HOLE AND BROKE HIS LIGHT AND HAT.
A FOREMAN WAS HOLDING A TRANSITION TUBE SO THE SCOOP OPERATOR COULD TRAM UNDER IT AND THEN HE COULD DROP IT INTO SCOOP BUCKET. THE SCOOP BUCKET HIT ONE OF THE TUBE ENDS AND THE FOREMAN S LEFT HAND WAS CAUGHT BETWEEN THE TUBE AND THE FAN.
EMPLOYEE HIT HIS HEAD ON THE ROOF OF THE SHUTTLE CAR WHEN IT HIT A BUMP AND HE JAMMED HIS NECK.
EE WAS RIDING COAL HAULER TO END OF TRACK, HIT A HOLE AND TWISTED HIS BACK.
UTILITY MAN WAS DRIVING A PICK-UP TRUCK WHEN THE FRONT WHEELS FELL INTO A HOLE CREATED BY AN UNDERGROUND PIPELINE WATER LEAK. HIS HARD HAT APPARENTLY STRUCK HIS HEAD LACERATING HIS SCALP.
EMPLOYEE WAS WALKING ON CATWALK IN CLEAN COAL SILO. A NORFOLK/SOUTHERN TRAIN LOCOMOTIVE WAS LOCATED IN THE SILO. WHEN THE TRAIN PREPARED TO MOVE THE TRAIN ENGINEER BLEW HIS WHISTLE. THE SOUND OF THE WHISTLE CAUSED EAR RINGING (TINNUITIS) FOR THE EMPLOYEE. HE WENT OFF WORK ON 1/21/03 FOR TESTING ETC.
ATTEMPTING TO SLOW DOWN BELT/SHEAVE ASSEMBLY, BELT CAUGHT GLOVE PULLED FINGERS BETWEEN BELT AND SHEAVE.
THE INJURED WAS RIDING A MANBUS. THE BUS WAS HAVING TROUBLE PULLING A STEEP INCLINE. ANOTHER BUS WAS BROUGHT TO THE SCENE TO AID THE TROUBLED BUS UP THE HILL. WHILE ON THE INCLINE THE TROUBLED BUS ROLLED BACKWARD AND STRUCK THE SUMMONED BUS. THE INJURED SAW THAT THE 2 BUSES WERE GOING TO HIT. THE BATTERY SLID & CAUGHT FINGERS BETWEEN.
INJURED WAS RIDING A MANBUS WHICH WAS HAVING TROUBLE PULLING A STEEP INCLINE. ANOTHER BUS WAS BROUGHT IN TO AID THE TROUBLED BUS. WHILE ON THE INCLINE THE TROUBLED BUS ROLLED BACKWARDS & STRUC K THE SUMMONED BUS. THE INJURED SAW THAT THEY WERE GOING TO HIT & EXTENDED HIS HAND TO BRACE FOR IMPACT. WHEN THE BUSES HIT THE BATTERY FOR THE BUS SLID & CAUGHT HIS FINGER.
INJURED WAS LOADING SHUTTLE CAR WHEN A PIECE OF ROCK WAS THROWN FROM CONVEYOR CHAIN OF MINER. ROCK STRUCK HIM ON LEFT CHEEK (FACE).
THE EE WAS ATTEMPTING TO GET OUT OF A STALLED MANBUS. THE MANBUS WAS STUCK BY A MOTOR. HE REPORTED HIS BACK BEGAN HURTING BECAUSE OF THE INCIDENT.
DRIVING MAC 8. STEERING WHEEL SLIPPED OUT OF HAND HITTING BACK OF HAND W/KNOB.
LEAVING AREA, WITH MANTRIP IN REVERSE, HIS FOOT WAS OUTSIDE MANTRIP. AREA WAS LOW W/LIMITED VISIBILITY. THE FOOT WAS PINCHED BETWEEN MANTRIP & CRIBS. SEVERE CONTUSIONS TO FOOT. EE RETURNED TO WORK. ADMITTED TO HOSPITAL FOR POSSIBLE BLOOD CLOTS/INFECTION. FIRST DAY LOST 08/12/02.
EE WAS RUNNING THE TAIL MOTOR OF A TRIP THAT CONSISTED OF A FLAT OF OIL & LW EMERGENCY SUPPLY SHED. PASSING THROUGH A RUN-THROUGH CHECK, ONE OF THE "J" CHANNELS ON THE SLED CAUGHT THE PLASTIC ON THE RUN-THROUGH CHECK CAUSING IT TO BACKSLIDETOWARDS THE TAIL MOTOR. INJURED SAW IT COMING & PUT OUT HIS LEFT HAND TO PUSH IT AWAY FROM HIM. THE "J" CHANNEL CUT HIM UNDER THE LEFT BICEP.
INJURED WAS RIDING ON 3-WHEELER & HIT HEAD ON MINE ROOF. THIS WAS ORIGINALLY NLT. EE BEGAN MISSING WORK ON 10/09/02.
A TRESPASSER WAS APPARENTLY ATTEMPTING TO RIDE A NORFOLK SOUTHERN TRAIN BY GETTING ONTO A COAL CAR ON A RIGHT-OP-WAY SIDING THROUGH EMERALD PROPERTY. HE WAS KILLED WHEN HE FELL UNDER COAL CAR S. THIS IS NOT A MINE ACCIDENT AND IS BEING SUBMITTED ONLY FOR INFORMATION PURPOSES. THIS IS NOTCHARGEABLE.
AS HE WAS COMING OFF THE FEEDER WITH THE SHUTTLE CAR, THE EMPLOYEE HIT A HOLE & STRUCK HIS HEAD AGAINST THE CAR'S CANOPY. HE BEGAN MISSING TIME ON 5/16/02 (REPORT LATE WAITING FOR DR. REPORT).
INJURED WAS PASSENGER IN MANTRIP WHEN MANTIP WAS BACKING OUT OF A CROSSCUT. TIRE WENT INTO A DITCH AT A CULVERT, CAUSING INJURED;S HEAD TO STRIKE CAB. HE AGGRAVATED AN OLD NECK INJURY, HE IS N OW ON RESTRICTED DUTY.
WAS STANDING GUIDING A SCOOP OPERATOR PICKING UP A LIFT OF BLOCKS. WHEN THE SCOOP OPERATOR RAISED THE LIFT THE FORK PACKAGE CAUGHT A HOSE THAT WAS HANGING DOWN ACROSS THE ENTRY. WHEN THE SCOO P MOVED THE HOSE SPRUNG FREE AND STRUCK THE INJURED ACROSS THE FACE BRUISING HIS FACE.
PARKED SCOOP ON HILLSIDE, GOT OUT TO LOAD CANOPY, SCOOP DRIFTED CAUSING EMPLOYEE TO BE STRUCK BY CAOPIES IN SCOOP BUCKET.
TRUCK WAS BROKEN DOWN AT BASE OF HAUL ROAD WITH TRUCKS STOPPED WAITING. WHEN OPERATOR APPLIED BRAKES THE TRUCK DIDN'T SLOW SO HE PULLED THE PARK BRAKE. WHEN THIS FAILED TO STOP THE TRUCK HE WE NT ALL THE WAY UP AN ESCAPE RAMP. AFTER A FEW SECONDS THE TRUCK TURNED ONTO THE DRIVER'S SIDE. THIS RESULTED IN MINOR BUMPS AND BRUISES TO THE DRIVER.
EE WAS HELPING UNCOUPLE FLAT-CATS ONE OF THE CARS WHICH ARE VERY LOW. SLID OVER HIS FOOT. HE WAS OF WORK 2 DAYS AND RETURNED TO WORK AND WORKED UNTIL 1-17-02. HE THEN WENT TO THE HOSPITAL WHER E IT WAS DETERMINED HE HAD A FRACTURE HE STARTEDMISSING WORK THEN. I WAS NOT INFORMED ABOUT THIS ACCIDENT UNTIL 1-28-02 THIS IS WHY THE REPORT IS LATE.
PUSHING A ONE INCH WASH DOWN HOSE BETWEEN A TOP ROLLER AND BELT STRUCTURE FRAME DECK FROM THE WIDE SIDE TOWARD THE TIGHT SIDE. HIS SLEEVE CAUGHT IN THE OPERATING BELT ROLLER AND PULLED HIS ARM IN. HE WAS ABLE TO PULL HIS ARM OUT. A BONE IN HIS LEFT ARM WAS FRACTURED AND HIS LEFT ELBOW WAS DISLOCATED.
HAD LOADED 10' TIMBER INTO UNILOADER WHEN A SCOOP CAME BY TO PASS AND HIT TIMBER PUSHING LEFT ARM AND WRIST BETWEEN TIMBER & UNILOADER. THE SCOOP HAD HONKED BY HE DIDN'T GET OUT OF THE WAY IN TIME.
EE WAS OPERATING LOCOMOTIVE ALONG 5 SOUTH TRACK WAY. WHILE TRAMMING THE LOCOMOTIVE, IT BOUNCED AND THE CANOPY ON THE LOCOMOTIVE STRUCK A STEEL BEAM THAT WAS OVER THE TRACKWAY. THE OPERATOR STR UCK THE CANOPY AND CAUSED A LACERATION TO HIS HEAD REQUIRING STITCHES TO CLOSE.
TWO PREP PLANT MECHANICS WERE CHANGING A MOTOR BASE. THEY HAD THE MOTOR RAISED USING A 5 TON HOIST & A 1 TON HOIST. AS THEY WERE ALIGNING THE BASE WITH THE MOTOR MOUNTS USING BOLTS TO ALIGN TH E HOLES THE CABLE IN THE 5 TON HOIST SLIPPED APPROXIMATELY 2". THE MECHANIC'S FINGERS WERE PINCHED BY THE BOLT & BASE.
