Employee was leveling a tail pulley by placing shims under one foot to level it while another employee lifted the foot with a skid steer bucket. The skid steer operator lowered the foot pinching the employees thumb between the foot and wood blocking. This fractured the employees thumb.

Mostrando los 40Employee was driving a lube truck in a tramway underground. They hit a bump causing the top of their hard hat to hit the FOPS of the cab. This caused the employee to have neck pain. The employee was taken to the clinic and diagnosed with a cervical strain.
Injured employee was front seat passenger in a four seat UTV personnel vehicle. The driver turned too wide and tipped the UTV over on the driver's side. Injured employee reached across chest with left hand to brace for impact, injuring left shoulder. Did not become recordable accident until 8/23/16
Employee was drilling in 4 tunnel when another employee pulled up in a skid steer to discuss where to clean next. He was sent to 5 tunnel. As the employee turned the skid steer to exit 4 tunnel, the skid steer boom pinched the other employees ankle in between the two pieces of equipment.
The employee was attempting to close the width of the forks on a 4 wheel drive Gradall Material Handler. He placed his left hand into the pinch point behind the fork and his right hand slipped off the fork. His left middle finger tip was crushed in between the fork and the mast.
Employee was tramming the 980 loader to the work area and reached down to adjust the seat. He over-reached and his finger went inside the seat suspension. Hitting another bump, the weight of the seat along with his body weight, collapsed the suspension causing it to pinch the middle finger on his right hand.
Employee was negotiating a corner in a John Deere Gator on a slope underground and the right tires climbed the berm guarding the right rib after turning too short. The Gator turned over on it's side and the employee's left leg was injured in the accident.
Around 6:25 the driver side rear wheel of the gator became dislodged causing the vehicle to turn sharply to the left. The gator struck the rib and ejected the employee, with his right thigh striking the vehicle and fracturing his femur.
While exiting the loader the employee tweaked his knee. That night his knee swelled up and he began to experience pain. He was taken to the doctor the next day. No restrictions were placed on the employee, but he was referred to a specialist for further diagnosis and an MRI which found he had a torn meniscus. The employee was placed on restricted duty as of 4/16/10.
Employee ran a John Deer Gator XUV into a MTI truck. Employee's face struck the ROPS on the Gator and sustained lacerations above and below right eye, busted teeth out of mouth, punctured right cheek and fractured portions of his upper jaw.
Employee had fueled LHD, stepped off LHD onto back bumper of fuel truck, and signaled the LHD operator to leave. As LHD pulled away, injured employee fell between tires. Left forearm was caught under front tire.
Employee driving mine tractor turned into vehicle passing him. Following day employee complained about discomfort in shoulder and lower back. Employee examined by Dr and given non-prescription medication. Employee returned to full duty with recomendation to avoid any heavy lifting for a week. *possible over-reporting (RTW with 5 days restricted)*
Miner was operating LHD on first shift. As he turned into an intersection to approach the muck pile he ran over a large piece of salt on the ground. The jolt of hitting the large chunk caused him to bounce out of his seat and contact the fire suppression monitor box located above his head. The monitoring box cut his scalp about 4" long and 1/4" deep. He received 10 stitches.
19 year employee, scooptram operator, alleges banging head on canopy numerous times throughout career resulting in cumulatiuve trauma to the neck.
While Cargill employee was loading a flatbed truck in the yard, the truck driver moved behind the forklift, causing the steering tire to run over both feet, when the forklift backed up. No fractures. **Truck driver was employed by a customer, not contracting for Cargill.**
(2 EMPLOYEES INJURED IN THIS ACCIDENT.) TWO EMPLOYEES WERE HANGING BRATTICE FROM A LIFT TRUCK. A FITTING BROKE ON THE BASKET LEVELING VALVE CAUSING THE BASKET TO TIP FORWARD UNEXPECTEDLY. THIS EE HUNG IN BASKET, THEN DROPPED TO GROUND. (THE COWORKER FELL 8' TO THE GROUND.)
(2 EMPLOYEES INJURED IN THIS ACCIDENT.) TWO EMPLOYEES WERE HANGING BRATTICE FROM A LIFT TRUCK. A FITTING BROKE ON THE BASKET LEVELING VALVE CAUSING THE BASKET TO TIP FORWARD UNEXPECTEDLY. THIS EE FELL 8' TO THE GROUND. (THE COWORKER HUNG IN BASKET, THEN DROPPED TO GROUND.)
LOADER EE WAS OPERATING FOR CLEANUP AROUND MUDBALL REJECT PUMP TANK BECAMSE STUCK & WHEN IT WAS PULLED OUT THE FRONT TIRES DROPPED INTO RUTS MADE BY REAR TIRES, JARRING OPREATOR INSIDE THE CAB. EE PLACED ON RESTRICTED DUTY 4/29/04
INCIDENT WAS REPORTED ON 11-19-03. THE EE WAS OPERATING A SKID STEER TO MUCK IN A HOLE. HE STARTED DIGGING AND THE HYDRAULICS GAVE WAY AND THE KID STEER BUCKED HIM. AS THIS HAPPENED, HE WENT F ORWARD AND TWISTED AND HIT HIS KNEE AT THE SAME TIME.
EE WAS DRIVING IN #769 ALONG 8-WEST BELT AND HIT A BUMP IN ROADWAY, CAUSING A WASHBOARDING EFFECT AND LOSS OF CONTROL OF GATOR SENDING IT INTO THE CONVEYOR STRUCTURE.
FORKLIFT OPERATORS HARD HAT FELL OVER HIS EYES. WHEN ADJUSTING IT WHILE MOVING HE FOUND HIMSELF LEFT OF CENTER, HE TRIED TO AVOID CONCRETE FOOTER OF STEEL PILLAR. BY THE TIME HE APPLIED HIS BRAKE, HE WAS UP ON THE BASE AND SLOWLY ROLLED THE FORKLIFT OVER.
EMPLOYEE WAS OPERATING FRONT END LOADER AND WAS THROWN AGAINST THE FRONT WINDOW GLASS WHEN THE FORKS OF THE LOADER HIT THE GROUND AND DUG IN CAUSING THE LOADER TO ABRUPTLY STOP. EMPLOYEE WAS K NOCKED UNCONSCIOUS AS A RESULT OF THE BLOW TO THE HEAD. EE WAS WEARING SEAT BELT.
DRIVING DOWN HAUL ROAD, EQUIP HIT BUMP, BOUNCED HIM UP AND JAMMED HIS ELBOW ON ARM REST, DISLOCATED LEFT SHOULDER.
EE WAS INSTALLING A 6"X20' STEEL CONVEYOR BELT LINE STRUCTURE. ONE END FELL OF FORKS OF FORKLIFT. EE STRADDED THE STRUCTURE AND LIFTED IT BACK ONTO FORKS THE FORKLIFT OPERATOR RAISED THE FORKS BEFORE EE WAS CLEAR, STRIKING EE IN GROIN.
EE STOOD TOO CLOSE TO MOVING VEHICLE & HAD RIGHTFOOT BROKEN BY REAR WHEEL.
EE WAS SITTING IN THE INTERSECTION OF 45P & 44E ON THE GROUND FACINT SOUTH. EE WAS INSTALLING SURVEYING CENTER PINS. THE SCALER WAS TRAVELING SOUTH DOWN 45P WITH ALL EIGHT LIGHTS ON. SCALER OP ERATOR FAILED TO SEE SURVEYOR AND STRUCK HIM IN THE BACK, FOLDING HIM OVER AND UNDER THE SCALER. EE SUFFERED FRACTURE LEFT & RIGHT FEMUR, LEFT TIBIA & FIBULA, LEFT ANKLE.
EE WAS CLEANING TAIL PULLEY AREA OF ONE CONVEYORAND A CHUNK OF SALT FELL OFF OF ANOTHER CONVEYORBELT, STRIKING HER RIGHT SHOULDER.
EE WAS DRIVING AWAY FROM QUARRY PAD AFTER UNLOADING SALT. HE SAID HE WAS NEAR THE BLACKTOP PLANTWHEN HE REACHED TO HIS RT. TO TURN ON WIPERS. HEDROVE OVER A BUMP PUSHING HIM INTO THE STEERING WHEEL. HE HIT HIS LOWER RT. RIB WHICH KNOCKED THE WIND OUT OF HIM. HE WAS NOT WEARING HIS SEAT BELT.
21 MILL CONVEYOR STOPPED WHEN SPILLAGE WAS CARRIED ON THE RETURN SIDE OF THE BELT, TO THE TAIL PULLEY UNTIL THE PULLEY WAS CHOKED WITH SALT. THIS WOULD HAVE PUT TENSION ON THE TOP BELT. WITH P OWER LOCKED AND TAGGED OUT AND GUARDS REMOVED, EMPLOYEE WAS INVESTIGATING THE TAIL PULLEY WHEN THE TENSION RELEASED, AND RETURN SIDE OF BELT TRAVELED INTO PULLEY, PULLING LEFT ARM BETWEEN BEL
CREW TRUCK WAS ASCENDING #1 SLOPE, WITH CREWS IN BACK. EMPLOYEE INJURED WAS HOLDING ONTO ROLLBAR WITH RIGHT HAND. WHEN TRUCK REACHED TOP AND ONTO FLAT SURFACE, TRUCK TRANSMISSION MALFUNCTIONED CAUSING ABRUPT STOP. OTHER EMPLOYEES SLID ON SEAT INTO HIM, PUSHING HIM AGAINST THE ROLLBAR. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 5-10-01.BRUISED RIBS ON RIGHT SIDE.
EE WAS TRAMMING WAGNER LHD TO MUCK PILE WHEN HIT A POTHOLE IN THE TUNNEL, BOUNCING UP, HIS HEAD/HARD HAT HIT THE CANOPY. THIS AGGRAVATED A PREVIOUS NECK INJURY THAT HE IS STILL RECEIVING TREAT MENT FOR. HE WAS PLACED ON RESTRICTED DUTY.
WHILE ATTEMPTING TO ESCAPE FROM FALLING POWER LINES DUE TO TRACTOR OPERATOR HITTING GUY WIRE, EMPLOYEE JUMPED 3 FOOT OBSTRUCTION AND SUFFERED A SPRAIN/ STRAIN OF LEFT LOWER BACK. HE REMAINED ON REGULAR DUTY UNTIL 3/6/01 WHEN CONTINUED COMPLAINTS OF BACK DISCOMFORT RESULTED IN RESTRICTED DUTY.
THE MEPLOYEE WAS SEATED IN THE PASSENGER SEAT OF A PICKUP TRUCK WHEN A SUPERVISOR BACKED INTO AN OUTRIGGER OF A PARKED ROOF BOLTER. THE EMPLOYEEWAS WEARING HIS HARD HAT, HE STRUCK HIS HEAD ON ROOF OF CAB. HE FELT THAT HE WAS NOT INJURED AT THE TIME, BUT AT THE END OF THE SHIFT HIS NECK AND BACK HAD STIFFENED UP.
EE WAS DESCRIBING AN INCIDENT ON ANOTHER MACHINEAS TALKING EE REACHED TO SHOW EE WHERE BAG HAD BEEN CAUGHT & LEFT HAND WENT INTO PINCH POINT-HEAD PULLEY-IDLER ROLLER AREA. ABRASION TO BACK OF LEFT HAND & POSSIBLE FRACTURE TO RING FINGER BOTTOM KNUCKLE
EE RAISED COVER ON AUTOMATIC BAGGING LINE TO REMOVE JAMMED BAG. EE REACHED INTO THE BAG TAKE AWAY AREA AND CAUGHT LITTLE FINGER BETWEEN DRIVE BELT AND PULLY. EE DID NOT LOCK OUT EQUIPMENT.
EMPLOYEE WAS TRAMMING LUBE TRUCK FROM LUBE ROOM TO MAIN SHOP. AS HE TURNED RIGHT ONTO MAIN TRAMWAY, THE TRUCK HIT A HIGH CUT, JAMMING HIS HEAD UP INTO THE CANOPY. INCIDENT OCCURRED ON 8-5-00 NO MEDICAL AID WANTED OR NEEDED. EMPLOYEE REQUESTED VISIT TO DR ON 8-8-00.
HIT A HOLE IN HAUL ROAD.
EE WAS OPERATIN A TRACTOR AND CONTINUOUSLY TWISTING AND TURNING TO LOOK OVER SHOULDER TO OBSERVE THE OPERATION OF THE WINCH. AT END OF SHIFT HE COMPLAINED OF LOW BACK PAIN. HE WAS SEEN BY THE PHYSICIAN AND PLACED ON RESTRICTED DUTY FOR TWO DAYS.
EE WAS DRIVING SKID STEER LOADER OVER ROUGH TERRAIN. THIS CAUSED EE TO STRIKE ELBOW ON THE SIDE PANEL IN CAB. BECAME REPORTABLE ON 4/28/00.
SCOOPTRAM OPERATOR STOCKPILING SALT IN PANEL RELIEVER. AS HE WAS BACKING OUT AFTER DUMPING BUCKET, CYLINDER GUARD ON BUCKET CAUGHT ROOF BOLT. SNAPPED OFF STRIKING OPERATOR IN UPPER LIP. REQUIR ED SEVEN STITCHES.
