Contractor employee traveled on UTV to load the drill core on service truck. On the travel back to drill, the UTV began sliding out of control, becoming sideways, catching the edge of the road and tipping the UTV on the side causing the contractor to hit abdomen area on roll cage bar.
Miner used a forklift to move hopper filled with hot sand from a kiln system. Once at the tipping location, miner lifted hopper to shoulder height, got out of the forklift and released the lever to tip the load. The hot sand spread out radially and ran up EE's pant leg, burning EE's skin above the boot and sock line. Diagnosis was partial thickness burn of lower right leg.
Employee was turning in reverse on electric pallet jack and hit left fifth finger on metal tote behind EE. EE was taken to hospital and laceration was sutured. EE returned to work the following day with modified duties.
The employee reported feeling pain in left shoulder towards the end of shift. The next day the employee stated that the pain was a little better and wanted to continue to monitor it. On 4/22/20 EE reported pain to the Plant Superintendent once again and asked to seek medical attention. The employee sought medical attention on 4/22/20 and was put on modified duty.
Employee attempting to flip forks on forklift, the fork rolled back and struck right thumb. Employee reported the incident & continued to work. Employee was taken for an exam on 1/10 and was diagnosed with fractured right thumb w/no restrictions. Doctor visit on 1/16 recommended restriction of use right hand as tolerated until ortho. visit. .
On 01/24/2020, the mine was informed that its workers compensation insurance deemed and approved a loader operator's right elbow pain as work-related. The loader operator informed the mine operator of the concern on 01/06/2020. Medical treatment was obtained.
Maintenance employee's leg and foot caught under personal vehicle and HDPE pipe, that Maintenance crew were dragging.
1 Driver and 5 Passengers were riding in a UTV during non-work related activities. The driver lost control of the UTV and the vehicle ended up on its side. The bump to the head is believed to happen at some point during this time.
Contributing conditions: potential human error and suspended load contact. EE reported the accident on 7/3/18
While going to muck a pile, the loader entered the pile and struck a large object, possibly a ledge or a boulder. This jarred the FEL and the impact caused the operator to experience significant pain in the back.
Employee was driving scraper in overburden cut and when front left tire spun out on a large rock it resulted in jarring the machine and employee strained neck. Employee reported incident to supervisor at end of shift but refused medical treatment at that time. Employee sought medical attention (chiropractor) on 5/22/18 at which time ee was given restrictions.
The mine loader operator was returning to the high wall when right arm hit the bucket controllers causing the bucket to drop to the ground, and resulting in a sudden stop of the loader. Due to the sudden stop the employee was thrown into the windshield, causing injury to the right eye.
Employee had a previous injury and is now exhibiting the same symptoms as before.
Employee was in a haul truck and hit a rut in the road, felt a strain in neck. Employee was referred to the clinic and placed on modified duty.
Employee strained neck when the haul truck ee was operating slid into a rut on the road.
The employee was attempting to turn left on the southbound side of the haul road, when the 1/2 ton pickup ee was driving was struck on the driver's side by a CAT 773 truck that was coming up from behind ee. Employee was taken to the hospital and checked out. Employee was released to light duty. Damage to pickup was substantial on drivers side. Minimal damage to CAT 773 truck.
The employee was covering the end loader during the operators lunch and jarred their back. Employee went to the Chiropractor and was given light duty.
Back is hurting but no accident nor incident reported.
Employee was operating a Volvo 220F front end loader in the stockpile when he felt a pain in his lower left back due to the jarring motion of the loader.
Employee was helping to remove head pulley lagging on a conveyor, as he was turning the pulley by use of drive belts, his finger became caught in the motor sheave, since conveyor had a back/stop sheave would not reverse, so two drive belts were cut off to free employees index finger from between drive belts and sheave of motor. Tip of employees index finger was broken.
EE was using a bobcat loader to stand up a pipe secured to the loader bucket. Once set the rigging was removed, EE was backing the loader away, the pipe tipped toward the loader, EE reached out to catch the pipe and the tip of his right little finger was trapped between the pipe and loader frame causing a small laceration requiring stitches and fracture to the tip of the finger.
Employee reported back pain as a result of the air-ride suspension seat's air compressor not functioning at full capacity. Employee woke the following day experiencing intensified pain and did not report to work. He received medical attention the following day and was returned to work.
Haul Truck driver was parked at tailings area waiting to be loaded with material. Loader operator did not see the haul truck and backed into the side of the haul truck. The haul truck driver sustained whiplash from resulting collision.
Employee complained about shoulder strain. Employee went to the doctor and has to attend physical therapy.
Employee was driving a lull and turned a corner too sharp tipping it onto its side, employee jumped out of the cab as it was tipping which resulted in a laceration to his head and a dislocated shoulder.
the front end loader had a faulty seat that was bottoming out, and the operator received muscle spasms as a result
The employee was operating a loader when the left front tire blew out. The employee was jerked forward into the seatbelt. he went to the doctor, was given medication, and returned on his next scheduled shift. **On October 14, 2013 the employee was reassessed and assigned modified duty due to issues with his leg. He is expected to be out 2 weeks.
Disregarding instructions and a traffic sign, truck driver drove off the main traffic road and onto a cross-over road used by Plant personnel. Trucker exited his truck without instruction and started to attach the tow pin to the front part of the truck. The Loader Operator, not realizing or expecting a truck to be there backed into the trucker and truck while back-dragging road.
We are unsure when this started EE went to her doctor for painful back she had been having for a while and the doctor required her to be off work until 2/21/12 so she could go to a specialist for further testing of what is happening with her back.
EE HAD A SKIN GRAFT ON RIGHT ARM DUE TO INJURY CAUSED BY CONTACT WITH MOVING CONVEYOR BELT.
EE was working on uneven quarry floor. He is not sure what caused injury in his words, more of a cummlative injury by being jostled in the endloader cab.
The employee was assisting in lifting the stationary jaw from the jaw crusher and sitting the jaw on to the floor. The rail trolley and chain hoist being used fell from the beam to which it was attached. The trolley fell on to the employees left hand, which he was using to guide the jaw, and cut the middle finger of his left hand.
EE driving loaded haul truck downgrade, met pickup and gave room to pass at safe speed. RF wheel of haul truck entered ditch before berm. He braked, but weight pushed truck to a drainage break that forced front wheels to R. Truck crossed berm, fell 30' to toe, landed on top. ee wore seat belt, taken to ER, observed and released unconditionally.
Employee was doing maintenance on a material handling boom (rake) attachment for the utility loader the rake rolled and the corner struck the employees foot breaking bones in the employees right foot.
Employee's chair collapsed, pinning his left knee between the seat and the base, spraining his left knee.
An employee was walking through the warehouse portion of the plant between rows of tall supersack bags. He stopped as he approached an open area and another employee was backing out of a row on a forklift. The employee driving the forklift ran over the left foot of the first employee just as he stopped to look out for moving mobile equipment.
Employee was working from South Quarry stockpile with endloader. While backing from pile tire entered uneven area swaying endloader. At this time employee felt pain in his right side lower back.
Trailer had load of sand. Driver was dumping load and tractor trailer went on its side.
EE was driving forklift and suddenly felt something in his eye which he thought was an eyelash. After his shift he was home and looked closer and discovered a foreign object which caused irritation. EE went to the doctor and they removed object and gave him prescription eyedrops.
After loading a tank truck, the employee was retracting the loading spout from the truck . While swinging the loading spout from the truck at the pivoting point , the tip of his right ring finger was severed in the pinch point where the spout pivots.
Worker was riding on the left step of Case front end loader. The operator began making a left turn. This caused the right leg of the rider to be twisted slightly in the pivot pinch point of the loader.
Employee was exiting over-the-road trailer with pallets of sand when trailer floor broke through causing forklift to tilt to the left. He apparently cut fingers as the forklift was tilting and going down.
The employee claimed he injured his back by operating the plant's L-330 Endloader performing routine duties. This accident involves SUSPICIOUS CIRCUMSTANCES because the employee was sent home on Administrative Leave on 10/04/07 due to being caught sleeping on the job (3rd offense). Eight hours after the employee was sent home, he called the plant and claimed to be hurt.
Employee was operating end loader in quarry and drove over rough area and jarred his back.
After replacing the splice on a short conveyor, the EE was completing adjustments to center the belt. The guarding had been removed and the EE inadvertently came into contact with a return idler which caught his arm between the idler and the belt. A co-worker was able to free his arm and the EE sustained a minor strain the his arm resulting in restrictive duties.
On the above date and approximate time, the loader operator was working in Pit #4. While attempting to fill the bucket by digging into the muckpile, a portion of the pile collapsed onto the loader bucket. This caused the rear of the loader to come off the ground, and slam back down.
While guiding a V-belt onto a sheave, the tip of the little finger was caught between the belt and the sheave, crushing the tip and breaking the bone.
The employee was working out near the dredge when the accident occurred. He was sitting on the bow of the jon boat attempting to pull the starboard swing cable back over the sheave while his heels were positioned on either side of the sheave to hold the boat in place. While the employee was pulling on the cable, the jon boat backed away turning his right leg.
EE was adjusting the track of conveyor belt. He was bent over looking under the cover. He placed his left hand on the cover to brace himself. His hand slipped and was pulled half way through the troughing idler.
