Employee was driving a concrete mixer truck approx. 15-18 mph on the plant road toward the river shop. The driver's side front steer tire blew out causing the truck to immediately pull to the left and struck an abandoned fix support structure. This caused the employee to strike the steering wheel, resulting in laceration that required sutures.

Loader operator ran over a rock in the roadway, jarring the operator.

EE was driving a 135-foot manlift. As EE approached the railroad crossing, the front tires of the manlift dropped into a swale before the crossing. The two front tires dropped first, then the two rear tires. This initiated the manlift basket to bounce EE causing EE to twist their arm as EE held onto the guard rail. EE sprained left elbow, placed on restricted duty 2/5/24.

Employee was striking a stuck carry roll on a conveyor. The roll started to spin and pulled employee's hammer in to it. This drew employee's hand across a sharp piece of metal on the framework

While operating a standup skid loader, the miner was cleaning the basement of the crusher and backed into a column. EE's foot was not inside the skid loader therefore mashing EE's foot. EE was diagnosed with a strained ankle and put on light duty for two days.

Miner was backing up in an electric cart and had right foot outside of cart. While looking over right shoulder miner made contact with a ballard and smashed foot between the ballard and the front cowl right behind the steel toe of boot. After taking xrays the tech could not tell for sure if there was an issue.

employee was standing between the front end loader access ladder and the front wheel fender checking for oil leaks. Another employee started the loader and the front end loader articulated, pinning the first employee between the ladder and the wheel fender

While moving a forklift to a different location the forklift tipped toward the vacuum truck and employee's hand was caught between the ROPS on the forklift and the vacuum truck. Left hand was severely smashed.

Buzzi employee was backing forklift and backed into OCC contractor working area and rapidly turned forklift striking an OCC contractor with the back of forklift knocking EE to the ground.

Employee unchoking hopper, locked out wrong discharge rotary valve. Reached into port, fingers pinched between valve flights and housing. Amputated left middle fingertip to nail and ring fingertip to cuticle. No surgery required. Will result in lost time and restricted duty.

Employee was operating an off road forklift transporting some scrap metal, when EE hit a bump some debris on the load became airborne and entered the cab and got behind in between EE's face and safety glasses and into EE's eye.

Employee was driving a haul truck and complained to supervisor that EE's eye hurt.

Ee stepped backwards and was struck by the railing of a locomotive.

While trying to remove blockage and improve product flow an employee reached up and inserted hand into an energized rotary feeder. It resulted in the partial amputation of three fingers of the right hand.

Laborer was holding lower part of a pallet that separated from its top part and the laborer tried to position pallet so it would sit flat on pallet's stand. A forklift operator set pallet down before laborer moved hand away from pallet. Laborer's right index finger was pinched between pallet and pallet's stand. This caused a laceration to finger resulting in 2 stitches.

At the start of a Monday shift, the Quarry Haul Truck driver bumped the berm while backing into the load out bench, causing a jostling to the driver's body. When dumping the load, the driver felt a twinge in lower back when the driver twisted. Doctor stated strain, and advised one week of no truck driving.

Employee was working on the primary, ee foot was over the kick plate on man lift, other employee lifted the basket pinning foot between kick plate and belt

EE was on the property of the Stockertown Quarry and needed to pass a cement truck that was parked. As EE was passing the cement truck the driver of that truck pulled rapidly away from parking spot thereby sideswiping EE's truck & pushing it to the right. Truck damaged & EE's wrist hurt.

Truck Transport driver moved vehicle while employee was in Safety Cage on top of trailer. Employee suffered impact to R Lower leg calf.

My employee was working on a slurry pump as a front end loader that was (operated by Alamo Concrete employee) carrying a full bucket of slurry material as it passed by pump material spilled out of bucket and material accidently landed on my employee. Employee was suppose to return to work on light duty but was sent home for the fact we have no light duty work

Employee was prescribed antibiotics after debris flew behind his safety glasses and entered his eye.

Employee was standing in front of a door to a hopper. Material fell from the top of the hopper and came out the door. The employee turned his back to the material and it covered his back. The harness being worn by the employee held the material in place long enough for it to burn his back.

While working to clean a small pile of material in a dust hopper the employee was covered by previously unseen hot material from the bags above. The employee has had skin grafts to both legs for burns.

while climbing down the ladder on the loader, right shoulder popped loudly, felt sharp pain causing right shoulder strain

While driving a water truck with the window down the employee got something in his eye. The doctor prescribed medicine for the irritation. He returned to duty.

Employee was cleaning a pile of material under a screw conveyor. He noticed a leak in the conveyor housing and attempted to stop the leak by inserting a rag into the hole. The moving screw inside the housing amputated the tips of his right index and middle fingers just below the fingernails.

Employee placed himself between a forklift and an unstable load that was being lifted. Suffered 3 broken ribs and a bruised lung.

EMPLOYEE WAS LOADING TRUCKS WITH A FRONT END LOADER. LOADER HAD A FULL BUCKET OF 1/8" MATERIAL WHEN THE BUCKET WENT DOWN HURTING OPERATOR'S LOWER BACK. EMPLOYEE RETURNED TO DUTY WITH RESTRICTIONS.

While digging in pile, the loader found some hard digging. When the loader bucket came loose, it caused the machine to bounce.

Employee was operating his loader and when moving the shift lever he felt something "pop" in his left shoulder.

employee was driving to locate another employee for some chew tobacco, took eyes off of road hitting gangway framework with golf cart. employee is sore and cart is repairable

EMPLOYEE WAS REVERSING FROM FEEDER IN A FRONT END LOADER AND RAN OVER A LARGE ROCK WITH FRONT LEFT WHEEL. THE JOLT CAUSED A LUMBAR STRAIN.

Employee was replacing an access door on a vertical screw when his glove got caught by the screw. The tip of his left index finger was severed.

Unknown - still investigating.

Employee complained of headache and feeling sick after welding in the kiln while bobcats were mucking the kiln.

EMPLOYEE WAS STANDING ON THE NORTH SIDE OF TAIL PULLEY RESTING FOOT ON THE TOE PLATE, UNDER THE TAIL PULLEY SUPPORT FRAME. BELT TRAVELING UPWARDS CAUGHT THE TOE OF THE BOOT AND PULLED HIS FOOT INTO THE STRUCTURAL IRON.

Employee was in a bent over postion and using a jerking motion to try and move forks on a forklift.

Cable broke on Ameco loader causing the operator station to jerk and the employee fell into the handrail causing discomfort in his stomach and back area.

Employee was operator of a "Guzzler" suck truck tasked to suction out inside of a hopper. Prior to entry, employee had secured hose on outside of hopper to prevent movement, but realized he needed more slack inside to complete job. Employee pulled and lifted on secured hose (approx 30-40lb) resulting in muscles strain in left arm bicep.

Injury was hooking up a car vibrator and the rail car moved and the vibrator hit him on the left knee. he went to the hospital, but at that time no medical treatment was needed. He went to a doctor on 3-31-09 and at this time medicat treatment is required. He Missed work on 4-1-09. .

Employee is a loader operator and over time feels that operating it hurt his back. He reported it on 9/17/08 and it became a lost time on 9/18/08.

Employee smashed middle 2 fingers on left hand between counter weight and support pole when counter weight was nugged by front end loader.

This is an alleged accident. Loader operator stated that he injured his left wrist. He does NOT know the date of injury and he does NOT know the time of injury. He also does NOT know where the accident took place.

Descending the hill at stone plant operating a front end loader, accidentally hit lower bucket lever, causing the bucket to dig into the ground, throwing operator into the steering wheel. Employee sustained contusion of the chest wall.

Operator backed and stopped loader within 3' of kiln door. After operator stopped, EE stepped behind the loader to pick up wire cable and shackle. When he stood up, the loader inexplicably lurched backwards pinning him against the kiln door. The right rear of the loader struck EE's right arm, hip/torso area, compressing him against the door.

Worker was attempting to reposition forks. (Forks are hard to move). He let it down with his right hand to push over with his foot, it swung back and smashed finger on left hand.

EE was operating the cat skid loader moving clinker from outside under kilns to clinker drag. The conditions while running the skid loader were very dusty between the coolers. Being in the dust for a long time, his eyes got aggravated, he rubbed them, and got some clinker dust in his right eye.
