EE states while moving a manlift from the operator's basket EE sustained an injury to EE's mouth. While the machine was in motion, EE hit a hole in the roadway resulting in a bounce causing EE's mouth to contact the man basket.

This incident was not reported to management until 3 days after the incident. The employee states they received medical treatment on the day of injury.
Injured reports that the haul truck EE was operating was jarred while being loaded which caused EE's shoulder to strike the inside of the door. Injured did not report incident until the following day. Medical treatment was provided and work restrictions were given.
Employee was operating a haul truck in the quarry when a bracket broke under the front of the seat. This caused the seat to move up and back causing a sprain to the lower back. The employee did have an existing back issue.
Employee was working in the Kiln 3 screen room, cleaning under a screw conveyor using the vacuum truck when some lime went under EE's safety glasses. EE flushed eyes then and reported the incident the next day. EE was seen by a doctor, eyes washed, given prescription drops, and released to work.
Employee stepped back into the path of forklift that was backing up. Forklift knocked EE down to the ground and the forklift tire rolled up onto foot and ankle. Employee was on restricted duty for a few days then written out of work awaiting MRI results.

An employee was injured while operating a Toro Dingo. Operator entered 200 Tunnel in a forward direction and proceeded down the incline. EE filled loader bucket with muck, raising it approximately 3 feet and started to back up the incline The rear of the Dingo started to raise where the operator was standing. EE was thrown off the machine landing on the tunnel floor.
Injured reached between the frame and conveyor belt to strike a troughing roller and arm was pulled in between the pulley and conveyor belt. EE received stitches in middle finger, index finger and wrist. Is currently off work.
Employee was using the bobcat to clear the plug up in #1 dust chamber. A rush of dust came down around employee. The dust burned EE's cheeks on the outer edges where safety glasses and respirator fit.
While cleaning K-121 belt in the cooler pit, a temp employee noticed a troughing roller between 4 and 5 kiln coolers that was stuck and not turning. EE starting clearing material away from center roller with a shovel. When the roller broke free, it caught the shovel causing finger to be pinched between the shovel handle and the conveyor structure fracturing pinky finger.
Injured had removed a 3' section of DCR screw conveyor cover in an attempt un-jam it. To clear material, EE ran the screw in reverse. The employee then walked to the screw to observe progress when right glove was snagged by the end of the screw flight and pulled in causing injury.
While relocating piping to another jobsite an employee was walking along side a all terrain forklift when his foot got under the tire. Employee was taken to emergency room and x ray showed fracture in big toe and a laceration between the toes.

Injured was loading trucks from a stockpile with a 980H loader when a large piece of material rolled down and struck the loader arms jarring the machine. Injured initially requested medical attention, but was released with no restrictions. Injured subsequently was diagnosed with a hernia, underwent surgery on 5/21, and was unable to return for scheduled work on 5/23.
Employee strained his back while putting on a harness style seat belt.
Employee claims that his neck has been hurting because of driving a haul truck on the quarry roads. There was nothing specific noted for the cause. The pain has gradually gotten worse over the past few weeks. This was initially reported on 2/8/10 and employee asked to see a dr on 3/4/10.
Mechanic was in a JLG man-basket welding on kiln. Welding lead was secured to the man-basket and as it traveled upwards, the lead caught the toe board of the platform. While observing the snag and bringing the basket back down, the welding lead snapped causing the man-basket to jump from it's present location.
Quarry loader operator was filling in as a quarry haul truck driver was jarred around when another loader operator dropped a boulder into the bed of his haul truck.
Driver failed to depressurize his trailer. he attempted to open the dome lid when pressure blew the lid open under pressure. Driver didn't tie off harness. he was blinded & inhaled lime He fractured leg hips pelvis.
Employee was operating a CAT 966. After a shot, he was moving rock. He traveled into the rock pile and the bucket hit a toe. The loader stopped and jarred him. He was wearing a seatbelt. Diagnosed with Lumbar Radiculopathy (strain) on 4/7/06.
WHILE LOADING HAUL TRUCKS AT THE BENCH ON ROUGH GROUND THE BOUNCING OF THE LOADER CAUSED A NECK MUSCLE TO BECOME STRAINED.
THREE MECHANICS WERE CHANGING CONVEYOR BELT ON INCLINED SLOPE BELT 14-100. INJURED EE WAS GUIDEING THE BELT ACROSS THE TOP OF RETURN ROLLERS ONBOTTOM SIDE OF BELT. RETURN ROLLER CAME OUT OF HANGER WHICH IN TURN STRUCK EE ON RIGHT SIDE OF LOWER BACK RESULTING IN CONTUSION AND ALSO STRAIN OF LOWER BACK. EE PLACED ON RESTRICTED DUTY DUE TO LIFTING .
EMPLOYEE WAS OPERATING HAUL TRUCK. COMPLAINED OF PAIN IN LOWER BACK. THE MEDICAL EXAM SHOWED A BRUISED TAIL BONE. THE SEAT WAS IMPROPERLY ADJUSTED TO ALLOW FOR CUSHIONING OF RIDE.
