Employee (Mechanic) got his middle finger (right hand) caught between the cable and load block pulley on a mobile crane. Emp. was attempting to push cable back into pulley groove when injury occurred. Emp. was not wearing gloves.

Employee was dropping railcars when it bumped into another railcar. The impact caused him to be thrown against the side of the railcar, which caused multiple bruises to his right side.
EE was operating the water truck on the haul road. He started up the ramp entrance to the pit area, as he got to the top of the hill, his truck started spinning on the wet road. The truck slip ped backwards, down the ramp and hit the berm, which caused the truck to turn over. There was cab damage to the truck.
HSI driver was traveling east on Gibraltar Mine haul rd. As the driver started negotiating to make a left turn curve he lost control of his vehicle.
Truck No. 1180 rolled over on a curve hauling a load of kaolin. The truck was running too fast for conditions and could not make the curve.
Left the pit loaded, came to a point in the haul road where setting sun hits the mirror. Other truck passed on his left in opposite direction, sun hit mirror & blinded him. He was close to the edge of the road next to the berm & drifted even closer right tire caught the berm & almost came to a stop. Berm gave way/truck rolled down slope 60 ft. climbed out window and climb up hill.
Employee was checking the belt number on the drive belts and the conveyor started in motion. Employee's left hand went through the nip created by the drive belt and the sheave, resulting in laceration to left index finger and compound fracture to left middle finger.
EE was driving off road truck and backed under the backhoe bucket to get loaded he did not stop when the bh operator blew the horn and the bh operator had to stop the truck with the bucket. He said this made his back hurt.
Employee was driving a 10 wheel dump truck on haul road at Dixie Mine. Road surface was wet clay. As vehicle was negoiating a curve, the vehicle started slidding. Vehicle rolled over.
Employee was hanging a Big Bag on hooks when his right little finger was caught by the forks on the fork lift. He abruptly tried to remove it and sustained an amputation to the distal end of his right little finger.
INJURED EE WAS WORKING ON M-55 SCREW CONVEYOR TRYING TO RESOLVE A FEED PROBLEM TO THE PACKER THIS AUGER FEEDS. HE WANTED TO VERIFY THAT FEED WASGETTING TO THE AUGER, AND REMOVED THE CAP ON THE BOTTOM OF THE AUGER. USING THE HOA SWITCH AS HISISOLATION OF ENERGY, HE STATED THAT "HE THOUGHT HE HAD IT OFF" BUT IT WAS IN AUTOMATIC. HE PLACED HIS HAND INTO THE MOVING AUGER.
EE WAS OPERATING A PORTABLE LIFT AND WHILE BACKING UP THE LIFT HE GOT HIS RT. THUMB PINCHED BETWEEN THE LIFT HANDLE AND A PALLET. HE RECEIVED FIRST AID BY THE COMP. NURSE FOR SWELLING & BRUISI NG OF THE HAND AND RETURNED TO RESTRICTED DUTY.
EMPLOYEE LOST RIGHT MIDDLE FINGER TIP WHEN HIS GLOVED HAND CAME IN CONTACT WITH AN OPERATING ROTARY AIRLOCK FEEDER. HE WAS ATTEMPTING TO CLEAN A CLAY POWDER BLOCKAGE AT A CLEAN OUT PORT.
EE WAS DRIVING FORKLIFT IN LOADING ROOM WHEN A FOREIGN PARTICLE ENTERED HIS EYE RIGHT ALTHOUGH HE THOROUGHLY FLUSHED HIS EYE WITH WATER, THE EYE WAS STILL IRRITATED THE NEXT DAY. HE WAS SENT T O THE HOSPITAL WHERE HE WAS DIAGNOSED WITH DEBRIIN THE EYE AND SCLERAL ABRASION. ANTIBIOTIC OINTMENT WAS PRESCRIBED TO PREVENT INFECTION.
