Miner was using "walk behind" skid steer to clean spillage off the ground under the roll crusher return conveyor. The employee trammed the machine into the belt structure causing their left hand to be caught between the top of the machine and the metal belt frame.
Employee suffered minor sprain to neck while operating FEL
Miner was attempting to re-position the forks on a bobcat. The fork bounced and when it did his right hand got pinched between the fork and the support behind the fork causing a laceration which required stitches at the local medical facility.
Employee was backing up on hyster. He had his foot on the step of the hyster and caught his heel on a super sack twisting his heel and ankle resulting in strain/brusing to the ankle and foot.
EE was dumping over a stockpile with a Cat haul truck when the stockpile failed which caused the truck to overturn. MSHA was immediately notified due to serious potential of accident.
Employee was operating bobcat on unlevel ground which turned over on its side. The employee was wearing a seatbelt, but his elbow hit a piece of metal causing a cut. The employee was taken to a local walk-in clinic where he received stitches and medication. The doctor released the employee to return to work the same day for regular duty with no time off due to this injury.
On 12/11/2007 a haul truck leaving the mine travling to the mill was involved in a one vehicle accident. The driver of the haul truck swerved to miss a dog in the road and over corrected causing the truck to leave the highway and come to rest in a field. The driver suffered a broken foot and the truck recieved damage to the front hood and cab area.
Employee came around a curve in a loader, miss-judged distance and struck pillar.
EE dumped at the crusher and did not lower the bed on the haul truck. The bed impacted the brow at the second level entrance. Jamming the cab of the truck into the back.
Employee notice that his eyes were irrated after being around diesel smoke. He was take to the doctor and given prescription eyedrops. The doctor was unsure of the cause of the irritation.
A LOAD BEING LIFTED WITH NYLON KEVLAR FIBER FLAT SLING OVER THE EDGE OF THE FORK TIRES. AS THE TINES WERE SLIGHTLY POINTED UPWARD WHILE LIFTING THE LOAD, THE WEIGHT OF THE SAID LOAD CAUSED SLI NG TO SLIDE DOWN TINES & CUT AND BURNED THE FIBER, CUTTING THE SLING INTO. THE LOAD THEN FELL ONTO THE RT LEG OF THE INJURED EMPLOYEE.
