Operator was loading trucks steadily throughout the day when EE starting getting unbearable neck/back pain. Suspected cause was twisting and jarring from forklift.

Employee was operating haul truck near the new pond when a light vehicle pulled out in front of EE causing EE to lock up the brakes resulting in aggravation of the lower back.

Operator sustained a fracture to right big toe when a Skidsteer Loader Bucket slipped while the employee was in the process of training coworker on the bucket change out procedure. The bucket which dislodged from the hydraulic lift assembly, dropped approximately 6 inches striking the employee across the steel toe of work boot.

Employee reports backing into a stationary roof support pole while operating a forklift. The employee reported neck and shoulder pain but denied medical attention at the time of the event. The employee request a medical evaluation on 7/8/14 as the pain persisted. The employee was diagnosed with a right shoulder (rotator cuff) strain and prescribed physical therapy.

Employee reached onto conveyor to straighten misaligned bag near entry into (double conveyor) incline flattener. Arm was drawn in to shoulder. Co-worker shut down system and helped release employee.

CONTRACTOR injured left pinky finger when it was caught between cable and pulley of crane block. Used his hand to steady or guide the block. Given proper first aid at site, he elected treatment after shift which included a stitch to prevent loss of fingernail. Contractor is VERY INFREQUENTLY used at site. His experience in this submission is estimated as unknown at this time.

While operating forklift from plant to warehouse got something in left eye. This route takes the employee outside the building. Employee was wearing safety glasses. Employee was given a prescription antibiotic drops as preventative.

Employee loading truck with forklift in shipping area when piece of dust or debris got in his eye. Irritation got worse and eventually it was decided he needed the attention of a doctor when flushing did not work. Debris was gone at hospital; employee given prescription to prevent infection.

Haul Truck driver was proceeding to dump site, just after having a break. Truck immediately turned almost 90% from the straight haul path, He hit the berm squarely with both tires and slid down the hill approx. 35 feet. He stated that he saw the truck was turning and heading for the berm, but he could not move his legs or arms to stop the truck.

While dumping a small dumpster into a larger dumpster with a forklift, the employee's finger was caught between the fork and the fork access on the bottom of the dumpster as he manually pulled the release lever. The employee was transported to the emergency room. The top third of his ring finger was surgically amputated.

EE WAS PULLING BAGS OFF CONVEYOR FROM TABLE & PUT HAND ON UNDERROLLER TO ADJST TABLE HAND SLIPPED BETWEEN CONVEYOR BELT & ROLLER RESULTING IN ANAVULSION & CRUSHING INJURY TO HAND

SHUTTING DOWN DOOR FOR TRUCK DRIVER, WHO THEN\ PULLED OUT FROM THE DOCK & EE FELL OFF DECK TO GROUND

EE WAS NOT THINKING SAFETY, JUST DIDN'T REALIZE WHERE HE HAD PLACED HIS LEFT HAND. ROADWAY GOOD, WEATHER GOOD, & EQUIPMENT VERY GOOD. LEFT INDEX FINGER HAD ABOUT HALF THE TOP INSIDE SKIN TORN OFF. MEDICAL TREATMENT NEEDED.

EMPLOYEE CALLED ME STATING HIS NECK PAIN IS CAUSED BY THE VIBRATION OF THE TRUCKS HE DRIVES. HE WAS TOLD THIS BY HIS DOCTOR WHO HAS PRESCRIBED PHYSICAL THERAPY. THE EMPLOYEE ALSO ALLEGES THIS DIFFICULTY IS A RESULT OF TWO INCIDENTS HE HAD ONE 8-20-90 AND TWO 2-27-92.

EMPLOYEE TOLD HIS SUPERVISOR BY PHONE THAT HE WAS UNABLE TO WORK DUE TO HIS RIGHT ARM AND SHOULDER HURTING FROM REPETITIVE MOTION. WORKER'S COMPENSATION IS BEING PAID TO HIM ON A DISPUTED LIAB ILITY PENDING FURTHER INVESTIGATION.
