EE was involved in a motor vehicle accident side swiped by vehicle. EE was wearing seatbelt. There was no loss of consciousness. EE was ambulatory on scene. EE complains of continued discomfort and stiffness to the left hip and lower back area. An MRI was ordered and results reviewed on 11/4/24. Dr ordered PT Restrictions are no lifting or carrying greater than 20 pounds

Injured employee (IE) completed pre-operation inspection of the truck and proceeded to operate the truck, hauling material. IE shortly after realized right eye was sensitive to light and felt discomfort. IE stopped and notified supervisor. IE was taken to an optometrist where a small foreign object was found and removed from the right eye. No specific event was indentified.

Employees were connecting sections of large diameter pipe. To align the pipes, employee was using a tool known as a spud bar to line up the bolt holes while the end of a pipe was suspended from a front-end loader (forks) to aid in positioning. When the equipment operator lowered the pipe, the movement caused the spud bar to pinch the employee's finger against the pipe.

EE was driving company vehicle when EE believes EE "dozed off" and drifted to the right of the mine road into a ditch and landed on passenger side of truck. Airbags deployed, the EE was wearing seatbelt.

EE fell asleep while driving. Hit a berm and landed in ditch, injuring left wrist. EE taken to hospital and prescribed Toradol and Robaxin.

EE was driving work vehicle and hit a bump. This caused EE to hit head on roof of vehicle. EE states grabbed shoulders because of pain caused by head hitting the roof and when EE looked ahead, EE was driving directly into sun. The next thing EE remembers is people talking to EE after vehicle had turned over. EE also injured left knee and was placed on restrictions on 6/7/22.

Employee was operating a Polaris XP 1000 UTV on a mine road and hit a pothole which caused employee to briefly lose control of UTV. After hitting the pothole employee briefly lost control and swerved to the left off the road. This caused the left tire area of the UTV to strike a tree.

While changing Drag cables a tractor was in the process of pulling the cables from the spool secured to a sled. As the employee approached the sled the last two wraps of cable released from the spool, striking the employee on the head and arm resulting in sutures.

While in the process of relocating tool lockers. One employee was assisting a forklift operator when they walked behind a locker after it had been placed on the ground. As the forklift operator retracted the boom, the fork tips contacted the bottom of the locker causing it to tip and fall backwards landing on the employees back. Given Injection, prescription & PT given 10/13/20

While traveling through the mine employee became disoriented due to decreased visibility caused by fog and other factors. Employee failed to navigate a turn and drove off the mine road into a BMP clear water ditch. Employee was treated on 03/19 and released, upon a follow up appointment on 03/23 the EE was placed on restricted duty pending further diagnostic testing.

Trespassers suspected of poaching were spotted by FWC on mine property. When FWC officer activated the emergency lights, the suspects took off on an ATV at a high rate of speed, traveling on the crest road of a dam. The ATV left the roadway traveling down the dam coming to rest on it's roof on the dam's toe road. Both suspects were taken to the hospital.

Trespassers suspected of poaching were spotted by FWC on mine property. Mechanic operating high reach lifted basket to back out of the tight area. The other mechanic in the basket was in the process of checking top side of pipe when EE became pinned between top rail of basket and I-beam structure. (As a result of the basket impact, the man lift could not be operated from the basket, requiring a nearby employee to operate the basket from the ground.)

EE & co-worker were tasked w/applying herbicide to plants & were using ATVs to access the site. EE became stuck. EE walked up to co-worker's ATV & tried to play out the winch line. After finding it wouldn't play out EE placed middle finger of lt hand in winch line hook & asked co-worker to run winch out. They didn't use remote winch controller & EE didn't use safety pull strap.

A front-end loader traveling on a mine road broke through into a water conveyance ditch and overturned. A contractor noticed the overturned equipment and responded to the site. Contractor employees immediately notified Mosaic supervision who activated the site ERT. ERT contacted 911.

EE was operating a dump truck on a dyke road of the F-9 Phase III dam. EE had traveled this road over 100 times. While traveling in the middle of the straight run, EE turned the truck hard left causing the truck to flip off of the road & roll approximately 28 feet. There were no conditions that contributed to the accident, other than driver negligence. EE received serious injuries.

While hauling fill, the employee hit a series of holes in the haul road causing injury to his back

A trespasser died on mine property. The driver lost control, struck the tow road berm, partially ejecting the driver and pinning him under the truck.

Employee was driving locomotive down the main line to retrieve an empty rail car set. The weather conditions were foggy and the locomotive collided with loaded Big Bend cars that occupied the same track. Employee suffered a 2 inch cut on his forehead; he also identified his neck, knee, and elbow as hurting.

Injured employee was a passenger in a pickup truck of which the driver lost control of the truck just south of the 18 BMP ditch. He drove through the south side BMP berm and landed on the North side of the BMP water ditch.

While moving a trommel screen with a 20 ton overhead hoist, the trommel turned and the eye-bolt pulled through the urethane screen. A release of stored energy resulted in a laceration to the employee's chin area.

On road truck driver was exiting mine on paved entrance road with a fully loaded dump truck. Truck wheels dropped of right shoulder, driver steered back to the right, lost control and overturn on the right side. Driver sustained non-life threatening injuries to right shoulder and ribs. Vehicle was damaged extensively. Driver is being treated for injuries at local hospital

Mechanic was driving freightliner 089 truck to 18 dragline for repair day, when he felt something hit him in his left eye. When he arrived to the dragline he noticed his left eye was irritated and he immediately notified his supervisor.

During replacement of a 22.5 degree fitting on the 20 Matrix line, pipe was being moved by rubber tire. When the pipe was lowered, the pipe sprang off the forks towards the injured employee, pining both of his legs (shin) under the pipe.

While driving on bpm berm; IW was distracted. Upon, his looking back to the road; he was in wind row pulling him over the edge of the berm.

WHILE GUIDING OUT WINCH TRACTOR CABLE; CABLE TWISTED HITTING INJURED EMPLOYEE

Employee was on his way to the pit in a company vehicle when he struck a power pole. Initially treated as a First Aid incident on 10/16. When no improvement was noted by medical staff, employee was placed on restricted duty on 10/30.

While employee was maneuvering a flatbed truck through water the front wheels hit a washout, causing the steering wheel to turn sharply striking driver's left wrist and hand area.

Injured employee was closing the gap on the forks on the Sky Trac Forklift. The forks are designed to be slide back and forth on the round fork support bar to accommodate the application. The employee had raised the end of the fork to make the relocating process easier. When he lowered the end of the fork, his finger was caught between the fork and the frame mount.

While hooking up transport, trailor moved forward and rolled over employees foot.

Injured party was riding in passenger seat of rock truck when the driver lost control. The truck turned on its side. Accident took place on paved road near mine exit onto Route 37. Employee seen by doctor and taken off work for three days pending follow-up visit.

Employee was changing out a gasket in pipeline. Employee had loader to pick up the pipe. Employee put the head of a bolt in between the flanges to wedge the pipe open. Employee was still holding bolt in hand when pipe was lowered; pipe twisted causing bolt to fly out from between flanges and cut employee's right hand on inside just under the thumb.

While walking down the stairway a piece of out of service pipe fell and laid across handrail. Crane was called to pick up pipe as it was suspended above another working area. While picking up pipe, employees, thinking the pipe was wedged, let down load. While shaking load employee's left index finger got pinched between pipe and screen rail.

Locomotive 3802 was stopped position waiting locomotive 3803 to couple up. The locomotive engineer of 3803 was unable to stop the locomotive before coupling resulting in a collision with excessive speed. The two crew members on the locomotive 3802 were knocked to the floor. One flagman was treated at the Dispensary and sent to the hospital for sutures for a head laceration.

TWO PANS COLLIDED CAUSING CAB OF ONE TO CAVE IN, TRAPPING EMPLOYEE INSIDE.

TWO SCRAPERS IN HEAD ON COLLISION

Driver was operating Terex end dump and hit a hole in the road. The truck bounced and he hurt his neck. Employee was wearing seat belt.

Claimant was hauling - took curve too fast & rolled over causing windows to break.

EE STATES HE WAS OPERATING PAYLOADER AND DOOR CLOSED ON TIP OF FINGER. DIAGNOSED WITH FRACTURE ON 11-10-03.

EMPLOYEE STATES HE WAS MAKING LEFT TURN OFF HWY 37 ONTO C.R. 640 WHEN WOMAN IN ANOTHER VEHICLE RAN THROUGH STOP SIGN & STRUCK HIS TRUCK BEHIND DRIVER DOOR. WEARING SEATBELT REDUCED HIS INJURIES.

EMPLOYEE STATES SHE WAS REMOVING WEEDS FROM WATER INLET WHEN CLAY FELL FROM OVERHEAD BELT STRUCTURE & HIT HER SHOULDER. SHE WAS TREATED CONSERVATIVELY FOR 3 WEEKS, BUT PLACED ON MODIFIED DUTY ON 5-28-03, MAKING INJURY RECORDABLE AT THAT TIME.

EMPLOYEE STATES HE HAD RIGHT FOOT RUN OVER WHILE WORKING NEAR BUCKET TRUCK. PLACED ON RESTRICTED DUTY ON 1/17/02.

EMPLOYEES STATE THAT WHILE COMING TO WORK A VEHICLE SWERVED IN FRONT OF HIM ON MINE ENTRANCE ROAD AND CAUSED VEHICLE COLLISION.

EMPLOYEES STATE THAT WHILE LEAVING WORK HE SWERVED AROUND POTHOLE IN ROAD AND INTO PATH OF ON-COMING CAR RESULTING IN COLLISION.

EE ALLEGES HE PASSED OUT WHILE DRIVING. HE RAN INTO PIPE AND RECEIVED FRACTURE TO LEFT ARM. NO MEDICAL EVIDENCE FOUND TO SUBSTANTIATE CLAIM OF PASSING OUT.

EMPLOYEE STATES HE WAS OPERATING HI-REACH AND MADE TURN. BASKET CONTINUED TO SWING AROUND AND MAN STUMBLED INTO SIDE OF BASKET, ALLEGING INJURY TO SHOULDER. GIVEN RESTRICTIONS AND PRESCRIPTION ON 6-18-01.

EMPLOYEE STATES HE WAS WALKING UNDER OVERHEAD CONVEYER AND WAS STRUCK ON HEAD BY A PIECE OF MATERIAL OFF CONVEYER STRUCTURE. MATERIAL WAS UNCONSOLIDATED AND IT CAN NOT BE DETERMINED HOW BIG OB JECT WAS OR HOW FAR IT FELL. EMPLOYEE SUFFERED CRACKED VERTEBRAE.

SEAT ROLLED FORWARD AND HE BUMPED HIS KNEE ON BOTTOM OF STEERING WHEEL.

OPERATOR LOWERED BUCKET AND HIT EE IN BACK CONTUSION LEFT ARM, SHOULDER AND CHEST. X-RAY NEGATIVE.

EMPLOYEE STATES SHE WAS DRIVING HER TRUCK TO WORK BEFORE HER SHIFT STARTED. AS SHE TRAVELED ALONG A MINE ROAD, SHE TOOK HER EYE OFF THE ROAD, MISSED A TURN, & ROLLED HER TRUCK. COMPLAINED OF P AIN IN NECK & SHOULDER WHERE SEATBELT TIGHTENED.
