Employee was trying to unstop a suction hose when draw rock fell on his upper right shoulder

An unintentional roof fall occurred in the KY Ridge Mains in the #6 entry between Xcut 26 and 27. The fall is approximately 40' long, 20 ' wide and 7-8' height. The fall does not effect the passage of miners or the ventilation system for this area. This area is part of an inactive area of the mine on the intake side and is examined weekly.

While prying rock out of tailpiece with a slate bar the bar slipped and the miner cut his left hand.

Cut finger with a belt knife.

Employee was lifting objects, putting up supplies and felt pain in the lower back and in left leg.

Injury said that he was trying to exit the scoop & his foot got stuck under the starting box. He twisted his knee trying to get out. He said if the foreman would take it easy on him, he wouldn't go outside and fill out an acc. rpt or go home and seek medical attention. There were two witness besides the foreman to this statement.

The miner was going underground, operating a scoop. The scoop hit the outside canopy, the miner slid over and hit his neck.

Making belt setup, squaring u belt to put splice on, knife slipped and cut hand.

The miner was trying to access the feeder. He was assisted in taking the guard off the pick breaker. As both miners were raising the guard, one miner screamed with pain.

Employee was cleaning up debris on the bottom floor. A ceramic lined chute was being removed 3 floors above. A 1/2" X 4" X 6" pc of ceramic fell from the chute into the machinery well, bouncing from side to side. The employee app 30' away from the barricaded area was struck on the rt ring finger.

Changing batteries on scoop, latch pin slipped mashing fingertip.

Two men were tied off in the basket of a manlift approx. 15' in the air. They went to swing the basket in closer to the concrete forms on the stacking tube. The basket moved quickly causing injured to hit his lower back against the rail of the basket.

Rock fall occurred in old face area, weekly examiner found fall. Top is laminated slate and sandstone.

Employee was setting headdrive when the chain ratchet slipped and threw him into the drive roller.

Valve stem hung up on tire. Tried to pry it off with a slate bar and it came off all at once and the slate bar with all the tire weight fell across the foot.

Beating track pin in miner, pin chipped striking injured person in left thigh.

an unintentional fall occured in the 11th crosscut on the #3 entry. The fall measured 20'x38'x45' thick.

EE was struck on the left foot by the back end of the bolt machine.

A roof fall occurred in the 11th crosscut in the track entry along the no. 4 belt measuring 20'x30'x3' thick. The area had been developed approx 2 years ago.

Roof fall found in the abandoned area of the 004 section, in the no. 4 entry, measuring 20' x 20' x 7' thick. This area will be cleaned and re-supported.

Roof fall found in the abandoned area of the 004 section. Fall was in the 50th crosscut in the no. 7 entry of the previous no. 7 belt, measuring 20'x 20'x 8' thick. This area will be cleaned and re-supported.

EMPLOYEE STATED THAT HE WAS CROSSING THE CROSS-OVER OUTSIDE WHEN HE SLIPPED AND FELL, STRIKING HIS RIGHT SHOULDER.

Employee was using a belt knife when it slipped and struck his left leg, just above the knee.

Employee was operating a bolt machine when the ear broke off of the stab jack, causing the machine to drop and the canopy struck him on the head.

Employee was walking outside from loadout office slipped on gravel in parking lot. Employee drove himself to emergency room c pineville hospital.

Employee was using a belt knife when it slipped and struck his left thumb. The laceration required sutures.

Employee stated that strained his back while lifing a belt roller.

Employee stated that he strained his back while pulling on belt structure.

A roof fall measuring approx. 30' long, 20' wide and aprox. 4.5 thick occurred in the no. 6 entry of the submains. The fall did not impair ventilation or emtrap equipment.

A roof fall measuring approx. 38' long, 20' wide and approx. 8' thick occurred in the neutal entry just outby the no. 6 belt drive at the second panel left off of the submains.

Employee was using a belt knife when it slipped and struck his index finger. The laceration required sutures.

Employee was unloading supplies when he pulled something in his abdominal area.

EE WAS RIDING IN A MANTRIP THAT WAS DEAD. IN AN ATTEMPT TO GET THE UNIT TO THE CHARGING STATION, HE HAD A SCOOP PUSH THE UNIT. IT THEN WEDGED AND CAUSED THE EE TO HIT THE MINE ROOF.

EMPLOYEE WAS ATTEMPTING TO MOVE A SHUTTLE CAR CABLE WHEN HE CAME INTO CONTACT WITH A BARE PLACE IN THE CABLE, CAUSING HIM TO RECEIVE A SHOCK.

EE WAS WALKING IN THE ROADWAY WHEN HE STUMBLED AND FELL, LANDING ON HIS RIGHT HAND. THE INCIDENT RESULTED IN A SLIGHT FRACTURE IN HIS RIGHT WRIST

Employee was riding in a scoop when he hit his back against the scoop bucket.

EMPLOYEE STEPPED ON A SHOVEL & TWISTED HIS KNEE.

EMPLOYEE HIT HIS HEAD AGAINST THE TOP WHILE RIDING IN A SCOOP.

EMPLOYEE WAS HANDLING A ROOF BOLTER CABLE AND STRAINED HIS BACK

EMPLOYEE WAS RIDING IN A SCOOP WHEN IT HIT A HOLE, CAUSING HIM TO STRIKE HIS BACK AGAINST THE FRAME OF THE MACHINE. EE BEGAN MISSING WORK ON 6/8/05 & RETURNED TO WORK ON 6/26/05.

EE WAS ASSISTING WITH THE INSTALLATION OF A BELT RAIL WHEN HE GOT HIS LEFT MIDDLE FINGER PINCHED BETWEEN THE RAIL AND THE STAND. THE LACERATION REQUIRED SUTURES.

EE was cutting a piece of belt when the knife slipped & struck his right forearm the laceration required sutures.

EE was struck on left hand with anchor, causing a fracture to ring finger.
