EE lifting/pulling 3ft 6in pipe weighing 50lbs from rack. They felt tendon pop in left arm, and immediate pain. Seen at ER, diagnosis complete lt. bicep tear, released with restrictions. Seen at clinic, referred for MRI and ortho consult.

EE was loosening bolt on pot head when the wrench slipped and pinched their left index finger between the wrench and the pot head. EE reported incident on DOI. Seen at clinic 8/7, diagnosed with left index finger fracture, referred to orthopedics. Released to restrictions which are being accommodated.

Light vehicle traveling up decline when an employee detected a burning odor. Smoke and flames were found coming from underneath vehicle. Attempts were made to extinguish however was unsuccessful and the mine was evacuated. No injuries occurred.

Ee removing feed screw chute when crane overran and caught pinky between snatch block and handrail. Seen at clinic, sutures requred to close wound, no fracture, released to modified duty. Seen at ortho, released to full duty 8/5/26.

EE was doing heading inspection when a rock fell from the back, striking them on ther hard hat. Seen at hospital and followed-up at clinic, diagnosed with neck/back injury. Released with restrictions. EE resigned effective 7/30 after taking PTO for 2 days rather than returning to work under restrictions.

IE was assisting an EE in reconnecting a broom handle to a vacuum hose, Prior to the repair, IE properly shut down the vacuum using the safety T to eliminate the vacuum hazard Once the repair was completed, EE turned to resume work while IE walked away and loss IE's footing due to slippery surfaces resulting in injuring IE's foot.

Ee working from scissor deck. Deck tipped, ee hung onto the upper rail, but reports striking ground when deck landed. Reported the incident, no injury reported. Worked 2 shifts, went on 5 days off. On 7/30 EE reported to NGM that EE was seen by a doctor on 7/28 and diagnosed with a fractured rib. Put off work as of 7/30, filed WC claim 8/4, follow-up 8/17.

EE pulling tote from conveyor belt drop chute using rope w/come-along, felt sudden pain in rt shoulder. Reported incident, went home, slept, asked to be seen by provider after they woke up with pain. Diagnosed with strain, referred for MRI, released to modified duty.

Newly hired employee was in a man-basket torquing nuts on Nevada bolts. EE placed EE's hand over the rear of the basket and under the impact torque gun. The operator of the forklift lifted the man-basket thus pinching EE's hand and causing the injury. There was not direct line of sight so effective communication was lost.

Ee was gripping rack by the glass tubes and one had a hairline fracture and shattered in EE's hand. Seen at clinic, diagnosed with laceration to left thumb, released to modified duty. On follow-up on 7/20, ee released to full duty.

EE stepped wrong on uneven ground, heard and felt pop in knee. Reports previous knee pain starting on 6/30. Seen at clinic, given injection of steroid, released and worked one day of restricted duty.

EE grinding, blade caught glove and wheel cut right index finger. Seen at hospital, laceration closed with sutures, EE released to full duty.

During a pre-trip inspection, the rigger was directed to the opposite side of the engine compartment. Instead of climbing down to go around, they walked across stored wood mats on the crane. The mats shifted, causing them to fall and fracture their ankle

EE was sitting in cab of haul truck. They put their middle finger in the hole of the A/C vent and their finger was cut by the fan. Seen at ER, diagnosed with laceration to the right middle finger and put on restrictions that do not preclude full duty.

EE was taking radiator cap of dozer, coolant splashed onto left wrist. 2nd degree burns to left wrist, top of hand, thumb. Seen at ER and clinic, released to modified duty and referred to wound care.

Ee reports stepping onto bolter to turn off pressure washer. Stepped up w/right leg and felt immediate pain/burning sensation. Seen at Golden Valley, diagnosed with shin splints (right medial tibial stress syndrome). Released to modified duty.

EE was leaving dry and they high fived another EE. Reported immediate hand pain. Seen at clinic 6/18, did not file a C4. On 6/25 EE went to Hospital and filed a claim for workers' comp. Referred to ortho who did previous surgery. X-rays shows issues with screws and plates from previous surgery. Both providers noted this issue is non-occupational.

Ee boarding RH shovel boarding ladder when another ee activated the ladder, resulting in the injured ee being struck by the ladder below EE's ribs. Ee fell to ground, landing on right wrist. Xrays negative, diagnosed contusions, released to modified duty.

An Unplanned Fall of Ground occurred at the 4725 - 102 Axs. No injuries or entrapment occurred from this event.

Ee reports numbness/tingling in rt hand/fingers for 2 weeks. Symptoms wake EE up. Fingers swollen. Ee also reports tingling in lt hand/fingers, not as bad as rt. Ee operates a shovel, suspected diagnosis is carpal tunnel. Referred to hand surgery.

Ee removing grating from around pump to clean sump. Ee's left index finger pinched between grating and concrete floor. Seen at ER, diagnosed with laceration of finger/nailbed, and avulsion of nail. Wound closed wtih stitches and ee released to light duty on next scheduled shift.

During jumbo shank replacement, the flushing head was stiff and difficult to seat. Force was applied while the employee's hand remained near the pinch point. The head slipped/shifted unexpectedly, contacting the left middle finger and causing a laceration and fracture requiring one suture.

Ee reports working in wet ESP at roaster with improper ppe 9/22/25. Alleges mercury got in right eye twice and also exposure through inhalation. Ee diagnosed by DO on 6/8/26 w/ mercury toxicity and cervical radiculitis. Ee filed claim for workers' comp on 6/9/26. NGM has no biological monitoring evidence of exposure leading to removal from work area for this ee.

Ee using blue room that had just been serviced. Wiped down seat with provided scrubs. Sat on seat, came in contact with chemicals used to service blue room. After a short period of time, experienced swelling and dermatitis on genitals and anus. Seen at HGH, released to full duty, with 48 hours off allowed if needed.

Ee sustained a Standard Threshold Shift in hearing. Confirmed reportable noise induced hearing loss. Occupational exposures could not be ruled out. Confirmation received from, Au.D. CPS/A, on 06/01/26. Retraining and refitting complete.

Ee adding bolts to carousel on bolter when rock came down, striking EE on the head. Transported to HGH, diagnosed with a scalp laceration and closed head injury, released to modified duty. Ee followed-up on 6/2 and put off work. Diagnosis was changed to concussion.

Ee was driving Ford F250 underground, heard a loud bang, slowed to stop, saw flames coming from clothes, jumped out of truck, rolled on ground. Truck rolled, ee grabbed fire extinguisher, called mayday, tried to put fire out. Shifter arrived, took ee to surface. Seen at ER, diagnosed with 2nd degree burns to left hand, referred to burn unit.

Ee working to clear apron feeder was about 3-4 steps up, slightly turned upper torso to grab mill bar, felt sharp pinch/shooting pain from back to hips into legs. Stopped work, reported issue. Seen at NNRH, diagnosed with lower back pain, released to full duty. On follow-up, put on restrictions.

Employee was directed to retrieve a part from another on-site location. A loader blocked the roadway, requiring travel around it on uneven terrain in a side-by-side. While driving, rough ground caused the vehicle to jerk toward the loader. The employee's left hand, holding the safety handle, was caught in a pinch point during impact, causing an open fracture to the left wrist area

On 5/16/26 ee was driving a light vehicle underground on the 4260 ramp. EE ran over a piece of metal that went through the floorboard of the truck, inches away from EE's leg. There was no injury. On 6/9 EE reported feelings of doom, panic attacks and anxiety. EE was seen and filed a claim for workers comp. The claim is denied pending medical investigation. Ee diagnosed with PTSD.

A fall of ground occurred in the FED2352 level on May 13th, 2026. The fall of ground happened between shifts and was discovered by the ore control geologist team. Approximately 200 tons of material fell.

Night was clear normal temperatures for evening this time of year. Pit traffic conditions were slow. Helper was 1 week into EE's second hitch with company. Cyclone was clogged; helper tried to use EE's hand to unclog it. Hand was injured by the gate closing inside the cyclone.

Ee was stepping out of conex holding a box of uniforms and EE rolled EE's right foot on the lift gate. Seen at clinic, diagnosed with right ankle strain, and released with restrictions that do not preclude EE's full duty as a planner.

Ee climbed on scissor deck to assess what would be needed for the job. Reported pain in back. No injury by accident, ee has long history of back issues that are non-occupational. Claim is being denied by workers' comp pending medical investigation. Ee on restrictions, site cannot offer modified duty at this time.

Ee reports stepping up on top of berm while doing an inspection. EE reports feeling pop to the bottom of right foot. Seen at Golden Valley, negative for swelling, redness, numbness, tingling. Xrays taken, but no results reported. Released to modified duty and referred to podiatry.

Ee walking downhill to EE's equipment when EE's knee dislocated, causing EE to fall to the ground. Seen at Golden Valley, diagnosed with strains to knee and back, and released to modified duty.

Debris entered eye requiring medical treatment to remove.

EE was installing 42" pipe when the wind blew into the pipe causing dust and debris to enter EE's left eye. Seen at eye doctor on 4/23, diagnosed with a foreign body in the eye which was removed and prescription eye drops given. Released to full duty on next shift.

EE was assembling core shed table. Putting bolt and nut on, had left hand on table holding bolt. While pulling down on the table, felt sharp pain in left side of chest. Seen at clinic, diagnosed with a pectoralis strain, released to light duty.

EE was repairing a defective windshield wiper unit. EE saw motor linkage was disconnected. While reconnecting the linkage, the wiper motor unexpectedly activated, pinching their left ring finger between the linkage and the ball. Seen at clinic, diagnosed with fractures to the left ring fingertip, and wound closed with sutures. Released to modified duty.

EE using hammer to loosen a bull hose from cam fitting. Other EE assisting and pulling on bull hose. Bull hose released unexpectedly while the EE was swinging hammer, and their thumb was pinched between the two pound hammer and the cam fitting. At ER, thumb nail was removed and one suture placed in nail bed. Small fracture at tip of thumb. EE released to restricted duty.

EE has been off work since 3/28/25. On 4/9/26 they contacted NGM to notify us they were diagnosed with an occupational disease, bronchiolitis. EE was seen by a doctor who confirmed the diagnosis of bronchiolitis. They kept the EE off work pending ongoing treatment.

Ee was disconnecting a fire hose, walking while looking down, turned EE's head and ran into the outrigger of a crane with EE's face. Fractured EE's front teeth. Seen at Any Day Dental for treatment starting on 4/7 and went back in on 4/8 to complete. Released to full duty 4/9.

Ee had climbed up into EE's haul truck and sat down. EE dropped a paper, pulled the arm rest up to reach the paper, when EE felt a pop in the right side of EE's right shoulder, then instant pain. Seen at ER, diagnosed with a right proximal humerus fracture. EE has been diagnosed with osteopenia which can lead to non-traumatic fractures.

Ee reported to NGM on 5/4/26 that on 4/3/26 ee was working 'in EE's columns' when a contractor and crew started hydro blasting above EE, causing debris to fall onto EE. EE reports that EE and a coworker yelled and went to the control room. Injury not reported to NGM until 5/4/26. No signs of injury to ee. Site cannot confirm incident occurred as reported by ee.

Ee reports steam cleaning on top of shovel when EE slipped an fell, landing on EE's right side and shoulder. Seen at ER where EE was diagnosed with rib and back contusions and shoulder strain. Seen at ortho on 4/9, diagnosed with AC joint separation. Referred for surgery.

At approximately 10:35 PM on 4/1/26 smoke was observed inside the refinery via camera feed in the AARL Control Room. Security was notified. By 10:50 PM the affected cells were fully engulfed. Fire suppression was activated at 11:01 PM, and the fire was fully extinguished by 12:25 AM. No personnel were inside the refinery, and no injuries occurred during incident.

At approximately 8:50pm a ground fall was reported in the MBD2894. The ground fall occured on day shift and was witnessed by two geo techs. Approximately 150 Tonnes of material fell.

EE was walking past an unoccupied jammer underground when they slipped and began to fall. EE struck out right hand to catch themselves, contacted a broken bolt sticking out of the rib, lacerating right palm. Seen at clinic, wound closed with sutures, and EE released to restricted duty. Site offering modified duty.
