Injured employee was clearing chunks in an air slide. During the process, dust got past the safety glasses and irritated eyes. Employee rinsed eyes, but the left eye still felt a little irritated. The next day employee decided to go to the doctor and the doctor issued prescription eyedrops making this a MSHA recordable event.

Employee was loading trucks in pit and load shifted and material (shale) came over the bucket striking the windshield and came into cab and broke the dash. Dash cut caused a laceration and contusions to left leg and left little finger requiring two stitches to finger and leg.

The employee had a possible minor allergic reaction to a beaded compound.

EE was lifting a 5-gallon bucket of oily waste to empty into the oil dumpster when EE felt a pop in left shoulder.

Employee was on a platform washing down the plant when EE was observed collapsing to the floor. CPR was performed until EMTs arrived and EE was transported to the hospital for a heart attack.

EE felt a pop in right shoulder while installing buckets on a bucket elevator, EE was placed on light duty 4-26-18 with a possible rotator cuff tear

Injured employee got dust in the left eye while cleaning under the clinker cooler drag. Irrigated the eye at work and thought a shower later that night would take care of any remaining irritation. The next day the eye was still bothering them, so they decided to go to an eye doctor. The eye doctor prescribed ointment and it felt better later that day.

EE was passing brick and stacking them on the board for installation. A brick fell from above and hit EE on EE's right hand smashing EE's fingers and making a cut on right ring finger.

Lift rock approx. 75 lb to place into tumbler.

Employee said they have had numbness & pain in both hands/wrists for some time and had been going to a doctor who diagnosed it now as Carpal Tunnel.

An employee was pushing an empty cart into the passenger elevator on the preheater tower when EE felt a pop in EE's left elbow. EE initially felt it would be ok but pain persisted until went to a doctor who diagnosed EE with a detached tendon from EE's bicep. On 3/12/2018 EE had surgery to repair the problem.

On 2/22/18, Injured Employee (IE) tweaked back moving a wheelbarrow full of material during a housekeeping task. On 3/20/18, IE went to doctor because ee was not getting better. IE's doctor stated back is sore, ee needed time to heal, and placed ee on two months of restricted duty.

The injured employee was attempting to drill out a plugged purge line with a large hand drill. The bit caught in the hole and cause the drill motor to spin around and strike the employee in the right cheek. On Monday 02/19/2018 the employee experienced increased swelling and sought medical attention where they treated employee for infection.

Driller and mechanic were performing drill maintenance/repair when driller began to double over attempting to vomit. Chest was sore and left arm was numb. Mechanic rushed them to the ER at Chanute Med. 1 hr later, mechanic was told that driller had a heart attack and was going to Freeman Med in Joplin, MO. Driller was placed in ICU. On 2/17, employee was released.

The employee was emptying a trash barrel into the dumpster when the barrel slipped further into the dumpster pinching EE's right ring finger between the barrel and the edge of the dumpster. The ring finger required three stitches to close the laceration.

The employee was removing a bucket tooth and retainer pin on a Cat 988 loader, employee missed the driver tool used to remove pins striking right thumb with a 3lb hammer. The thumb broke in two places near the mid-knuckle.

Injured employee (IE) was backing a forklift down a concrete forklift ramp. When the forklift got near the bottom a piece of the concrete broke loose and caused a hole approximate 12 inches in diameter. The forklift tire went into the hole and the front end of the forklift quickly dropped approximately four inches. The sudden drop and stop jarred the IE and injured IE's back.

Injury became reportable on 04/02/18 when EE had surgery. Employee was digging a headline cable that was buried in the sand and did not release pressure on the winch. Employee went to step over the cable, when it released from the sand it caught EE's leg it flipped EE. EE landed on right shoulder.

While installing a door on a crusher when the wind blew some crushed rock dust into employee's left eye. EE states that EE was wearing EE's safety glasses.

Employee was sorting 1 1/4" steel, mill balls used to grind cement in a roller mill when a stray mill ball rolled down the chute incline and ramped up towards the employee who was in the process of securing the chute discharge to the upright position. As such, the steel mill ball struck the employee in the mouth.

Employee was performing an overhead patch repair on the kiln eyebrow. EE was tamping plastic using a hand held pneumatic pounder. The air exhaust from the tamper blew material behind safety glasses into right eye.

On 18 Jan 18, injured employee (IE) strained left knee while moving 40' of vac truck hose. IE felt slight pain, reported the incident to supervisor, and started first-aid/self-care treatment. On 29 Jan 18, IE wanted to go to the doctor because IE felt the knee was unstable. The doctor prescribed six physical therapy treatments and the IE is feeling better.

EE was preparing to remove a chunk from a cement silo cone. When the port was opened material flowed out. While trying to close the port dust got into EE's eyes. EE was seen by a Dr. and prescribed antibiotics.

Employees were using a hammer bit on a mini-ex to extract large bolts from a raw mill tire. A washer from a bolt shot out in the process striking an employee on the upper lip which required two stitches.

The employee was working near the raw mill when EE tripped on the catwalk but caught himself on the handrail. When EE caught self EE felt a strain in right small finger. We monitored the strain to see if it would improve but it didn't. A doctor diagnosed that the tendon in the finger had torn and needed repair. The employee had surgery on 3/22/18 making it reportable.

A Production employee entered the tower elevator at ground level. During ascent the elevator stopped between floors 1 and 2. The employee radioed for assistance and attempts to reset proved unsuccessful. A safe rescue was performed via the roof hatch. A call was made to MSHA when it became apparent the rescue would take longer than 30 minutes.

Loader Operator began having neck pain approximately two weeks prior to reporting. Employee believes the symptoms are from "bumping around in the loader."

Employee trying to free up a stuck rail car door along with another employee. The door was freed up and closed. In doing so the employees left hand had gotten caught in a pinch point of the door mechanism.

Employee was walking to the picnic table for lunch and twisted right ankle. X-rayed and given Ibuprofen and an ace wrap and told to keep elevated with ice. Released to regular duty.

Injured Employee (IE) was torch cutting an approximate 3-inch hole through 3/8-inch steel with refractory behind the steel. Unknown to IE, a piece of molten metal fell, hit IE's blue jeans and caught left lower pant leg on fire. IE's coworker noticed and smothered the fire. The result was an approximate 3-inch diameter second degree burn.

EE was replacing a spout plug in the shipping silos spout. When EE looked up to align the plug with the bolt holes dust fell into EE's eyes. EE was wearing safety glasses and hard hat. EE had lowered the spout and knocked any loose material out before beginning work.

While backing a 775 CAT haul truck to dump material the rear wheels slide sideways into a slight depression and slowly tipped onto its right side. The driver was wearing seatbelt, was assisted out of the cab by a coworker, but bruised inner left thigh during the incident and was place on light duty.

On 15 Oct 17, employee was tasked to replace a light bulb. While doing so, the heavy light cover came off and either struck or strained a finger during the attempt to stop the cover from falling. Initially they thought the finger was sprained, but a follow up doctor's visit on 17 Oct 17 revealed a broken ring finger.

EE was helping tear down and clean up cables around the plant. EE began experiencing tingling in arms and became sweaty, dizzy and lightheaded. The foreman helped EE into air conditioning and drove EE to the ER at Cedar County Memorial Hospital. EE was tested and observed and dismissed with diagnosis of kidney failure due to dehydration. EE was released.

Fire was possibly started by welders working in the area earlier in the day. MSHA was called but the length of the fire did not meet MSHA report. Hot work permit issued and followed. Coal system has been locked out. No coal was in the day tank. Belting was the only loss. Fire was reported at 6:21pm and extinguished at 6:28pm.

A welder / repairman was using a pry bar to lift up a welded bearing mount plate. The bar slipped and struck the employee in the mouth, causing a small, superficial lip cut and a cracked canine tooth.

At 10:16 an employee reported was stuck in the elevator at the cement silos. The elevator car was about 3 to 4 feet above ground. The employee was able to get out at 11:16. The elevator over speed brake locked and stops the elevator from operating.

While an employee was removing a heavy 2'x3' door, it swung out of position striking the employee in the leg causing a laceration which required 8 stitches.

Workers left ankle came in contact with metal object resulting in a puncture wound requiring 2 stitches.

Employee was working on installing some Mill Liner Plates in a Finish Mill. They reported soreness in their lower back on the right side. The employee does not recall a specific incident that lead to the injury.

Employee was lifting an auger bit on the rack and dropped it. It pinched/crushed the tip of left ring finger.

EE reported pain radiating down from neck into right arm that they believe occurred from years of shifting haul truck. The employee received a shot to relieve pressure around the nerve in the cervical plexus.

Injured employee was tasked to repair/replace a transfer pump seal. While removing the pump impeller with gloved hands, the unexpected sharpness of the blades cut through glove and into left middle finger. The cut required three sutures.

Employee was replacing cutting edge on a Cat 988 loader, when removing a bolt employee grabbed the head of the bolt and cut left ring finger requiring four stitches to close.

The injured employee was operating a forklift on a gravel drive / parking area to empty a material tube. When returning to EE's area the forklift was struck by a semi-truck traveling at approximately 5 mph. The impact jostled the lift and operator resulting in a cervical and lumbar strain and restricted duty.

The EE reports that they have been experiencing soreness and numbness in their hands and wrists, which has been occurring off and on for a few months. The employee is unable to provide a mechanism or singular event that lead to this condition. Employee was seen by a medical professional and was diagnosed and treated for Carpal tunnel Syndrome.

Injured employee was attempting to seal a leaking port hole on a cement silo. Cement dust got under or around EE's safety glasses and into EE's eyes. EE rinsed EE's eyes thoroughly, but an hour later felt there was still dust or a scratch in EE's left eye so EE was taken to the doctor for additional care. EE received prescription eye drops for EE's left eye.

Employee received a laceration to their left thumb from broken glass. The laceration severed a tendon which was repaired surgically through an outpatient procedure.

EE was lifting the secondary crusher suspension bar into place when felt a strain in lower back. The incident occurred on Monday, July 3rd, 2017 and EE reported the injury on July 7, 2017. EE received treatment from a chiropractor and is working modified duty.
