Welding angle under a screen. Welder ground was not making contact, so employee took off hard hat to crawl out of narrow opening and stood up without EE's hard hat and hit EE's head on a 3/8" bolt.

Removing a fan from a track unit and the wrench slipped, resulting in a cut to the palm of employee's right hand, near thumb area. Employee received five (5) stitches.

Employee was changing the bucket on the skid steer. EE was using foot as a brace while pulling on the bucket to latch it into place. The bucket slipped before it was latched and smashed top of left foot and big toe area.

Taking a block of wood out from under some angle iron on a trailer. Finger got smashed between a piece of angle iron and the trailer. Tip of middle finger on left hand was torn off to the bone(nail was torn off), tip of finger was broken.

Possible stroke.

Employee was working on stacker head section and became entangled in head pulley causing a dismemberment of right arm.

While climbing into a skid loader, felt a stinging sensation in right knee.

Employee was assisting with torquing the rear end of a truck and his ankle gave out. Did not step on anything, shifted his weight and heard ankle pop.

Employee was pushing the feeder back over the HP500 cone crusher. The plate that shuts the bin off had shifted enough to one side that it fell down, hitting employee on back lower part of leg below knee area.

Employee was tightening upper bolts on breaker plates using a 3/4" ratchet in a tight area. The ratchet slipped out of the socket and with the force trying to tighten it, smashed his right hand pinky finger.

Employee climbed off of a ladder. After he was off of the ladder, took a step on uneven ground surface and twisted his ankle.

Employee was using a utility knife to cut open the center strip of rubber on the 8 x 20 screen. Knife slipped and cut his left palm of hand.

Employee was working at left side of desk. Employee turned and stood up and started to walk toward the right when she tripped over the bottom desk drawer. The drawer was closed, but doesn't latch properly and sometimes comes back open.

Employee was cleaning up debris (dirt, dust) around the jaw crusher with a flat shovel for about 20 minutes. The next day he started complaining about back pain. The following day (March 23/2011)he came to work with the same problem and was sent to see a doctor.

Employee was running toward the plant. After taking several steps, he lost his footing and fell hard on his right arm. The right forearm began swelling immediately. The fall resulted in two (2) cracked ribs and deep bruising.

Employee was getting ready to pick up 200 lb shaft with crane when another contractor pulled on sling to tighten it. While employee was looking up at the hook being lowered, shaft rolled off stand and landed on right foot behind steel toe. Other contractor didn't speak up to alert employee.

IE was crawling under gyratory crusher twisting and bending to perform maintenance. Later that night IE noticed knee was swollen. He wrapped in ice. On 4/20/10, IE was working on a jaw crusher and his knee began to hurt again. He noticed fluid build up under the knee cap. On 4/29/10, he went to doctor who drained the fluid and cleaned out area around joint.

Employee hit right shin on metal frame. Resulting injury was about size of a pea. Applied antibiotic and a bandage.On 1/24/10 while showering at home the scab fell off.The next morning area was very red and painful to touch. On 1/25/10, he reported to work but pain became too much. Went to ER, serious infection (cellulitis) had set in. Anitbiotics prescribed.

Employee was walking toward electrical room at plant on upper level when he slipped on a patch of ice that was covered by a thin layer of snow. He landed on his left side. Was transported to ER and was found to have cracked two ribs and had deep brusing.

Plant personnel were loading jaw dies onto pickup truck, for return to Warfordsburg, using a wheel loader with a chain. Employee tried to steady swinging jaw die, when it pushed against straightened left arm and caused cracking sound in left shoulder. A small amount of pain immediately appeared in his left shoulder.

Employee was bent over parts washer rack and was getting ready to lift starter from rack. As he picked up on the starter, he sneezed and felt a pop in his belly area. He has been diagnosed with an umbilical hernia. A 2-ton job hoist is available to be used to lift items out of the parts washer and employee has been instructed to use it more frequently during all lifting.

Miner was working inside piece of equipment getting ready to lift part of it. When he turned around, he hit his right elbow into crane hook that was positioned for the lift. No immediate problem. Several weeks later, his elbow began to swell up and required medical treatment to drain it.

Employee was cutting with a torch when spal caught his left pants leg on fire. Employee's pants had small frays at the bottom of leg. Employee put out fire with hand but received a small burn to left leg near ankle on shin area.

While welding underneath a test stand, spal fell down and landed on employee's leg. He was unable to move in time to get it off. On 1/17/08 employee noticed his leg had become infected because it was a large red area around the burn. Employee went to the hospital and was diagnosed with a staph infection. Employee received antibiotics.

Employee was cutting conveyor belting from a roll to use as a chute deflector. The knife did not cut all the way through, so the employee made a cut by pulling the knife toward him. The knife slipped and cut him in the left knee.

Employe was holding a piece of sheet metal while working on a piece of equipment. He lost his grip and the metal cut his left pinkie finger.

Employee was collecting a test sample of stone and lost control of the sample tray she was holding. The tray came down on her wrist, pinching it against a metal railing. A permanent access platform has been installed for easier access and will be modified to reduce the risk of injury.

While employee was using a 16 lb. sledge hammer he felt a strain in his lower back. The employee has been trained in the proper handling of a sledge hammer to avoid future injuries.

Employee was using a Porta-Power tool which slipped from it's location and struck him in the nose and face.

Employee's hand became pinched between a manlift basket and a crusher flywheel.

A shaft that secures a wheel on a tire balancing machine broke. The EE's finger was caught between the machine and the wheel, causing a laceration and a fracture to the tip of his left ring finger. The machine was tagged out and will be replaced with a larger capacity model. Training is being provided to prevent over-torqueing of shaft.

Employee was doing maintenance work on his loader. He was using a sledgehammer to drive a pin out of a loader tooth. The sledgehammer glanced off the corner of the punch and struck the left hand index finger.

Employee was removing a grease plug from a grease jack on a conveyor belt. The plug blew out and struck the employee on the left hand, causing a laceration to the palm between the third and fourth digits. Currently investigating and procuring bleeder valves that will allow pressure to be relieved prior to removing the plugs.

Employee was "striking an arc" with one hand and holding the welding helmet with the other hand. He did not properly use the welding helmet and received "welder's flash." Employee is being trained on proper welding techniques to prevent any further injuries.

EMPLOYEE WAS DRIVING A HAUL TRUCK UP A HILL WHEN THE TRUCK STALLED. IT BEGAN DRIFTING BACKWARDS AND THEN ROLLED ONTO IT'S SIDE.

EMPLOYEE WAS USING A PORTA-POWER RAN TO STRAIGHTEN A BENT BRACKET ON A CONVEYOR. THE PISTON/RAM CAME OUT AND LANDED ON HIS LEFT INDEX FINGER. HIS FINGER WAS PUSHED INTO AN ANGLE IRON CAUSING A LACERATION.

Employee was performing preventive maintenance on a haul truck. She then experienced severe pain in the lower left side of her back.

EE was using an electric, hand held drill to drill a hole in a broken bolt to get it out. The drill bit caught and twisted the drill and the ee's arm.

EE WAS UNLOADING AN OXYGEN CYLINDER FROM THE BUCKET OF A SKID LOADER ONTO A TRUCK. THE SKID LOADER HAD BACKED UP, BUT THEN PULLED FORWARD AGAIN WHEN THE OPERATOR SAW THAT THE EE AND CYLINDER WERE ABOUT TO FALL. EE'S LEG GOT CAUGHT BETWEEN THE SKID LOADER BUCKET AND THE TRUCK.

Employee was inflating a tire which blew out. The tire struck him in his right lower back area.

Employee was standing next to a skidloader. The skidloader operator began to move the skidloader and ran over the employee's right foot.

EMPLOYEE OPENED AN INSPECTION DOOR APPROX 24" X 24" IN SIZE, IN ORDER TO SEE WHY THE MACHINE WASN'T OPERATING PEOPERLY. WHILE WORKING ON THE MACHINE, THE UNSECURED DOOR FELL ON HIS RIGHT HAND AND WRIST.

EE WAS BENDING OVER TO TURN ON WATER VALVES & TWISTED HIS BACK.

EE WAS SHOVELING MATERIAL OFF A CONVEYOR BELT WHEN HE FELT SOMETHING "POP" IN HIS SHOULDER, CAUSING PAIN & NUMBNESS IN HIS RIGHT ARM.

EE WAS WORKING ON TAILPULLEY SECTION WHEN GREASE SEAL FAILED ALLOWING TAIL PULLEY TO COLLAPSE TO FRAME STRUCTURE PINCHING LEFT HAND.

EE STATED HE WAS NOT AWARE OF ANY SPECIFIC EVENTCONTRIBUTING TO THE ACCIDENT. EXACT CAUSE UNKNOWN.

WHILE REMOVING A PIECE OF ANGLE IRON, EMPLOYEE PINCHED HIS FINGER BETWEEN ANGLE IRON AND STEEL FRAME.

WHILE CLEANING UP TOOLS AT THE END OF SHIFT THE WIND WAS BLOWING HARD & BLEW DIRT INTO LEFT EYE.

EE WAS USING A #20 SLEDGE HAMMER AND FELT PAIN AFTER REPEATED USE.
