Mostrando los 27Employee was operating the 992 Loader. Just as the sun was rising, the employee attempted to clean the window when he fell to the ground, injuring his back. Other operators saw came to assistance and stabilized the employee until outside medical services arrived, who transported the employee to the local hospital for evaluation
Maintenance person removed gloves while attempting to fasten screen above a belt guard. This person caught his hand in the expanded metal screen, cutting the index finger which required 5 stitches. EE released to regular work duty by attending physician (no restriction).
Employee was tightening nut w/socket wrench on expanded metal tail pulley guard. EE removed his glove to perform the task & cut his index finger on the edge of the expanded metal.
EE WAS CLEANING SIDE DELIVERY BELT. ONCE THE MATERIAL WAS REMOVED BETWEEN THE TAIL PULLEY AND THE BELT, THE BELT MOVED IN REVERSE DIRECTION FOR ABOUT 2'. THE EE WAS LYING FACE DOWN ON THE BELT WITH HIS HEAD CLOSEST TO THE TAIL PULLEY. WHEN THE BELT MOVED, IT SLAMMED HIS HEAD INTO THE FRAME OF THE SCREEN.
EE WAS CUTTING A PIECE OF RUBBER BELTING WHEN THE UTILITY KNIFE HE WAS USING CUT THROUGH THE BELTING AND CUT THE UNDERSIDE OF HIS LEFT FOREARM CAUSING A 3/4" LACERATION.
TWO EMPLOYEES HAD THE LID OF THE IMPACTOR IN THE RAISED POSITION TO CHECK OUT A PROBLEM. THE WEDGE SHAPED STOP BLOCK WAS IN ON ONE SIDE. VICTIM WAS ADJUSTING THE BLOCK ON HIS SIDE. HIS LEFT TH UMB WAS PINCHED BETWEEN THE TOP OF THE BLOCK AND THE BOTTOM OF THE HOOD. HE LOST THE THUMBNAIL.
EE WAS INSTALLING A WHEEL SEAL ON THE LT. REAR DRIVE AXLE OF A BELLY DUMP. AS HE WAS HAMMERING ON THE WHEEL SEAL DRIVER, THE HAMMER STRUCK HIS LT. INDEX FINGER. THIS RESULTED IN A LACERATION THAT REQUIRED STITCHES TO CLOSE. EE WAS USING TOO SMALL OF A HAMMER AND MISSED THE SEAL DRIVING TOOL AND STRUCK HIS LT. INDEX FINGER.
EE WAS REPLACING SCREEN CLOTH ON THE #1 WASH SCREEN. HE LIFTED A PIECE OF CLOTH WITH HIS RIGHT ARM EXTENDED FELT A TWINGE IN HIS LOWER BACK ON THE RIGHT SIDE. HE OPERATED A 769 CAT WAGON AFTER WARDS AND THE PAIN BECAME MORE SEVER AND HIS BACK TIGHTENED UP.
EE WAS REPOSITIONING 10" PVC PIPE WHEN PIPE SLIPPED OFF BRACE AND CAUGHT EE'S LEFT HAND BETWEEN BRACE AND PIPE SEVERING MIDDLE FINGER OF LEFT HAND JUST BELOW FINGERNAIL.
EE WAS ATTEMPTING TO FLIP 480 BOX SWITCH TO OPEN POSITION WHEN METAL MET ELECTRICLA LEADS CAUSING FLASH BACK.
EE STEPPED OFF A SEMI-TRUCK WITHOUT USING GRAB IRONS AND FELL APPROX. 2' AGAINST ANOTHER TRUCK'SFUEL TANK INJURING HIS LOWER BACK.
EMPLOYEE WAS INSTALLING A NEW CURTAIN LINER IN THE CRUSHER. THE LINER SLIPPED AND PINCHED THE LITTLE FINGER ON HIS LEFT HAND BETWEEN THE NEW LINER AND AN ADJACENT LINER. AS HE PULLED HIS HAND OUT OF THE WAY, THE END OF HIS LITTLE FINGER WASCUT AND THE NAIL WAS DAMAGED. STITCHES REQUIRED TO TREAT THE INJURY.
REMOVING TIRE FROM VEHICLE AND AIR BLAST GUN BLEW DIRT IN FACE. SMALL PIECE OF METAL PENETRATED EYE.
HAUL TRUCK WAS DUMPING ROCK ON STOCKPILE. AS BED WAS BEING RAISED, TRUCK WENT OVER SIDE OF STOCKPILE, DOWN THE SLOP AND CAME TO REST AT TOE OF PILE. DRIVER WAS WEARIGN SEAT BELT. CONTRIBUTING CONDITIONS INCLUDE DRIVER PROCEEDING TO DUMP WAS TOO CLOSE TO EDGE AT AREA OF STOCKPILE WHERE BVERM HAD SLUGHED. THE TRUCK'S CATWALKS, HANDRAILS, CAB DOOR AND WINDOWS WERE DAMAGED.
EMPLOYEE WAS JUMP STARTING COMPANY VEHICLE, WHILE EMPLOYEE WAS UNHOKKING TRUCK FROM OTHER VEHICLE, THE BRAKES FAILED, TRUCK ROLLED AND EMPLOYEE WAS TRAPPED BRTWEEN THE TWO VEHICLES.
EE CLIMBED CRUSHER TO REMOVE DEBRIS FROM AREA. EE LOST HIS BALANCE FALLING APPROX 15 FT
EE WAS CHANGING A TIRE ON HAUL TRK WHEN HE SAID HE FELT PAIN IN RIGHT GROIN AREA ON 12/28/00 HE DECIDED HE NEEDED TO HAVE SURGERY ON BOTH PREVIOUS HERNIAS BLATERIAL
EE SLIPPED ON STEEP STAIRS WHILE DESCENDING INTO FEEDER TUNNEL. POSSIBLE DEBRIS ON STEPS, EE UNABLE TO RECALL. EE WAS CARRYING A SHOVEL AND WAS NOT ABLE TO MAINTAIN GRASP ON HANDRAIL AS SHE BE GAN TO SLIP.
WHILE REMOVING SKID PLATES ON 988 F LOADER MECHANIC TRIED TO HOLD BOLT FROM SPINNING AND IN TURN CUT RIGHT HAND.
REPAIRING TIRE-REMOVING OBJECT FROM TIRE WITH KNIFE AND CUT FINGER.
EMPLOYEE WAS LEANING AGAINST RAILING ON CATWALK WHEN ANCHOR POST BROKE CAUSING HIM TO FALL BACKWAYRS APPROX. 2 1/2 FT., LANDING ON HIS BACK AND CAUSING A COMPRESSION FRACTURE TO LUMBAR #1.
LIFTING TIRE AND WHEEL FROM GROUND TO STAND UPRIGHT.
LIFTING 46 TRUCK TIRES FROM SERVICE BAY INTO WAREHOUSE, EACH TIRE WEIGHING 100-120 LBS. DELIVERY DRIVER UNLOADING TIRES ON GROUND INSTEAD OF ON DECK. FORK LIFT NOT AVAILABLE AT TIME.
CUTTING CONVEYOR BELT FOR SKIRTING, BELT WAS ON THE GROUND, CUTTING IN PULLING MOTION SLIPPING AT END OF BELT AND PUNCTURED LEG.
