Mechanic employee was using a 30mm wrench to tighten a jam nut on the drill when the wrench slipped off the nut and struck them in the face causing 2 lacerations on their lip and breaking 3 caps on teeth.

When EE lowered the tailgate there is a battery terminal underneath where the tail gate comes down, and EE's left middle finger got pinched between the tailgate and a battery. EE decided to be seen on 7/29/24 and was placed on restrictions.
Employee was checking a battery to a welder while standing on the back step of a Mechanic Truck. When EE leaned the step moved because the pin was not in place to secure it causing EE to fall landing on right side. Seen in ER 4/23/24 no restrictions. Seen again 4/26/24 placed on restrictions.
While lifting the cutting edge to put back onto the loader bucket, the chain supporting the cutting edge broke. The cutting edge came down onto EE's thumb smashing it.
Employee was attempting to operate a 980 loader. As employee was walking around the loader they tripped on a rock causing them to fall on left knee.
Employee was leaving work for the day and was looking up at the hill while walking to car. Employee tripped on a concrete barrier, falling to hands and knees.
Employee had a prior surgery done years back. While lifting 5 gallon buckets the scar where employee had surgery hurt and was red and swollen.
Employee was cutting crown bar rubber with a utility knife and they cut themselves on the upper left leg.
Employee stepped off the bumper of pick up truck to unlevel ground. When EE's right foot came in contact with the ground, EE twisted the ankle and fell on knee.
Employee was helping secure the screen from inside the screen deck. The screen was turned on and began vibrating material onto EE's screen. EE scraped and bruised EE's back as EE was exiting the screen.
Shoveling when ee twisted and heard a pop in back.
Employee was going to go clean up around wash plant, when EE stepped in an area of mud and EE's foot sank about 6-8 inches in the mud, twisting the ankle.
Insect bite on top left foot.
Skid load rolled pinning feet against rock pile.
Worker rolled left ankle when stepping down onto a grouser bar and sprained EE's left ankle.
Employee was shoveling/loading holes for blasting when he felt a pop in his right hand. As time went by it began to hurt - the next day 10.16.12 his thumb began to swell and was difficult to move. Decided at this time to seek medical attention.
This accident is still under investigation and my be revised at a later date. Employee had parked rock truck to help crusher crew. When he stepped on the second to last step he slipped and fell to the ground into his back. He did not wish for medical attention at that time. On 1/20/12 he decided to see a doctor.
After the morning walk around with safety chare and tool box meeting the crew then completed stretch and flex. The crew was then instructed to change out used screens on the screen deck. When the last bolt was being tightened is when employee felt a sharp pain in his lower back.
This is an "Alleged Injury", this report may be amended apon further investigation. Employee was checking/cleaning spray heads when he slipped in some mud and fell onto right hip. Did not report this incident to management until 1/25/10.

This is an "Alleged Injury", this report may be amended upon further investigation. EE was changing bearing on conveyor, feeder was locked out, taged out, along with conveyor. EE was crawling out of feeder, when he took off hard hat and a rock flew from secondary feeder and hit him in the head.
This is an "Alleged Injury", this report may be amended upon further investigation. Injured was removing a pin from a tooth on a trackhoe bucket. Hitting the pin with a sledge hammer a small piece broke off and hit him in the left inner thigh. Causing a small cut/burn. Did not report or ask to be taken to a doctor until 2/2/09.
Checking voltage at an electrical plug marked voltage meter prong across 2 terminals causing brite flash, employee was looking right at it, not turn in to reporting until 10/11/08, tried to file online 10/11/08 web site down til monday 10/13/08
Employee was looking for a piece to his grease gun that had fallen into a pile of scrap metal. When he lifted a piece of metal, it caught his glove and caused him to drop the metal on his hand which cut his right 3rd and 4th fingers requiring sutures.
Employee was removing a 15 Hp motor from a conveyor belt, when the motor slid and hit him on the right arm.
Employee was shoveling around the AC screen deck in very windy and dusty conditions and something blew into his left eye. He did not seek medical attention until 11/3/06.

Employee was welding back to back with another employee. The reflection from the other welder bounced off the inside of her hood burning her eyes.
EE WAS CUTTING THE CONVEYOR BELT WITH A CARPENTER KNIFE. THE KNIFE SLIPPED AND CUT THE PALM OF HIS LEFT HAND, REQUIRING SUTURES.
EE WAS TRYING TO OPEN PRIMARY JAW WITH BORROWED PORTER POWER WHEN ADJUSTER BOLT HOLDING TWO SUPPRESSED SPRINGS BROKE AND THE PLATE HOLDING THE SPRINGS HIT HIS RIGHT LEG. WAS GIVEN A RESTRICTE D DUTY RELEASE ON 2/4/04 WHICH WAS WITHIN HIS REGULAR JOB FUNCTIONS. HOWEVER, HE RETURNED TO THE DOCTOR ON 2/10/04 AND WAS GIVEN A RELEASE THAT WAS NOT WITHIN HIS NORMAL DUTIES.
PERSONNEL WERE CHANGING A WEAR PLATE ON A CONVEYOR EE CRAWLED UNDER TO OTHER CARRIAGE BOLTS FRO LAGGING. EE DROPPED A WEAR PLATE STRIKING EE BELOW. APPROX. 50 LBS DROPPED 3 FT.
EMPLOYEE WAS EXITING A CASE SKID LOADER AND STEPPED ON THE EDGE OF THE BUCKET, TWISTING HIS LEFT ANKLE.
EMPLOYEES WERE ATTEMPTING TO SWING THE LID CLOSED ON A ROCKEATER USING A ROCKBAR. THEY HAD BEEN INSTRUCTED TO CLOSE IT BY MECHANICAL MEANS ONLY. WHILE PUSHING ON THE BAR, IT BROKE AND BOTH EMP LOYEES FELL APPROXIMATELY 10' TO THE GROUND. ONE LANDED ON HIS FEET AND HAD NO COMPLAINTS OF ANY INJURY. THE OTHER SCRATCHED THE LEFT SIDE OF HIS FACE AND UPPER BACK STRAIN.
EE OPERATING A CAT LOADER PULLED UP TO THE FEEDER BIN & DUMPED. BEHIND HIM GOING TO HIS LEFT WAS A 2ND LOADER, WHICH PULLED IN TO THE BASE COURSE PILE. THE INJURED OPERATOR SAW THE OTHER LOADE R START TO BACK UP & APPLIED HIS HORN.THE OTHER LOADER STOPPED QUICKLY BUT BUMPED THE LOADER AT HIS RIGHT REAR STEPS, BENDING THE BRACKET. INITIALLY DENIED TREATMENT BUT LATER REQ. MED.9/25/0
EE WAS ATTEMPTING TO UNCLOG/DISLODGE A LARGE CLUMP OF BASE COURSE IN A FEEDER BIN. HE WAS SUING A PRY BAR AND WAS PULLING AND PUSHING AGGRESSIVELY WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.
EMPLOYEE WAS OPERATING A DOZER AND GOT A LITTLE EXUBERANT. IN HIS HASTE HE DID NOT TAKE BREAKS TO REST HIS MUSCLES AND THE NEXT DAY DISCOVERED HE WAS SORE. HE DID NOT NOTIFY EMPLOYER UNTIL 7-2 9-03 AND WAS TAKEN TO SEE A PHYSICIAN ON 7-31-03.
EE WAS SERVICING EQUIPMENT AND DUST/ DEBRIS IN HIS LEFT EYE. SUPERVISOR FLUSHED EYE ON 7-1-03. ON 7-2-03 THE EYE WAS STILL BOTHERING EE AND WE TOOK HIM IN FOR TREATMENT 7-2-03.
EMPLOYEE, CHANGING ANVILS AND SHOES ON FEED DISK OF ROCKEATER AT CRUSHER, STRAINED HIS BACK AND NECK.

EE WAS LAYING ON HIS BACK REMOVING BOLTS FROM SKHAKER SCREENS USING AN AIR IMPACT WRENCH. THE EE HAD ON RX GLASSES WITH SIDE SHIELDS. DIRT FELL ONTO HIS FACE AND GLASSES AND EYES. HE ROLLED OV ER TO CLEAR THEM & WENT BACK TO WORK. CAME IN THE NEXT DAY WITH HIS LEFT EYE SWOLLEN, RED AND TEARING.
ON NOV 21, 02 THE EMPLOYEE WAS TOLD HE WAS GOING TO BE LAID OFF ON 11/22/02. HE THEN STATED THAT HE HAD INJURED HIS SHOULDER SHOVELING ROCK AT THE CRUSHER ON 10/31/02 AND HAD NOT REPORTED IT PREVIOUSLY.
EE WAS HITTING SPLITTER BOX WITH SLEDGE HAMMER AND TWISTED HIS BODY AND FELT SOMETHING POP.
AFTER LOGOUT/TAGOUT THE GUARD WAS REMOVED TO GET TO ROLLER. A COMEALONG WAS USED TO HOLD THE BELT IN PLACE. WHEN USING AN IMPACT TO REMOVE A BOLT, THE BOLT SUDDENLY CAME LOOSE AND THE TENSION ON THE BELT CAUSED IT TO JUMP, SLAMMING THE EE'S HAND BETWEEN THE IMPACT AND THE BOTTOM OF THE FEEDER.
EE WAS STANDING ON A ROCK TRYING TO DISLODGE SOME ROCKS FROM THE CONE, THE ROCK HE WAS STANDING ON ROLLED CAUSING HIM TO FALL TO HIS RIGHT KNEE.
