Employee hit knee on "C" clamp while being freed from material spillage. Employee also hit head on plant.
Employee was pressing a sleeve out of crusher with a port a power. After pressing it out, employee began disassembling port a power. During this time the steel cross bar that was holding the port a power fell and struck EE's finger.
Employee reached up through a guard to check belts shortly after the machine turned off. The machine was still in motion and the belts took the employee's fingers through the pulley.
Employee was walking to car at the end of day when employee lost balance and fell.
Employee was loosening bolt on a screen. While trying to keep the nut secure, the nut turned and pinched EE's thumb resulting in a small laceration.

Installing a hydraulic hose in the D6R cab tech needed to tighten the hose further. EE bent, scooted further down. One wrench in right hand and one in the left. EE pushed down on the left wrench while holding up on the right hand. Tech stood up felt a tight cramp above belly button. EE thought pulled abdominal muscle. Later saw bulge under belly button.
Employee was testing the operation of a crank on a winch. While testing the handle came out and struck the employee.

Performing engine repair on 992C Wheel Loader. Cleaning air filter with sanding pad attachment. Piece of metal flew into Left eye. Eye became irritated next day. Given Rx and referred to ophthalmologist 7/13.
While climbing down a ladder on the plant, the employee slipped and fell into the ladder on the right side.

When #1 took hold of part, #2 let go of part and the unexpected transfer of weight caused #1 to strain upper back muscles.
Employee was cleaning out a feeder shoot on the plant. While clearing out a tree root, employee brushed up against the shoot and cut arm.

The employee leaned over the side of the step ladder, it then tipped over, causing the employee to fall to the ground.
The maintenance crew was moving a conveyor up the ramp to the top of the tunnel stockpile area. A Kobelco LK 300A wheel loader was used to move the conveyor up the ramp. The chain used to lift the one end slipped through the eye in the bucket, causing the loader to overturn on the employee who was walking on the left side of the machine.
The truck owner was replacing a bolt in a pony axle. The owner had a bottle jack under one axle tire and a wooden block under the axle tire beside it. The owner then lowered the axles from the cab, positioned himself on a creeper between the 2 raised axle tires. The jack came out from under one axle tire; crushing the owner.
EE WAS DISASSEMBLING THE FMC MF800 VIBRATORY FEEDER. THE EE WAS INT HE PROCESS OF REMOVING THE FASTNERS FOR THE SECOND COUNTERWEIGHT WHEN THE FIRST COUNTERWEIGHT SPUN ON THE SHAFT. THE EE S RI GHT THUMB WAS PINCHED BETWEEN THE TWO COUNTERWEIGHTS.
EMPLOYEE WAS WORKING WITH EMET GUTIERREZ CHANGING A HOIST CABLE ON DB-04C AN 80D NORTHWEST CRANE. THE EMPLOYEE WAS ATTEMPTING TO HOLD THE 1 1/8"CABLE WHILE IT WAS BEING INSTALLED IN THE HOIST DRUM OF THE CRANE. IT CAME FREE STRIKING THE EE IN THE FACE.
EMPLOYEE WAS EXITING THE CAB OF THE FUEL TRUCK FT-03C, WITH AN UNPROTECTED RAZOR BLADE IN HIS RIGHT HAND. AS HE BEGAN TO CLIMB OUT OF THE CAB, HIS RIGHT HAND STRUCK HIS LEFT HAND WITH THE RAZO R BLADE.
EMPLOYEE WAS REMOVEING A 4 LB. HAMMER WHICH WAS USED AS A BLOCKING DEVICE ON THE 6'X14' SCREEN DECK. THE EMPLOYEE'S LEFT MIDDLE FINGER WAS PINCHED BETWEEN THE SCREEN DECK FRAME AND THE HAMMER HANDLE.
