Employee was placing beams on a slab at well #3. The sling slipped off the hook and pinched the employee's finger. This resulted in an avulsion to his left index finger requiring 11 sutures.
Employee was using a pick to remove clay that built up between the grinding teeth of a milling machine. When the pick accidentally struck one of the teeth on the machine, a sliver of metal hit the employee on the chin. Over the next 5 days the area began to fester until he was taken to the doctor for observation.
Employee was repairing/troubleshooting a water truck. He was positioned around the engine compartment. A coworker started the engine and two of his right-hand fingers were hit by the fan blade.
EE WAS USING CUTTING TORCH TO TRIM BEAMS UNDER SCALE-A PIECE OF SLAG FLEW INTO EE RIGHT EAR. SLAG WAS MOLTING HOT. THE EE HAD TO LAY ON HIS SIDE IN ORDER TO PERFORM THE JOB.
WHILE BREAKING LARGE BOULDERS WITH A PECKER ON A BACKHOE, SOME OF THE SHOT ROCK SLID OVER ONTO THE CAB OF THE BACKHOE, TRAPPING THE OPERATOR INSIDE.
EE WAS LIFTING ACCETANLYNE BOTTLE FROM TRAILER MOUNTED AREA TO TRUCK MOUNTED AREA. BOTTLE SLIPPED AND EE TRIED TO CATCH/OR STOP FALL OF BOTTLE.
