Man loading dump truck from stockpile. Did not set parking brake or chock tires. Truck rolled down hill 300 ft and over 10' embankment.
Unknown at this time. EE was terminated on 7/7/08 with no report of injury. After short term disability benefits expired, former alleged that he was hurt on the property. We were first notified of this event on 8/20/08 by his attorney---no injury occurred on the property.
Electrical outlet shorted out igniting wood interior wall. Fire dept. responded, extinguished fire. Gates were locked; no miners present at time of fire. Approx. time fire dept. was on scene 1 hr 10 min.

EE was helping install the kiln cover back over kiln after repairs was completed. he reached up to help unhang cover and the beam that ee was holding on to fell hitting him on left leg.
Wet and muddy, EE was filling up machine with water. He was climbing on machine. Went to move a 5 gallon jug of water to the next level of catwalk. His hand slipped off of handrail, he fell against machine bruising his ribs and his leg hit the tracks lacerating his leg.
Employee was fueling D-7 Dozer, when he slipped and caught is finger in the basket, causing a fracture.
