Employee was working on the Silo spout 7B, was in man lift with another employee untangling cable. Started to get cable untangled and used spud wrench to turn motor until cables untangled. When they untangled it activated the slack sensor and motor was energized and began to spin spud wrench cutting employee left arm top of wrist 3" down

Employee was Rescued via Emergency Ladder within Seconds. No Immediate Signs of injury at the time of the Accident, Employee was feeling soreness the next day and taken for Medical Evaluation 2 days after the incident
While Opening a Rotex Door Employees hand was pinched by the latch of the door causing a laceration to the left Ring finger.
Employee was driving an articulated haul truck leaving the pit when they drove over a bump in the road causing them to hit their left arm on the arm rest. The contact resulted in the employee feeling pain and numbness in their left arm.
Employee was opening and closing lids on customer trailers, and reported knee pain with no known cause.
Employee operated skid steer for 7 days from 12/05 to 12/11 and complained of knee soreness at the end of the week. Reported to plant on 12/23 that the knee was sore. Employee was sent to the doctor on 1/6/25 after some swelling occurred and was put on restricted duty. No strain or sprain identified during tasks by employee.
Employee was clearing a waste chute on Rotex 314 and hit their middle finger on right hand with a big bolt being used as a hammer.
While cleaning around the bucket elevator, employee tried to step back, when employees leg slipped, and EE felt a pop in their left knee.
While working on the Dryer 4 Platform Employee went to light a cigarette and Employee's right glove caught on fire leading to severe burns on hand.
Employee was performing the pre-operational inspection of the Komatsu 155AX dozer. Employee's foot got caught in the tracks of the dozer causing EE to fall off the tracks and fracturing their ankle.
While on a ladder beneath double dump on bag house 3, cleaning with one hand, employee suddenly felt a sharp pain around mid back region. Medical treatment diagnosed with thoracic muscle spasm, given 2 days off work, and light duty restrictions.
Employee was walking to get the dry plant sample, tripped and fell and hit elbow and was in moderate pain, didn't need medical assistance at that point. On 6/1, after sleeping, employee complained of swelling and pain, medical treatment with orthopedic follow-up to evaluate shoulder sprain or possible rotator cuff injury. Not permitted to return to work until 6/5.
Employee 1 (E1) attempted to separate two employees that were in a confrontation, E1 fell down the steps of the E-House injuring EE's ankle. E1 declined medical treatment. On April 21, E1 was unable to put on EE's work boots due to swelling and requested medical treatment. Injury was diagnosed as a broken ankle.
Employee was on SafeRacks opening and closing hatches during rain. Employee got on top of a box truck to close a box lid and slipped causing rib/stomach pain.
dry-plant employee (E1) was working closely by another employee (E2) using the air wand when the air wand slipped out of E1's hand, E2 attempted to control the wand and compressed air line, the wand and line became disconnected and the line struck E2 in the back of the head/neck area.
EE slipped on a water bottle and dislocated thumb when trying to catch themselves.
Employee was using a 36" pipe wrench to close the isolation wheel valve of DWS108. EE had the pipe wrench placed on the top of the wheel at the 1:00 position and began pushing the pipe wrench counterclockwise to close the valve when the pipe wrench slipped off of the wheel causing EE's left pinky finger to get smashed between the pipe wrench handle and the adjacent I-Beam.
Employee attempted to lower the suspended counterweight. The chainfall failed causing the counterweight to fall & strike the employee in the right shoulder.
Employee was cleaning the oversize protection on BE306, as they were putting the screen back in place it caught on something, and hit employee's lip, causing them to cut their lip
Employee was replacing the guarding on the top of BE307 and when they picked up a 5 gallon bucket and turned to walk back to the man lift. Employee stepped on a race from one of the bearings that had been replaced and fell hitting right leg on some guarding.
Employee was loading trucks in the silos and caught the toe of EE's shoe and sprained EE's knee.
Employee was installing a new belt on the 410 conveyor. Employee was manually rolling the gear box. While doing so they pinched the tip of left middle finger between the drive belt & pulley.
EE was walking and turned back to talk to another employee and tripped over forks of forklift using hand to catch fall. Wrist was not broken just had splint put on the sprained wrist.
EE was grinding off a seized bearing and a piece of metal got into EE's left eye. The metal was removed at a medical facility with no further treatment needed.
On 11/29/2020 Load-out operator was opening hatches on trucks and felt something pull on EE's left side. The operator stated EE did not need medical care at the time of the incident. On 12/7/2020 the operator decided to seek medical care and I met EE on 12/8/2020 at the clinic. The doctor took x-rays and the operator had 1 fractured rib
Employee had worked night shift. Had already clocked out and was about to head home when employee stopped into building by parking area to grab a burrito. Employee tripped on flat ground and fell into the stairs. Catching forehead on step, and cracking bone in left wrist while bracing for fall.
Employee was hooking cable on the crane and turned to look at the crane operator. When the employee turned back the headache ball hit the employee and chipped the front tooth.
Employee cutting bearing with grinder, cutting wheel. Wheel broke and piece flew off, cutting employee on right forearm in two places. Requiring stitches.
2-26-20 employee struck elbow on hand rail. 3-4-20 elbow swelled, prognosis Bursitis. No restrictions. 3-19-20 elbow infected. To Doctor. 3 prescriptions and work restriction until 4-1-20.
While shoveling accumulated sand at the base of the Primary A hopper, employee slipped and fell backwards striking back of head on steel beam. All PPE was in place. Hard Hat flew of as slip occurred. Laceration to back of head.
While determining which troughing rollers to be replaced, employee caught hand and arm in moving conveyor belt, causing abrasion to left hand and broken left arm. Employee transported to Emergency Room
Employee was attempting to remove a large nut on SCR 311 in order to adjust the hanger arm with a 36" pipe wrench. Nut would not break loose so second employee came in to help. Both employees were pulling on pipe wrench when nut gave way, causing second employees thumb to be bent backwards, injuring tendons in thumb.
Employee did not report until 12-6-2019. Employee was welding in a closed area on 12-5-2019 with 2 other welders. Back flash while welding, reflecting off walls caused irritation to eyes. Employee did not realize any eye irritation until the next day, then went to Doctor. Eye drops were given to employee. No shift missed. Employee had all required PPE.
Employee clocked in and returned to personal car to retrieve back pack. Using a pocket knife, employee was attempting to cut a zip tie off back pack. Knife slipped cutting left index finger. Initial first aid was preformed, however, bleeding continued and employee was transported to ER where stitches were received.
Employee was walking down a stairway, tripped and fell, spraining right knee. Took employee to ER. Some swelling, no damage to knee other than sprain. Put knee in brace. Use crutches as needed. Stairs were clean and dry. No obstructions, well lighted.
EE attempted to mount the water truck & failed to obtain 3 points of contact. EE placed right hand on the handrail, then attempted to place right foot onto the lowest rung of the ladder. EE's foot missed the ladder rung causing EE's body to fall forward swinging to the left. EE struck left ear causing a laceration resulting in 25 stitches.
Failure to LOTO while unbinding an auger at the lime silo. Employee applied a pipe-wrench and failed to remove it when the auger was being bump tested. Employee was struck on the left side of face causing an open fracture of the jaw.
EE was cutting a zip tie with pocket knife and cut EE's left thumb requiring 3 stitches
Employee received a small puncture and laceration to the 4th and 5th fingers of the left hand while touching a piece of metal with bare hands.
Employee was struck in the eyelid by a metal shard which was created by striking metal on metal. Employee was prescribed a medication for the eye to prevent injury. Returned to work with no restrictions.
