Miner was walking around his loader when he twisted his ankle. Injury was initially a first aid injury. On 11/15/2008 the miner left work early, telling his supervisor that his ankle hurt. He went to the emergency room and was told to take off work through 11/17/2008.

Seen by doctor & released to regular duty that day. 03/27/2008, employee back spasm getting into vehicle. Dr states result initial incident. Instructed by doctor stay home from work Friday, 03/28/2008. Employee returned work Saturday, 03/29/2008.
Miners were adjusting the timing of gates on the Lambert Palletizer Model 1200. Miners must be on equipment while in operation to make these adjustments. The guard had been removed from the left gate. The effected miner's left foot was pinned between the gate and the shaft when the gates were opened.
Miner states that he heard a load "squall". He shut off the belt to go see what was making the sound. He saw fire and smoke coming from the head box of the #4 belt. He attempted to fight the fire, but the fire grew. He called 911 at 2:10 pm. The fire was put out by the Brady Fire Department around 3:00pm.
Miners was lifting a damaged sand bag from a pallet, twisted and felt sudden pain in is back.
Miner was changing return idler under P2 conveyor when foot slipped causing body to turn and strain muscle in lower back.
Emplyoyee was walking under the #3 6x16 screen deck hit hard hat on the overhead water valve.
Employee was clearing rocks off catwalk with shovel. He raised the level of the shovel over the edge of the collector box, about 5 feet and strained muscle in left side of back.
EE was helping replace a bearing on a speed sensor on a conveyor that was at ground level. He was walking up a small (approx. 4 foot) mound of material. His right foot slipped and he fell to same level. He struck his right shoulder on the ground, causing a right side shoulder sprain.
EE was cutting conveyor belting with utility knife. The knife slipped out of work and cut EE's left thumb. EE received sutures in thumb.
Employee was exposed to welding flash when working on outside of sand screw tub while maintenance welders were working inside tub. The employee received welding flash burn injury to to eyes requiring employee to miss one day.
Employee was operating trackmobile. He stepped down to the ground and twisted his right knee. Note: This report is being file because employee has sought medical attention and has not returned to work.
Employee was spotting railroad hopper cars using a track mobile. He jumped down from the top rung of the ladder to exit the cab and fell backward when he hit the ground. He injured his vertebrae.
EE WAS USING A UTILITY KNIFE TO CUT CONVEYOR BELTING MATERIAL TO USE AS A LINER FOR A BELLY PAN. EE WAS PULLING THE KNIFE THROUGH THE MATERIAL WHEN IT SLIPPED OUT OF THE CUT AND WENT ACROSS HIS RIGHT UPPER LEG. EE RECEIVED STITCHES AND RETURNED TO WORK.
EE WAS OPERATING BAGGING MACHINE FROM A SEATED POSITION. HE ROSE TO STEP DOWN AFTER FINISHING A PALLET OF SAND BAGS. HE DID NOT PLANT HIS RIGHT FOOT SQUARE AND ROLLED THE ANKLE AS HE PUT HIS WEIGHT ON IT, CAUSING A SPRAIN OF THE RIGHT ANKLE.
EE WAS INSTALLING FLEXIBLE ELECTRICAL CONDUIT ON A JUNCTION BOX. THE INNER METAL CLADDING WAS EXPOSED AND EE'S HAND SLIPPED, CONTACTING THE SHARP EDGE. THE METAL CUT THROUGH THE LEATHER GLOVE AND CUT EE'S LEFT MIDDLE FINGER. EE RECEIVED 5 SUTURES.
Employee was driving a cat 771 haul truck and felt pain in upper back the following day.
EMPLOYEE WAS USING A 24" CRESCENT WRENCH TO LOOSEN A RUSTED TURNBUCKLE THAT WAS SECURED IN A BENCH VISE. AS HE PULLED BACK ON THE WRENCH HANDLE WITH BOTH ARMS, THE WRENCH CAME OFF THE TURNBUCKLE CAUSING THE EMPLOYEE TO LOSE HIS BALANCE AND TURN HIS LEFT ANKLE. THE LEG FRACTURED AT THE BASE OF THE LEFT FIBULA.
THIS IS A REPORT OF POSSIBLE OCCUPATIONAL INJURY. ON 8-16-2004 THE MINER RECEIVED NOTICE THAT A CT SCAN INDICATED EARLY STAGE SILICOSIS. THE FILING OF THIS FORM 7000-1 IS NOT TO BE CONSTRUED AS ADMISSION BY OPERATOR THAT AN OCCUPATIONAL RESPIRATORY ILLNESS HAS OCCURRED.

EMPLOYEE WAS WALKING ALONG THE TOP OF THE RAILROAD CAR WHEN HIS LANYARD HUNG UP ON A RAILING PULLING THE EE BACK & CAUSING HIM TO FALL. EE WAS BRUISED & SCRAPED ON LEFT ARM.
Employee was standing on top of an enclosed rail hopper car straddling the top hatch door. He stopped over door and lifted it open with both hands injuring his right elbow.
MAINTENANCE CREW WAS INSTALLING A NEW RUBBER PIPE. HE OFFERED TO HELP AND POSITIONED HIMSELF SITTING ON TOP OF THE 20-40 BIN, GRASPING THE 8" PIPE AT THE BOTTOM. WITH 3 OTHER WORKERS THEY AT TEMPTED TO RAISE THE VERTICALLY-ORIENTED PIPE, BUT REQUIRED ADDL. POSITIONING TO FIT IT ONTO THE BIN FLANGE. AS THEY LIFTED, EE FELT A "POP" IN HIS SHOULDER AND A SEVERE PAIN.
MAINTENANCE CREW HAD REPLACED A 480 VOLT TRANSFORMER AND ELECTRICIAN WAS RE-ENERGIZING THE SECOND BANK OF TRANSFORMERS ON A 3 PHASE 480 V SYSTEM AT THE SAME TIME MAINTENANCE CREW LEADMAN (VICTIM) WAS TESTING THE FIRST 3 PHASE 480 V BANK OF TRANSFORMERS. TRANSFORMER BANKS WERE ISOLATED CIRCUITS. UNEXPECTED SHORT IN METER CAUSED AN ELECTRICAL ARC CAUSING BURNS TO HAND.

EE WAS ASSISTING MAINTENANCE CREW REPLACE BEARINGS ON APUMP SHAFT. THE SHAFT WAS BEING REPOSITIONED WHEN IT FELL APPROX. 8 INCHES STRIKING EE LEFT HAND INJURING FORE AND MIDDLE FINGERS.
EE WAS WORKING WITH CREW REPLACING SCREEN IN WET TOWER. AS HE PUSHED THE SCREEN INTO POSITION HE FELT A TWINGE IN UPPER RT. THIGH/HIP CAUSED BY PULLED OR TORN HAMSTRING.
EE REMOVED LOWER DOOR ON ELEVATOR TO REMOVE SAND BUILD UP. EE LIFTED THE DOOR AWAY FROM OPENING AND AS HE SET IT DOWN A METAL SNAG CAUGHT HIS FINGER SNAGGING AND TEARING THE SKIN ONT HE INSIDE DISTAL PORTION OF HIS LEFT INDEX FINGER.
EMPLOYEE WAS ASSISTING MAINTENANCE CREW INSTALL CONVEYOR BELT SCALE. HE WAS LIFTING HEAVY OBJECTS INCLUDING TROUGHING ROLLER SETS. EMPLOYEE NOTICED PAIN IN LOWER BACK TOWARD THE END OF THE SHI FT. HE SOUGHT MEDICAL ATTENTION AND WAS UNABLE TO RETURN TO WORK THE FOLLOWING WORKDAY.
EMPLOYEE WAS CARRYING A FIVE GALLON WATER BOTTLE TO THE WET PLANT CONTROL ROOM WHEN HE STEPPED ON A 3" DIAMETER WATER HOSE THAT WAS BEING USED TO CLEAR MATERIAL BUILD-UP OFF SCREENS. HE TURNED HIS ANKLE CAUSING A SPRAIN.
EE WAS BRUSHING SCREEN WHEN FOOT SLIPPED CAUSING HIM TO TWIST BACK.
EE WAS REPLACING CENTER PIN ON 988 CAT LOADER. USING CUTTING TORCH TO CUT CENTER PIN THAT WAS STUCK IN A BEARING. HEAT FROM TORCH CAUSED GREASE IN BEARING TO EXPAND AND IGNITE SPRAYING EE WITH BURNING GAS. EE SUFFERED BURN ON FACE FROM HOT GREASE.
EMPLOYEE WAS WORKING INSIDE A WET SCRUBBER TUB USING A CUTTING TORCH TO CUT AND PATCH HOLES IN TUB.
DRIVER WAS IN THE PROCESS OF UNLOADING HIS TRUCK WHEN HE STUMBLED, FALLING AGAINST THE FENDER OF THE TRUCK TRACTOR. POSSIBLE TRIP OVER SLIGHT RISE IN DECKING AT DUMP DRIVE OVER. FATIGUE.
WHILE UNLOADING ANFO EMPLOYEE BENT OVER AND LEANED DOWN TO HIT THE LOWER PADS ON THE TRAILER TO LOOSEN THE ANFO WITH RUBBER MALLET. AS HE STEPPED BACK TO STAND UP HE STEPPED ON A ROCK CAUSING HIM TO FALL BACK AND TWISTING HIS RIGHT ANKLE. HIS ANKLE STARTED TO SWELL AND TURN PURPLE.
EARLY MORNING DARKNESS AND DEW CONDENSATION. EE WAS EXITING LOADER TO PICK UP PERSONAL RADIO AT EE DAY ROOM. HE STEPPED OUT ONTO DECK TURNED TO DESCEND LADDER & SLIPPED ON OILY GRIME SLID OFF LADDER HITTING LEG, LOST GRIP ON HAND RAILS AND FELL TO GROUND. LANDED ON LEFT SIDE CAUSING AN INJURY TO SHOULDER.
