Mechanic was opening new boxes of dust collector cages, as he was pulling the cardboard box out from under the cages the back of his hand struck the top band of the cage and cut his hand.

Mostrando 50 de 359Temporary Agency employee, reported that he had squatted down on one knee to lift a tire to its side so the tire could be rolled into position for disposal. In doing this, ee stated that he pulled a groin muscle. Employee reported his injury and then went home due to his pain. Later employee followed the Temp Agency policy and reported to their doctor for medical inspection.
WORKING ON SETTING MOTOR ON F2 HES SEPERATOR, EMPLOYEE TWISTED HIP AND POPPED HIP JOINT. EMPLOYEE WENT TO THE DOCTOR AND RECEIVED MRI ON JULY 19. WE'RE AWAITING RESULTS FROM MRI AND THE DOCTOR. THE EMPLOYEE CONTINUED TO WORK WITH NO MISS TIME. HE IS EXPERIENCING SOME TINGLING PAIN.
Employee was shuffling ball mill liners inside the mill when a liner shifted and smashed the tip of his pinky finger.
While assisting other co-workers with changing silos,the co-workers were using tools to releive the valve when a piece of metal flew off and went into the employees right forearm causing a laceration to his right forearm. Although this occured 8/28/06 there was no medical treatment other than X-Ray until 9/14/06, when metal was removed from right forearm.
While decending #34 conveyor belt walkway, employee slipped on debris/material builtup on walkway. Later, EE decided to have the knee checked due to swelling. EE visited and received an MRI on 4/17/06. As a result surgery was scheduled and performed on 4/20/06. EE is currently recouperating at home after a surgery to repair a tear.
Employee was shoveling in rail pit and felt something pull in the groin area. Employee was just recently diagnosed with a hernia condition and had surgery on March 6, 2006. He is expected to return to work on March 20, 2006.
Shift in hearing baseline.
While working on the 2nd step of a 6' step ladder, I began climbing down the ladder and missed the 1st step of the ladder and fell to the floor. When I landed on the floor, I struck the floor with my left elbow, causing me to jamb my left shoulder.Although this accident occured 1/13/06, there was no reportable medical treatment until 9/14/06.
The Employee was using a hand grinder on a conveyor belt when a wheel came off the conveyor belt hitting him and causing a laceration to the left middle finger.
Removing chunks from behind the #3 LSI feeder weigh belt material depth gate. The gate has to be pryed up to remove chunks. Employee was prying up the slide gate and slipped off the lip of the gate causing his hand to drive into the bin bottom slide cut off gate smashing and fracturing his right little finger tip.
Employee was operating a loader. The right side window was open and employee was backing up and the right window slammed shut, breaking the glass. He then parked the loader noticing that he received a laceration to the right arm from the glass.
While cleaning rock from barge that the crane had missed or could not get. The employee was operating the skid steer loader when it caught on a weld in the barge and jolted the employee sideway causing employee to hit the right side of head on the R.O.P.S. cutting employee's right ear.
The employee was climbing a ladder to install a hose in an oil tote when he lost his balance and fell backward. The employee experienced swelling in his left wrist and hand/arm as a result. On 8/15/05 additional testing determined that there were 2 small fractures and they put his wrist and hand in cast. Report was delayed due to final diagnosis.
Employee was starting a 2" gasoline powered water pump, when pump "kicked back" causing the pull cord to jerk, pulling the employee's right hand. Employee's hand sruck the pump causing a laceration between the index and middle finger on right hand, requiring medical attention, and resulting in 10 stitches.
Employee bumped head while walking in low headroom area. Employee stooped down to go under an "I-Beam" , raised up and hit head on 2nd "I-Beam". Stiff -sore neck resulted. Employee went to physician 7/29/04.
Employee was installing V-Belts, on a screw conveyor, when screw conveyor started and tried to run, pulling employees left index and center fingers between the V-Belts and the drive sheave. Employee was taken to hospital and found he had both fingers broken, lacerations, and missing finger nails from both fingers. Employee was treated and released from hospital.
EE FELL FROM LADDER, LANDING ON RIGHT SIDE ARM, RIBS, HAND AND TAILBONE.
EE WAS STANDING BENEATH SMALL HOPPER (EMPTY). THE HOPPER WAS OPEN @ THE BOTTOM & WAS BEING RE-LINED IN PREP. FOR REFRACTORY. EE WAS HOISTING EQUIMENT INTO HOPPER FR. BOTTOM HATCH USING CHAINFALL WHEN SOME LOOSE REFRACTORY DUST THAT WAS SITTING ON SCAFFOLD PLANKS FELL ONTO HEAD & NECK AREA. SOME DUST FELL BEHIND HIS SAFETY GLASSES & INTO HIS EYE.
WHILE WORKING IN #2 COOLER 1ST. SECTION A GUST OF WIND BLEW DUST IN THE COOLER, AND INTO EYES.
EE WAS CARRYING A COME-A-LONG TO THE JOBSITE, STEPPED ON UNEVEN CONCRETE, TWISTED RIGHT ANKLE.
EE WAS DOZING CLINKER INSIDE #3 CLINKER SILO IN POOR VISIBILITY. DOZER SLID INTO FEEDER, COMING TO A SUDDEN STOP, THRUSTING EE FOREWARD, CAUSING HIM TO HIT HIS HEAD ON DASH/MIRROR OF DOZER. EE WAS NOT WEARING SEAT BELT PROVIDED IN DOZER. EEWAS TAKEN TO MASSAC MEMORIAL HOSPITAL EMERGENCY ROOM, AND RECEIVED 11 STITCHES IN FOREHEAD, LEFT SIDE.
EE WAS MOVING A PALLET OF BRICKI. FORK SLIPPED CAUSING FINGER TO GET CAUGHT BETWEEN FORKS AND BRICK RESULTING IN A CUT TO EE RIGHT MIDDLE FINGER.
EMPLOYEE WAS CHANGING VESSEL COVERS ON TOP OF AIRSLIDE. HE STEPPED OFF GRATING APPROX 18" ABOVE AIRSLIDE ONTO THE AIRSLIDE (UNEVEN SURFACE) AND ROLLED HIS FOOT. EE WAS GIVEN AN AIRCAST TO WEAR INSIDE HIS WORKBOOT AND RETURNED TO WORK.
PARTIALLY BENT OVER REMOVING AN ITEM FROM A 2 DOOR CABINET IN THE MAINTENANCE SHOP. ANOTHER EMPLOYEE APPROACHED FROM BEHIND, PLACING HIS HANDS ON HIS BACK TO GET HIS ATTENTION. BEING IN AN AWK WARD POSITION, EMPLOYEE JERKED BACK TO KEEP FROM FALLING, CAUSING SEVERE PAIN IN LOWER BACK, HIP, AND LEFT LEG.
WHILE INSTALLING MILL SCREEN PLATES, EE WAS STRUCK IN THE FACE/MOUTH WITH SLEDGE HAMMER.
EMPLOYEE STEPPED DOWN OFF SCRAPER ONTO FROZEN, UNEVEN GROUND AND TWISTED HIS ANKLE. EMPLOYEE WAS PLACED ON RESTRICTED DUTY FOR EIGHT DAYS.
EMPLOYEE WAS CLOSING DOOR ON BALL CHARGE DUMPER IN FINISH MILL 14 WHEN HE HEARD A LOUD CRACK IN HIS ELBOW. EMPLOYEE'S ARM WAS PLACED IN A SLING AND HE WAS ON LIGHT DUTY FOR NINE DAYS.
EMPLOYEE WAS GRINDING THE BACK GAUGE WHEN PARTICLE FLEW IN HIS EYE.
PICKED UP BUCKET OF CONCRETE WHICH HE SAID CAUSED PAIN TO BACK.
HE STEPPED ON LOOSE MATERIAL, TWISTED HIS ANKLE, GRABBED A PIECE OF PIPE WHICH WAS ALSO LOOSE, HE FELL TO THE DECK AND THE PIPE HIT HIS ANKLE.
INJURED WAS WALKING ON WEST SIDE OF RAW GRIND WHEN HE SLIPPED ON COMPACTED SNOW AND FELL RESULTING IN SORE HIP AND LOWER BACK. EMPLOYEE HAS HISTORY OF BACK PROBLEMS. HE RECEIVED WORK RESTRICTI ONS DUE TO THIS INCIDENT.
EMPLOYEE WAS TAKING OUT CASTINGS IN THE BALL MILL WITH THE HELP OF ANOTHER EMPLOYEE. THE OTHER EE STRUCK THE CASTING IN SUCH A WAY THAT CAUSED IT TO CHIP AND FLY INTO THE EMPLOYEE'S FACE.
CARRYING AN AIR TUGGER ON HIS SHOULDER, HE STATED HE REMEMBERED SLIPPING WHILE CARRYING THE AIR TUGGER.
EE WAS SLUGGING SHELL HARDWARE AND HIT HAND WITH A HAMMER. EE WAS UNABLE TO SUE IMPACT WRENCH BECAUSE THE AIRLINES WERE FROZEN.
EE WAS STANDING UP PEDESTAL WITH A PRYBAR WHEN THE WEIGHT TRANSFERRED & PINCHED HIS FINGER BETWEEN THE PEDESTAL & THE CRIBBING.
EE WAS PUTTING INSULATION ON STEAMLINES. INSULATION KNOCKED DUST OFF TOP OF PIPE INTO HIS EYE. DUST WAS HOT GOT BEHIND SAFETY GLASSES CAUSING BURN. EE RECEIVED RT AND HAD FOLLOW UP VISIT WITH SPECIALIST.
EE WAS CLEANING ON THE SOUTHWEST FEED SHELF DOOR WHEN HIS SECOND RIGHT HAND FINGER WAS IMPACTED BETWEEN THE WOMA GUN HANDLE & A STRUCTURAL STEEL SUPPORT BEAM.
EMPLOYEE WAS INSPECTING THE 752 DRAG CONVEYOR WHEN THE CHAIN HIT THE LID, PINCHING HIS FINGER. THE LID WAS NOT FASTENED AND THE LOOSE CHAIN HIT THE LID.
EMPLOYEE WAS GRINDING CLINKER IN LAB. HE REACHED DOWN TO PULL DRAWERE OUT OF MACHINE & FELT PAIN JUST BELOW SHOULDER & LOWER BACK. FIRST AID ONLY AT TIME OF INJURY.
EE WAS USING POCKET KNIFE TO CUT 3/8" PLASTIC TUBING TO BE USED AT JOB SIGHT. KNIFE SLIPPED WHILE CUTTING LUBE COMING IN CONTACT WITH HIS LEFT HAND CUTTING HIS INDEX FINGER APPROX. FROM THE KN UCKLE TO THE TIP.
COMING OUT OF COLLECTOR AND TRIPPED ON LADDER.
WHILE CLEANING DRILL PRESS IN SHOP, EMPLOYEE WAS REMOVING SPIRAL SHAPED SHAVINGS. WHEN HE PULLED ON SHARP SHAVING, HE RECEIVED CUT ON FORREFINGER OF RIGHT HAND. EMPLOYEE WAS WEARING GLOVES.
EE WAS INSTALLING NEW STEPS TO ACCESS WALKWAY BY FEEDER. NEW STEPS WERE FABRICATED OUTSIDE OF THE PLANT AND SHIPPED IN BANDED TOGETHER WITH A METAL BAND. EE WAS WEARING GLOVES AND ATTEMPTED TO PICK UP A PIECE OF THE METAL BANDING MATERIAL TO DISCARD IN THE TRASH, AND WAS CUT ON HIS MIDDLE FINGER ON HIS LEFT HAND.
WHILE LIFTING ONE END OF WATER SPRAY PUMP BY THE SAND PIPE, OFF THE STUD BOLTS ANOTHER EE PRIED THE OTHER END OFF THE STUD BOLTS. THE PUMP FLIPPED OVER, AND THE STAND PIPE FLANGE THAT I WAS LI FTING WITH SMASHED MY FINGER AGAINST THE NEAR BY WATER RESERVOIR TANK.
EE WAS USING AN AIR HOSE TO CLEAN OUT BULLDOZER CAB. HE SLIPPED AND FELL WHEN TRYING TO DESCEND FROM EQUIPMENT.
ROLL DOOR LEFT OPEN; DRIVER NEEDED TO CLOSE DOOR TO TAKE TRAILER ON RUN. MAY HAVE BEEN DEW ON TAIL END OF TRAILER; DRIVER LOST FOOTING AND FELL TO THE GROUND.
EMPLOYEE WAS REINSTALLING A FAN GUEAR ON A HAUL TRUCK. HIS FEET SLIPPED ON THE LADDER AND HIS ARM CAUGHT THE STEEL ON THE FAN GUARD. THERE WAS A LACERATION TO THE RIGHT FOREARM.
EE WAS SHOVELING IRON ARE INTO THE INSPECTION DOOR AT THE TOP OF THE BURN. HE HAD A FULL SHOVEL AND TWISTED HIS BODY CAUSING STRAIN TO HIS BACK.
COAL PILE WAS PUSHED AGAINST LIGHT UNIT WHICH IGNITED COAL.
