IP completed loading railcar & advised trackmobile operator they were ready to move car while trackmobile Operator was pressuring up air system IP decided to go back on top of the car without notifying trackmobile Op. IP was strapped into fall arrest system which caused them to be drug across the top of moving cars. IP suffered a fracture to a vertebrae and bruising to torso.

Employee tested and had a standard threshold shift on EE's hearing test. Employee came in on day off

Clear, dry, noise from the forklift and semi trucks. Transverse fracture on middle finger right hand.

The employee was fixing a coolant leak of a D-10 dozer at Johnson SL-8 location, when the water pump being used to pump coolant from the top of the radiator fell, contacting the top of the employee's head.

EE was cleaning a chute that is part of a Crusher with Trommel screen. EE did not Lockout/Tagout/Try before removing the guard to the chute. While cleaning the chute the bar EE used bounced back at EE, causing EE to let go with EE's right hand. Hand came in contact with rotating trommel, taking off EE's impact glove and causing a laceration of EE's right-hand middle fingertip

Employee was removing an air line from the East blow bottle when they fell from ladder approximately 6 feet. Employee was taken to the hospital for x-rays which confirmed they fractured their right heel.

Employee was backing dozer up into a cut and ran over a large rock. The employee had EE's hand resting on the steering controls and was belted in, however, the motion caused pain in EE's shoulder. The employee is now off of work.

Employee was lowering a pinion shaft to the floor when EE lost EE's grip. When EE re-positioned quickly, EE felt a pain in EE's shoulder and arm.

When climbing into the loader, the employee skipped a step. Because the employee skipped a step, the step the employee took with right leg was high and awkward. Employee attempted to stand up when the right foot slipped resulting in the employee falling 3 feet to the ground. Employee felt pain in an area where EE had plate installed in neck.

Employee was opening gates on rail cars when EE tripped and hit left knee.

Employee was using a pocket knife to cut a zip tie holding wires employee needed to loosen to adjust sensor. When zip tie was cut knife continued and cut left arm above wrist.

Employee was returning from a production check when he twisted his knee on 9/1/15. He finished his shift and worked his next shift on 9/2/15 but continued to experience pain in his knee. He sought a medical examination after his shift on 9/2/15.

Employee was performing heavy equipment maintenance, installing a roller on a bulldozer; the roller slipped and smashed his small finger on his left hand.

Employee was retrieving a product sample from a cyclone sample port. The port was plugged, so he reached his right hand into the port to attempt to unplug it when the tip of his middle finger was severed by making contact with the rotary valve above the opening.

Employee was assisting another employee cutting metal; while handling a piece that was already cut, it shifted and when he pulled his hand away quickly he swiped his middle finger against the cut edge of the metal, causing a laceration to the underside of his right middle finger.

Employee was crossing level ground and was shuffling his feet, stubbed his toe and lost his balance causing him to fall into the adjacent conex. Employee struck his head against the outside wall. Employee sustained a laceration to his scalp. Laceration required 3 stitches with no other physical damage or impairment.

Employee was cleaning out a material chute below the 8 X 20 screen with a bar. A rock came down from the top of the screen and hit the bar, causing it to strike the employee in the face. Employee declined medical treatment until 5/8/14.

employee was stacking 100# bags when he felt a pain in his right shoulder. he informed the lead man and they stopped packing, once stopped packing, they informed the plant foreman. they brought employee to get a drug screen and see the doctor. the doctor gave him 2 prescriptions. his report from the doctor states that he can return to work with out restrictions.

Employee operating bag spout valve up and down filling 4000# bags at tank #1. he reported his shoulder was bothering him. We sent him to the company doctor and the doctor said that he had a bone spur to take aleve and he was released for full duty work (it was an first aid only case). employee says still hurting and went to see his own doctor and now he is waiting to get an MRI.

Employee was performing maintenance on the palletizer feed chain when he stepped down on a nail protruding from a piece of broken pallet in the work area. Nail penetrated through his work boot into his left foot which necessitated medical treatment.

Employee was installing wear parts in a roller mill when the angle iron supporting the come-a-long slipped, striking the employee in the upper lip and causing a laceration. No other facial or dental damage was indicated by the treating physician.

THE EMPLOYEE IS A FRONT END LOADER OPERATOR, AFTER HE FINISHED LOADING THE ORE HOPPER AND WENT TO PARK THE FRONT END LOADER, WHEN HE CLIMBED DOWN FROM THE MACHINE, AT THE TIME OF CONTACT WITH THE GROUND, HE STEPPED ON A BARITE ROCK, AND TWISTED HIS LEFT ANKLE

Employee was pulling on a pipe wrench trying to rotate a shaft on a screw. The shaft was locked out and tagged. The employee was using excessive effort that caused the shaft twist like a torsion bar. The employee lost his grip on the wrench and stored energy in the twisted shaft caused the wrench to hit the employee's right hand breaking a bone.

Employee was in the process of climbing of the bulldozer when his left foot slipped and he fell approximately 5 feet to the ground. The employee was not using 3 points of contact. The employee suffered a broken collarbone and 5 broken ribs.

Employee was drilling through metal using a power drill when the bit got caught in the metal and the drill torqued out of the employee's hand, striking and fracturing the small finger on his right hand.

Employee was in the process of loading a bulk truck - he was attempting to climb the ladder on the back side of the truck trailer when his left knee gave out on the first step. He was able to catch himself on the ladder and was lowered to the ground with the assistance of two co-workers. Employee advised to remain off work until recheck with treating physician.

Employee was hand stacking product bags when he jammed the fingers on his right hand while in the downward motions of placing a bag onto the pallet. Incident was reported at the end of the shift; employee did not seek medical treatment for the injury until September 5, 2012.

employee was swapping bulk loading hose from tank 6 over to tank 7. He squatted down to close ears for connection, stood back up and felt a slight release followed by what felt like fluid filling in my lower back. Went to walk to pumper scale house and was starting to feel pain and stiffness with each step.

An employee was in the process of adjusting the roll of shrink-wrap when one end of the bar holding the shrink-wrap slipped off the stand, this caused the roll weighing approximately 100 pounds to drop the opposite end of the bar raised crushed the employees right little finger between the bar and the stand.

Employee was walking on flat ground when he tripped and twisted his right knee in the wrong direction. He reported the incident and declined medical treatment at that time. On July 9, 2012 he employee notified the employer that he was seeking medical treatment. Employee was released to work on modified duty until recheck with his treating physician in approx 2 weeks.

Employee was cleaning a chute in front of an 8' X 20' screen using a bar. When the material in the chute came free, the employee dropped the bar into the chute. He stated at that time that he felt a pain in his right shoulder and requested a medical evaluation.

Employee was attempting to wash the windows on the loader when he slipped and fell to the ground. The employee bypassed guardrail to reach the window.

Employee was in the process of connecting a car puller cable to a rail car. The employee did not wait for the railcar to come to a stop before attempting to place the hook on the rail car. The hook on the car puller pinned the employees hand in between the car puller cable and the railcar. This resulted in the employee breaking a bone in his left hand.

Employee was operating a scraper when she felt hit a bump and felt a sharp pain in her lower back. The employee was seen by a Doctor and the results of an MRI revealed that a compression fracture occurred in the L1 vertebrae.

Employee was changing vacuum cleaner bag when bag broke, spilling dirt and debris up into her face, causing irritation to her left eye. Employee sought medical treatment the following day when first aid methods were unsuccessful in removing irritant from eye.

Contractor hauling and stockpiling crude ore @ facility. Contractor backed truck up to stockpile. When he attempted to dump, the cab of the haul truck came off of the ground. The cab turned to the right hand side and when it came down, haul truck operator's arm broke right side window causing laceration to contractor employee right forearm.

An employee was working on loading rail cars when the employees lanyard did not retract the lanyard got caught on a moving railcar and the employee fell about 3feet from a landing onto the top of a rail car. The employee received bruises and cuts to his right side and to his shoulder.

Employee was stacking bags when two bags came at the employee at the same time the employee attempted to stack both bags when he felt a sharp pain in his right shoulder.

Employee was being trained on the water truck, the trainer released him to drive on his own. He was descending a pit ramp when his brakes got hot. The truck gained speed and went across the pit floor through some rough terrain & stopped on a bench. He indicated neck pain so was taken to be medically examined by a physician. First aid only, he was given restricted duty for 1 week.

Employee experienced torso discomfort the day after material handling on this shift. Condition deteriorated until he sought doctor's treatment for a possible inguinal hernia on 6/28/11, he is working restricted duty and has been referred to a medical specialist for a surgery consultation.

EE on ladder pulling air tubing through conduit. EE's hand slipped off of tubing as he was pulling. Employee grabbed top of ladder, when he did, the momentum caused him and the ladder to fall backwards. EE landed on tail bone causing injury to pelvis area. EE was not tied off. EE did not have ladder in fully opened/locked position. EE did not have ladder secured.

Employee was poked in the finger by a wire from an Hydraulic line. The wire broke off inside the employees finger. The employee's finger became infected because of the wire. The infection spread through the rest of the employee's body.

The employee put all of his PPE on including the proper fall protection. The employee failed to notify the operators whom normally work in the area of his intentions. The employee began changing the light on the railcars when he felt the cars begin to move. The employee lost his footing and hit his head. The employee received 4 staples in the back of his head.

Employee was attempting to open a door on a railroad box car. The employee was pulling down on the handle and felt a sharp pain in his hip. The pain radiated down to his leg.

Employee was hand stacking 88# product bags. He reached up around the upper edge of the bag inside the rubber curtain guard of the conveyor and his hand was caught between the conveyor belt and the first guide roller. Employee was wearing gloves. He sustained a soft tissue injury of his right hand, released back to work modified duty for 7-10 days by the treating physician.

Employee was pulling up on a bar when he felt a sharp pain in his lower neck and upper back area.

Employee was inspecting truck to load. Employee lowered inspection stairs to top of trailer. Inspection stairs got hung up on trailer hatch. Employee tried to use pneumatic system to raise stairs. Stairs remained lodged. Employee attempted to dislodge by lifting on cage. When lifted, cage became dislodged and upper railing struck employee in chin causing laceration.

Injured his left shoulder while replacing the fan clutch on a haul truck. Employee was supporting the majority of the weight of the fan clutch in his left hand and was guiding the part with his right hand when he felt a sharp pain in his right shoulder.

Employee was exiting front-end loader using the permanent ladder stairway and handrails on the equipment when his foot slipped on the second step. Employee fell to the ground backwards, using his right arm to control the fall. Employee suffered a compound fracture of his lower right arm upon landing on the ground.

Employee was cutting steel strap with shears and it sprang back, striking him on the left thumb and cutting through his glove, causing a laceration.
