technician was torquing lug nuts on a haul truck when They put their fingers in the path of the reaction arm causing them to be caught in the pinch point.

EE was cutting water hose with personal pocket knife when knife closed on right index finger.

EE lost 3 point contact when accessing Excavator. EE caught self with left arm, causing injury.

EE was performing maintenance on a set of screens. While inside the screen deck EE looked up and a foreign object fell between EE's safety glasses and EE's eye. EE washed it out and reported to supervisor. The next Monday (1/13/2020) it was bothering EE. At this time supervisor took EE to clinic.

EE was dismounting boom truck bed to operator controls when knee buckled underneath EE. (previous injury)

EE jumped 3-4 feet to the ground, hurt right knee

Employee's had right hand resting on the side of the feed hopper near the top of it. EE was looking down into the crusher to see if the saddles were worn. As the feed hopper was being lifted EE did not remove their hand. This caused their fingers to get caught between the top of the feed hopper and the support beam the feeder slides back and forth on.

Secondary plant chute was brought down to ground level for maintenance on sides. Two temporary legs where attached to lift up. Left leg came unattached causing the chute's right leg to slide off a metal part onto employee's foot. Bottom of temporary leg entered the portion of steel toe boot where steel and boot material meet.

Employee was in the process of cutting blast hole tubes in half to tube the blast holes that EE had drilled. EE's knife slipped while cutting the tube and cut EE's leg above the knee.

Medical condition resulting in a heart attack.

Employee used a torch to cut bolt off when slag built up and went under safety glasses into eye

Employee was attempting to park the Kawasaki Mule at end of shift. Employee was turning the Mule as the pathway, which was flat, changed from gravel to asphalt and the Mule turned over on its side. Injuries were scraped left forearm, broken finger tips to left ring finger and left middle finger. Left palm required 3 stitches and the left index finger required 6 stitches.

EE was installing a rubber snout on the discharge hopper of C33 to C34. He was using a box knife to trim the rubber when the knife slipped and cut his lower left arm.

Top radiator hose was being removed from A 2468 skid steer, while pushing hose from radiator, right hand slipped from hose striking hose clamp cutting right outer knuckle.

Employee was moving a 20 lb water pump for back-hoe to pass. When employee lifted the pump out of the way, his foot slipped in the mud causing him to twist his lower back.

Customer truck was descending down the access road with 18.77 tons of jetty stone and blew both rear air pods causing a braking system failure. The drive jumped from the truck and the truck ran over the driver's legs.

Rock was lodged inside cone crusher. Employee got inside cone crusher and was twisted around to grab a rock. While pulling at a rock his back made a popping sound. Emploee was treated at local clinc; was diagnosed with a pulled muscle.

Employee was assisting in removing the bushings from the bucket of the excavator (changing over to the hammer) and he cut his right index finger requiring 5 stitches.

Injury occurred when employee stepped with left foot from primary shack and muddy boots slipped off the step. Right leg and foot got caught between building and first step, twisting knee and leg. Sprain of knee and leg.

Employee was grinding the end of a bolt and the grinder slipped, cutting his right hand, requiring 4 stitches.

Employee was helping to remove the door on the Penn Mill and bumped his elbow

Employee was cleaning the head box at the scalping screen and the bar slipped out of the employee's hand, hit the handrail, and fell to the ground striking the employee on the ground on the right ear. Employee required 6 stitches.

Employee was standing on the bottom step of a ladder while cutting a piece of angle iron, the ladder moved, causing him to lose his balance and fall against a piece of crossbeam, scraping his side.

Employee was carrying a cone pump down the steps and his right knee "popped out".

Employee was cleaning a dust chute that was clogged. He was poking the buildup with a shovel from the top. When the material gave way, the employee's right wrist came down and hit a piece of angle iron on top of the box. Employee has been instructed to use proper tool in the proper manner for the job.

Employee was digging in the pit with the WA700 pit loader and hit a toe and it jarred him straining his back. He was wearing his seatbelt.

THE INJURED EE WAS ASSISTING A CO-WORKER IN TRYING TO LOOSEN A TIGHT BOLT ON A 4248 JAW CRUSHER. THEY WERE USING A LONG PUNCH & A HAMMER TO TRY TO DRIVE THE BOLT OUT. THE EE'S HAND SLIPPED JUST AS THE OTHER EE SWUNG THE HAMMER TO HIT THE PUNCH. THE HAMMER STRUCK THE EE'S RT.RING FINGER.

Employee was trying to open the #2 bin door for washed stone and pinched his left hand below the thumb requiring 6 stitches

Employee was stepping off the Rock Hammer and twisted his back

THE CRUSHER TOP HAD BEEN SET ON THE GROUND BECAUSE WE WERE DOING SOME MAINTENANCE INSIDE CRUSHER. EMPLOYEE WAS CLEANING OFF THE BUILD-UP ON THE CRUSHER TOP WHEN THE CRUSHER TOP SLIPPED INTO TW O PIECES. (WE DIDN'T KNOW IT WAS 2 PIECES) AND CUT HIS RIGHT THIGH REQUIRING 24 STITCHES & A HIARLINE FRACTURE IN RIGHT ANKLE.

EMPLOYEE WAS TRACKING BELT AND HIS GLOVE GOT CAUGHT IN A ROLLER PINCHING HIS THUMB ON THE ELECTICAL BOX.

EE HAD FINISHED DUMPING HIS LOAD OF 825B AND DID NOT LOWER HIS BED AND HIT THE TUNNEL GOING UNDER I-75. HE DID NOT HAVE SEATBELT ON AND HIT HIS HEAD REQUIRING 6 STITCHES.

EMPLOYEE WAS CUTTING A PIECE OF RUBBER AND THE BLADE SLIPPED AND HE CUT HIS RIGHT THIGH REQUIRING THREE STITCHES.

MALE WAS FLEEING FROM POLICE FROM A ROUTINE SAFETY CHECK. HE RAN ACROSS ABC COKE PLANT'S ROAD & CLIMBED OVER A FENCE ONTO OUR PROPERTY. SRM AGGREGATES NOTIFIED POLICE TO ADVISE THEM THAT THERE WAS A MAN HANGING OFF OF OUR HIGHWALL. DETECTIVE CAME TO OUR PROPERTY. THE MALE HAD MOVED & THE OFFICERS BACKED OFF. THE MALE WAS SPOTTED AGAIN. ADVISED THE MALE HAD FALLEN FROM THE HIGHWALL.

EMPLOYEE WAS HITTING THE BELT SKIRT TO PUT IT BACK ON THE INSIDE OF THE CONVEYOR BELT. HE WAS USING A HAMMER, HOLDING THE HEAD AND USING THE HANDLE TO HIT BELT SKIRT. HE DID NOT PAY ATTENTION TO WHERE HIS HAND WAS AND STRUCK HIS 4TH FINGER ON HIS RIGHT HAND ON THE ROLLER FRAMEWORK REQUIRING 9 SUTURES.

EMPLOYEE WAS PUTTING CURTAIN LINERS IN CRUSHER &DROPPED ONE ON HIS RIGHT RING FINGER SMASHING THE NAIL. IT REQUIRED ONE STITCH.

EMPLOYEE WAS CLEANING UNDER JAW AND A ROCK HIT HIM IN THE HEAD REQUIRING 6 STITCHES.

EE STEPPED OFF OF LOADER AND TWISTED HIS KNEE REQUIRING KNEE SURGERY.

EE WAS LIFTING SOME WIRE TO REBUILD THE DRY SCREEN DISCHARGE CHUTE AND STRAINED HIS GROIN (RIGHT).

EE WAS REPLACING THE REAR END OF THE EUCLID R-40ROCK TRUCK AND HIS HAND SLIPPED CUTTING HIS 3RD FINGER ON RIGHT HAND REQUIRING 3 STITCHES.

WHILE HAULING ROCK FROM THE PIT TO THE PRIMARY JAW IN THE 465 ROCK TRUCK, EMPLOYEE HIT A BUMP AND JARRED HIS LOW BACK.

CHANGING JAW DIE AND PUTTING THROAT BACK OVER JAW, THE CHAIN WAS TOO LONG SO THEY WERE GOING TO REHOOK IT, EMPLOYEE WAS TRYING TO GET OUT OF THE WAY AND STUMBLED GRABBING A BAR AND THE THROAT SMASHED RIGHT INDEX FINGER.

STEPPED OFF LADDER ONTO GROUND (WHICH WAS FLAT) AND TWISTED RIGHT LEG.
