Employee was cleaning the bag compartment of the vacuum truck with a shovel. While cleaning the bags, EE smashed EE's left ring finger between the shovel handle and the bag compartment door frame resulting in an injury. Employee was treated at local emergency room and received 3 sutures in EE's finger.

Employee was picking up a mill door liner and felt pain in EE's lower back. EE's supervisor called work care and the employee agreed to self-treat. During the follow up with work care on 7/20/2026. Employee was seen at occupational health on 7/22/2026 and placed on restricted duty.

Employee reported progressively worsening bilateral hand discomfort with persistent numbness and tingling beginning in the middle fingers and radiating into the forearms, often disrupting sleep. Employee was diagnosed with bilateral carpal tunnel syndrome; prescribed prednisone 20 mg and PT.

The employee was lifting a tool bag (approx. 45 lbs.) out of the back of a stake body truck, turning and placing it onto the ground when they felt a pain in their abdomen. The employee sustained a strained abdomen.

A maintenance journeyman was using a cutoff wheel grinder at Quarry Belt #5 to cut a guard bracket. A fragment from the disc struck the employee's left knee, causing a 2-4 inch laceration. The employee was treated with nine stitches.

One employee was stuck on a personal elevator between floors 6 & 7 for 55 minutes. Employee was able to get out, elevator company was called to come investigate and make repairs. Repairs were made on 6-4-2026. No injuries or illness.

Employee was assigned to jackhammer the Finish Mill (FM) feed chute at FM6 due to material buildup/restriction inside the chute. While performing the task, the employee reported EE slipped and caught self with right elbow and has a pain in EE's right shoulder.

Mill utility employee was jackhammering in Silo 4 when the jackhammer became stuck. While attempting to free it, the jackhammer suddenly released and dropped, pinching the employee's right pinky between the jackhammer and a grizzly bar. The employee received medical treatment- reinserted fingernail and stitched it to hold in place. Was told to keep dry and clean.

Employee was working to get an airslide to flow material when dust from above dropped down onto the employee and into their eye.

UNDER THE SILOS IS A TIGHT SPACE. WAS USING A WALK BEHIND SKID STEER. MOBILITY WAS LIMITED, OPERATOR WAS GOING IN REVERSE WITH A LIMITED FIELD OF VIEW

EE fell down an undetermined amount of stairs below the kiln feed area. The worker received multiple lacerations to head and nose requiring extensive care. Also informed late evening at the ER that there was a crack on one or both sides of vertebrae.

Elevator was in the process of being test ran after previous repairs, during test run, elevator and one miner was stopped at the top landing unable to exit or move. Repairman was onsite and able to open the elevator and move to ground level. Miner was in the elevator for 33 minutes.

Employee was using a grinder with a wire wheel when EE stood up and accidentally hit the trigger of the grinder causing it to kick back and get bound up on EE's left forearm causing a laceration.

A maintenance millwright was walking down the stairs in the raw mill from the 1 1/2 floor and fell down the stairs hurting EE's knee.

Employee was running a walk behind skid steer removing snow around the scale-house. Employee was backing up and backed into a concrete bollard hitting right leg causing a large hematoma. Employee was sent to hospital via ambulance.

EE was cutting electrical tape off the wire connections while wires were in junction box. EE was using a ceramic knife and had on cut level 4 gloves. While cutting the second wire connection the knife blade went across EE's left palm causing a 2 inch laceration.

Use of sledgehammer to remove wear plates inside a mill. Employee missed the wedge, hit wear plate that a small metal piece broke off and punctured employee in right upper leg area.

Minor back strain from lifting old wear plates that were being removed from a miller lining.

EE slipped and fell on EE's right hip

An employee was a passenger inside of a fellow employee's personal vehicle that was involved in an incident on the quarry road headed to return to work from their break. Employee initially declined medical treatment but, on the 12th, requested to been seen by medical professionals. Employee was diagnosed with a mild concussion and was given restrictions.

Loader operator drove to Central Control for a pre-shift safety meeting. While exiting the payloader, the operator missed stepped on the last rung and fell backward, landing on the left hip.

Employee was using an air lance with 90 psi air to clear a chute plug in the #4 Finish Mill feed chute. The employee lost grip causing the air lance to hit them in the mouth. They initially declined medical attention. On 10/24/2025, they requested to be seen at the emergency room due to mouth pain.

Employee was installing a blank in the meal pipe. While pushing blank in and didn't realize a finger was in the bolt hole. When blank finally went in it cut tip of right ring finger.

Employee was working on the long reclaimer. Employee went to reposition and while doing so slipped and left butt cheek landed on the point of a metal plate. This caused a 2cm puncture to butt cheek. This didn't become a medical until they went to wound care and they started packing the wound. This started on Oct 29th

Cutting trees with chainsaw. Blade got stuck. Turned off. When EE pulled the chainsaw out, it swiped EE's leg, resulting in a small cut, requiring three stitches and no loss time.

EE was entering the building on EE's way to EE's office when EE tripped on the steps going up to the door. This was on Friday. We considered it first aid. On Tuesday EE went to the Clinic for an X-ray. X-ray showed a fracture in EE's right foot. EE came back with restrictions. This is when it became an MTI and recordable to MSHA

The employee was tightening bolts on a compressor using a wrench when EE felt pain in EE's right shoulder.

EE experienced Heat Exhaustion due to temperatures in the 90's range

EE1 was getting in the man basket. EE2 was already in man basket and moved it pinning EE1 leg between basket and concrete wall

Shift repair was working on the "small mills" FK pump to repair an air leak. While walking from the repair truck to the west side of the FK pump, they tripped on uneven floor plates and fell hitting their right knee. Employee was taken to the emergency room and was diagnosed with a fractured right patella (kneecap).

Employee was shoveling and felt a pull in back. Employee was put on restricted duty for a week.

Throwing dust collector bags into the rear of the suck truck the employee felt a pop in right shoulder. Employee started treatment with our online Teladoc with no change in pain or discomfort. Employee was advised to seek medical attention with a clinic. Right shoulder strain.

EE was closing up a rail car by prying with a bar. While doing this, EE felt a sharp pain in EE's left side. EE then noticed belly button started to swell twice the size. It is bright red and hard to touch. EE does not have a lot of pain in this area. EE will finish the shift out on the locomotive.

The employee was removing a wall filter down off of the all-thread and a section of the wire mesh that wraps the outside of the filter caught the left hand of the employee. This caused a laceration to the employees left thumb pad. The employee was not wearing gloves at the time of the incident.

Employee had a Standard Threshold Shift of greater than 25 db HL on their audiogram from 3-10-2025. Right ear results were 36.3 and the left ear results 31.3.

Employee was removing liners from ball mill when they dropped the liner striking them in the foot. Receiving a fracture in the foot.

Employee was cleaning material off 117 conveyor belt when they felt a pull in is abdomen/groin they reported this to supervisor and denied care at this time. The employee reported to safety on the 27May2025 they were still experiencing pain and we called care and EE was given a home care plan with no relief they were scheduled a clinic visit for further treatment.

The employee was poking lumps out of a discharge chute with a bar. As employee was poling the material the bar broke through the material, the employee pinched right middle finger between the bar and the edge of the chute.

The employee walked into the pressure washer to retrieve the pressure washer wand and hose and when EE walked back out of the doorway EE slipped on ice. Falling backwards and reaching out with right hand to break the fall, resulting in a sprained right wrist. Located next to the door was a bucket of ice melt, ice scraper, and a shovel.

Employee injured right shoulder while install a piece of screw conveyor during the second shift on 1/27/25. EE reported it to the supervisor on 1/28/2025 at 10:30 am. EE called Teladoc and was referred to see a clinical doctor.

Employee got dust in is eye on 1-17-2025 and over the weekend the employee had medical treatment which changed the treatment process to a medical to a lost time incident.

Employee was working with a crew installing putlog beams in a vessel. Due to a miscommunication, the crew on opposite side of vessel pulled the beam while employees hand was in a pinch point and the top of the employees right pinky finger became pinched between a beam clamp and the vessel. Employee pulled finger out and noticed blood. Injury required stitches.

Employee was to remove the rotary feeder. The rotary feeder is at ground level, so the employees were working from a seated or kneeling position. The crew used prybars to break the seal of the feeder. One side of the feeder came loose allowing the feeder to swing to the opposite side. As the last side broke free, the feeder fell (18") to the ground, landing on the employee's knee.

Employee was using the Fraco Elevator to travel the Preheat Tower when the elevator stopped between floors. Employee was stuck on the elevator due to a bad motor and was able to exit the elevator at 2:27 pm.

G&R employee was guiding sheet metal roofing into a dumpster. The sheet metal swung into the employee's gloved hand which was resting on a cross beam of the dumpster, cutting the pinky finger. Medical treatment was 3 stitches to the finger.

Employee was cleaning the ground floor of the shipping silos with a vacuum truck. As the employee was sucking up material, they felt discomfort in the groin. Employee was seen by the clinic and placed on restrictions.

The employee was walking in the packhouse to check bag inventory for a customer when EE walked around a pallet of bags and found what EE was looking for EE turned around and did not realize EE's proximity to the pallet and caught right foot on the corner of the pallet causing EE to fall, striking knee on the ground. The employee sustained a fractured kneecap.

Two employees got stuck inside the old shipping silo elevator for 40 minutes. Car was just below the 4th floor landing, so employees were able to climb out the top.

EE was replacing the windshield wiper motor on a F-250 truck. After installing, EE plugged the motor in. The wiper was not in the "Parked" position and cycled, pinching their left thumb in the wiper linkage. EE was wearing cut-resistant gloves, but it was a pinching situation resulting in a laceration requiring stitches to close. EE returned to work with no restrictions.
