IGNITION - A frictional methane ignition occurred on the mine floor of the #1 face on #12 section. The ignition lasted approx. 15-20 seconds and was extinguished with a fire extinguisher. The miner was cutting bottom when the ignition occurred due to a methane bleeder present in the mine floor.
Longwall shearer was cutting on the face when the shearer came into contact with an old de-gas poly line. A methane ignition occurred near the top/roof and was quickly extinguished with face water. No injuries resulted in this ignition.
While mining #7 entry, crosscut #40 of the Sub 4-F Sub Main (004-0 MMU) a methane ignition occurred. The face was approximately 25-30 foot deep. This ignition did not cause personal injury to miners or damage any company mining equipment.

IGNITION - Occurred while roof bolter was drilling holes in the roof to install six foot roof bolts. As they were drilling through the hole in a roof channel into the roof when the ignition occurred in the roof. The methane ignition lasted 5 to 10 seconds.
Left CMO briefly saw an orange glow around the head of his CM and believed it could have been a small ignition. Investigation determined there was no evidence of a methane ignition.
During mining operations on our #2 MMU a methane ignition, bleeder fire, occurred at approximately 1:15 pm. The bleeder fire was approx 3 feet long and burned for about 30-40 seconds. MSHA was notified at 1:25pm and a plan was developed and approved by the District Manager addressing this incident. No miner was injured due to this bleeder fire.
A methane ignition occurred on 20 Rt. in No. 1 entry of new longwall face. Miner operator had cut approx 18' when mine roof fell in from face outby for the length of cut & approx 9' wide. The Sandstone rock that fell was 3' to 12" thick. Mine operator was cutting up the rock to load out when the ignition occurred. The flame only lasted approx 15 sec & was extinguished w/water hose.
This 7000-1 is being submitted in response to citation #7116747 issued on 7/10/12. Following an investigation, the operator is not in agreement with MSHA that an immediately reportable methane ignition occurred. While preforming routine cutting / welding operations on the 19A tailgate shearer drum near shield #22, a small fire occurred which was immediately extinguished.
The number 7 section, cutting in the number 3 face, had a methane ignition.
MMU-002 #2 Entry x-cut 31 & 32 4 North B, a non-injury methane ignition occurred while EE was drilling a 12' cable hole. The drill steel contacted a roof mat and ignited methane inside the drill hole. The flame was put out with water and fire extinguishers.
A small methane ignition occurred in the face of the #5 entry of the 2nd West Mains when a 5-foot mechanically anchored roof bolt was installed in rock at the top of the rib and methane in a roof crack was ignited by friction or sparks.

IGNITION: Methane ignition on #2 LW at Shield 87 involving TG drum. Operator cutting coal headed to HG with 80% methane on monitor. Flame was orange and about 12 " high and lasted approximately 2 to 5 minutes, it was extinguished with the water sprays on the TG drum. No injuries were incurred.
There was a methane ignition in the #2 entry of the 12 East section.
A suspected methane ignition occurred in the #6 entry face on the 2 left off North Main section. The miner operator described seeing a orange flash as he was attempting to cut cap coal from the roof.
A possible methane ignition occurred on 002-#1 section in # 5 entry 60' inby SS# 15043 during the first left lift off of #4 pillar block. MSHA and DMME were notified 103j order was issued by phone and modified to a 103k upon arrival. No visible evidence of ignition on rib or Miner.
Ignition- The continuous miner CM81 was loading the cleanup buggy with coal in the no.2 to no.3 x-cut on the off curtain side of the face, when the methane ignition occurred on the no.8 section. The ignition was extinguished with two fire extinguishers.

On 7/29/09 a methane ignition occurred in the last rt. cut while retreat mining between the #3 and #4 headings on the 1-rt. panel off of the #6 east main on mmun #001 section. MSHA was notified by phone of the methane ignition. Both the MSHA call center and the supervisor were notified.
Miner operator was taking 2nd cut out of #5 left entry break when a methane ignition occurred. The ignition came back across the miner burning the operator.

A methane ignition occurred in the 8th East development section. The left side roof bolter operator drilled a hole to a depth of 17" in the mine roof, when his drill steel plugged (wet drilling). He was retracting the steel from the hole to clean the plugged steel, when the hot drill bit ignited a pocket of methane in the drill hole.
IGNITION: The Continuous Miner was cutting coal in the No. 1 Entry on No. 8 Section and while cutting close to the roof, a frictional methane ignition occurred. The ignition lasted approximately 2 minutes and was extinguished with 2 fire extinguishers and a water hose.
A frictional methane ignition occurred in the 8th East develpment section. The operator completed his final cut and was in the process of cleaning up when the miner bits contacted the mine floor, igniting a pocket of methane in the floor.
A frictional methane ignition occured in the 9th East CM section. Operator was turning a crosscut (15' depth) when the miner bits contacted fallen roof rock. The miner head was 1' above the floor when the ignition occurred. It appears that a spark was generated from the sandstone, subsequently igniting a methane floor feeder. The ignition lasted less than 10 seconds.
The miner was cutting the top in the #1 face on the #1 section when a methane ignition occurred in the mine roof.

A methane gas ignition occurred on the #1 section at approximately 7:30 am. The continuous miner had cut approximately 20 feet out of the face area in the #3 left cross cut and was cutting in the top when the methane ignition occurred.
