Employee was driving a CAT 769 Haul Truck. Reached behind the seat to see if the cooling fan in the cab was running and stuck his finger inside the guard and cut tip of middle finger on right hand. Taken to hospital and stitched up.
A flop gate (75 lbs) at the end of a conveyor had rolled up and landed on the tail pulley. Two employees raised it up to let up back in position. One employee let go of his end after the gate went over center before the other and the gate pinched the right ring finger of the second employee against the angle iron that the gate swung in.

EE was attempting to remove a rock that had become lodged under the belt in a conveyor tailpulley using his hands to roll the belt forward. The rock dropped out and belt rolled back, pinching both hands. The little finger on his left hand was broke & the tip above the first joint was badly cut. He was taken to the hospital and the little finger was stitched up.

72"ELJAY CONE WAS BEING ADJUSTED. EMPLOYEE WAS ON CONE PLATFORM USING A WRENCH TO LOCK IN WEDGES ON CONE AS IT'S SPACING WAS TAKEN UP. FOREMAN SAID AT TIME OF ACCIDENT NO ACTION WAS TAKING PLA CE ON CONE. THE WRENCH, ACCORDING TO EMPLOYEE, JUMPED OUT OF SLOT AND STRUCK HIS UPPER RT LEG, CAUSING A BRUISE. EMPLOYEE WAS BROUGHT TO TOWN FOR BROKEN BONE, BUT LEG WAS BRUISED & HE RETURNE
EMPLOYEES WERE WORKING ON 72" CONE BOWEL. THE EMPLOYEE WHO WAS INJURED WAS STANDING INSIDE THE BOWL HOLDING A WRENCH IN PLACE ON A NUT HEAD WHILE OTHER EMPLOYEES WERE TIGHTENING DOWN BOLT OUTSIDE THE BOWL. INJURED MAN HAD HIS HAND ON BOWL. SOCKET ON AIR WRENCH SHATTERED AND A FRAGMENT STRUCK EMPLOYEE'S HAND.
EMPLOYEE WAS HELPING TO TAKE OFF CUTTING EDGE ON ENDLOADER. BOLTS HAD BEEN REMOVED EXCEPT FOR ONE. WHEN EMPLOYEE TOOK IT OUT, METAL FELL 2 FEET AND STRUCK HIM ON RIGHT FOOT CAUSING CONTUSION.
EMPLOYEE, THE QUARRY FOREMAN, WAS CLIMBING DOWN FROM A CAT 769 HAUL TRUCK. HE SLIPPED ON THE STEPS COMING DOWN AND STRUCK RIGHT ELBOW ON GUARD RAIL AROUND THE CAB OF THE TRUCK. HE BRUISED ELB OW AND AN INFECTION GOT STARTED BY THE BONE AND SURGERY HAD TO BE DONE TO DRAIN THE INFECTION.

ROCK HUNG UP HE WAS MOVING SMALLER ROCKS TO GET LARGE ROCK ROCK ROLLED OVER UT FINGER INDEX ON R HAND STITCHES
EE WAS WELDING ON FRAMEWORK OVERHEAD. WHILE DOING SO A PIECE OF SLAG GO INSIDE SLEEVE AND BURNED HIS ARM.
EMPLOYEE WAS REPLACING SCREEN DECK ON SCREENING PLANT USING A WRENCH. HAD REMOVED SEVERAL BOLTS AND FOUND ONE THAT WOULD NOT BREAK LOOSE. USED EXTRA FORCE AND LATER FELT A PAIN IN LOWER BACK.
EMPLOYEE WAS WELDING ON FRAMEWORK AROUND CONE CRUSHER. HAD USED A PIECE OF ANGLE IRON AS A MARKER AND LAID DOWN BETWEEN FRAMEWORK HE WAS ON AND A SECOND FRAMWORK. AFTER WORKING FOR A WHILE WEN T TO WORK ON OTHER FRAME. HE STEPPED ACROST AND PUT FOOT ON ANGLE IRON WHICH CAUSED HIM TO FALL ABOUT 9 FEET TO MUDDY GROUND.
EMPLOYEE WAS WORKING ON SETTING UP PLANT. HE WAS WORKING UNDER A FRAMEWORK ABOUT FIVE FOOT HIGH AND STOOD UP AND HIT HEAD CAUSING AN ABRASION ON THE TOP OF HIS HEAD.

EMPLOYEE WAS CL9ANING OUT FROM UNDER CONVEYOR BELTS. A ROCK FELL APPROX. 1K FEET AND STRUCK EMPLOYEE ON TOE ON LEFT FOOT CAUSING A CONTUSION.
EMPLOYEE RUNS T2WER THAT RUNS CRUSHING EQUIPMENT. A ROCK LODGED IN THE JAW AND AFTER STOPPING JAW THE EMPLOYEE ATTEMPTED TO MOVE SMALLER ROCKS OUT OF THE WAY SO HE COULD BREAK LODGED ROCK WIT H MAUL. HE CAU1HT LITTLE FINGER RIGHT HAND BETWEEN 2 ROCKS CAUSING A LACERATION AND FRACTURE OF THE FINGER.

EMPLOYEE RUN THE TOWER AT OUR QUARRY CONTROLING THE PLANT OPERATION. A ROCK LODGED IN THE JAW AND EMPLOYEE WENT INTO HOPPER, AFTER SHUTING DOWN PLANT, TO BREAK IT LOOSE A ROCK FELL AND STRUCK HIS HAND WHILE HE WAS WORKING. HE DID NOT WHERE THE ROCK HAD COME FROM.

EE WAS LIFTING A CAT 8D BATTERY WEIGHING APPROX.150# WITH THE AID OF ANOTHER EE FROM THE BACK OF A PICKUP TRUCK INTO AN ENDLOADER BUCKET. LIFTING THE BATTERY CAUSED BACK STRAIN.
