Incident didn't involve any aspect of compliance with rules/regulations/PPE. Miner proficiency may have been a factor. Started missing work 2/27/18. Work was idle from 2/21/18 till 2/27/18.

EMERGENCY ESCAPE HOIST SHAFT ICED UP DUE TO COLD WEATHER DONDITIONS WEATHER CONDITIONS STARTED CHANGING AND ILE FELL DOWN SHAFT STRUCK GUIDE ROPE JUST RIGHT CAUSING GUIDE ROPE TO BREAK. CONDIT ON NOTED ON DATE ABOVE.
EMPLOYEE WAS OPERATING A GETMAN TRACTOR, HELPED LOAD MATERIAL ON MAN CAGE, WHEN CAGE RETURNED TO BOTTOM HE WALKED TOWARDS HIS TRACTOR AND TRIPPED ON A WIRE ROPE SLING AND FELL ON HIS HANDS A ND KNEES. ACCIDENT OCCURRED 1-24-03 BUT WAS NOT DIAGNOSED UNTIL 2-20-03 WITH A TORN MENISCUS RIGHT KNEE.
A FALL OF ROOF WAS FOUND AT 12:45PM 87XC MAIN EASTWAY. FALL FELL ON TOP OF ARCH & SOME ROCK CAMEOUT ONTO THE TWAY. FALL GUESSED TO BE 6' HIGH, 20'LONG, & 10' WIDE. ROOF OVER ARCHES SUPPORTED WITH 6' CONVENTIONAL
A FALL OF ROOF WAS FOUND DURING AN INSPECTION DURING AN INSPECTION OF THE MAIN EAST LEFT RETURN AT XC34. THE FALL WAS 16'WIDE, 6' HIGH, & 5O' LONG DID NOT INVOLVE THE INTERSECTION FELL IN XC BETWEEN ENTERIES 3 & 4 AC. 6 FT CONVENTIONAL BOLTS USED.
AT 10:00 A HOIST TEST WAS PREFORMED AND WHEN THE LILLY OVER SPEED CHECK WAS MADE THE NUT CAME OFF THE SHAFT THAT HOLDS THE OVER SPEED IN PLACE. REPAIRED @ 10 45 AM FULL SERVICE.

THE EE WAS HELPING A ROOF BOLTER OPERATOR DURING RECOVERY OPERATIONS ON THE 3J LW FACE. THE INJURED EE WAS STRUCK ON THE RT HAND BY FALLING ROCK AS THE INJURED WAS BALANCING AN AIR DRILL.
EE WAS ATTEMPTING TO PIN DRAW BAR TO PARTS WAGON SO SCOOP COULD MOVE PARTS WAGON. THE PIN MISSED HOLE ON BOTTOM END, SCOOP MOVED CAUSING PIN TO KICK BACKWARDS. THE EE'S LEFT LITTLE FINGER WAS PINCHED.
THE EE WAS OPERATING A DIESEL GATOR. AS THE EE DROVE INTO A XCUT ON THE M.E. TWAY @#69 XCUT, HIS LT. FOOT WAS BENT BACKWARD WHEN HIS FOOT WAS CONTACTED BY A PIECE OF CONVEYOR BELT FOLDED UP UN DER GOB.
THE EE WAS HOOKING UP A DIESEL TRACTOR TO A ROCKDUST. THE EE SMASHED HIS LEFT LITTLE FINGER BETWEEN THE DRAW BAR AND THE DRAW BAR HITCH ON THE TRACTOR. 7 STITCHES & FX OF TIP OF FINGER.
EE WAS USING A 4" HAND GRINDER. THE GRINDER WAS BEING CRADLED IN HIS RT. ARM WHILE HE WAS PUTTING ON HIS GLOVES. DURING THIS PROCESS THE POWER SWITCH WAS ACCIDENTALLY ACTIVATED. REC'D 7 STITCH ES.

TH E FALL IS 70' LONG, 16' WIDE AND 8' HIGH. THE FALL IS BETWEEN1AC AND 2AC IN 2J AT 78+80. DUE TO WEAKENED STRATA.
THE EMPLOYEE TWISTED HIS RT ANKLE AS HE WAS STEPPING OFF THE BACK END OF A ROOF BOLTING MACHINE.

EE SUFFERS FROM CHRONIC BACK PAIN AND WAS DIAGNOSED ON 11-30-01 WITH DEGENERATIVE DISC DISEASE.
THE EE WAS STRUCK ON THE RT. EYEBROW BY A SLEDGEHAMMER THAT SLIPPED FROM THE GRASP OF ANOTHER EEINJURED EE SUFFERED LACERATION REQUIRING 4 STITCHES..
A FALL OF ROOF OCCURRED AT THE TAILGATE OF 3T LONGWALK IN 3 AC OF 25 @ 89 + 20. USING 8'RIB BOLTS FOR TRUSSES AND 6' COMBINATION DOUBLE FORK IN CENTER AND 2 ROWS OF 6X6 TIMBERS BETWEEN XC'S. SLIP RUNS THROUGH AREA LOST POWER ON FACE COULD NOT PULL SHIELDS AND LOST THE ROOF.
