An unintentional fall of roof was discovered during the weekly exam one break inby spad #3163 on the Peters Branch Submains. The fall was approx. 20'x 20'x 4-6'.

An unintentional fall of roof was discovered during the weekly exam on Peters Branch Submains Break #21 at spad #3307. The fall was 140' long, 20' wide and 6' high. The fall extended through the crosscut to #4 and #5 entry.

AN UNINTENTIONAL FALL OF ROOF OCCURRED ON THE #10 BELTLINE BREAK #50. THE FALL WAS APPROX. 30' X 50' X 4' THICK AND CONSISTED OF LAMINATED SANDSTONE. CRIBBED AREA OFF AND MOVED SECTION.

EE ALLEGES HE RECEIVED AN ELECTRICAL SHOCK FROM HANDLING A ROOF BOLT MACHINE CABLE.

EE alleges he received an electrical shock from a roof bolt machine cable. EE did not seek medical attention until 2-23-05. He was admitted to Pineville Hospital for observation only and released on 2-25-05. (No defects were observed in the cable.)
