A roof fall had occurred on 001 section at 10:00 am in the #4 entry at break 5 outby last open break, the size of the fall was approx. 18'W x 20'L x7'H no ventilation effected, no equipment effected, At 4:07 am 9/14/2011 a fall occurred in #4 entry connecting to previous fall, above the last open break, the fall was approx.18'W x 60'L x 6'H.
A roof fall has occurred at 8 mains in the #1 entry at brk. 15, the size of the fall was approx. 20'W x 20'L x 7'-8'H, no equipment effected or ventilation effected.

On 7/28/11 employee carrier deck got on lo lo structure then dropped off striking ground with a sudden jerk. Employee became a lost time on 9/9/11 due to a strained lower back.
On 3/21/2011 employee was pulling on a piece of belt when he strained his left shoulder. Employee became restricted on 5/16/2011 due to this injury.
Employee was in #5 entry installing roof bolts, during this time employee raised head on bolter, not aware of where his right hand was placed employee caught his right 3rd digit finger with the canopy of bolter causing injury to finger, employee has returned to work on light duty due to injury on 12/5/11.
Employee was tramming roof bolter when a piece of draw rock fell striking left little finger, employee has been placed on light duty.
Employee was making a belt setup when a piece of drawrock fell striking head, employee was placed on restricted duty until 8/10/11.
Employee was working on track putting track tie in when chain that was holding up track fell striking left hand, employee received a cut to left wrist and fracture to left thumb. Employee has been placed on light duty.
The employee got off a fork and got on another forklift and backed into the forks of the other machine, the fork struck his left leg. He went to the doctor and had 12 sutures placed in the cut he received. He returned to work the next scheduled shift. Full duty.
The injured employee was loading belt structure in the scoop bucket when he caught his right thumb between the structure and the bucket. He had a simple (hair line fracture at the end of his thumb. Returned to regular duty.
Employee was getting lo lo ready to be scoop, moving miner and bridge cable out of way when drawrock fell striking him on head and right shoulder employee received cut to head. Employee has returned to reg. duty with no restrictions.
Employee is a scoop operator that was loading a bundle of roof bolts that were binded together with a metal strap, the strap broke loose striking employee on right forearm, cutting arm, employee received 6 sutures and returned to work with no restrictions.
Employee was drilling top with drill steel from roof bolter when drill steel kicked out striking employee in chin, employee has received 7 sutures in chin. Employee has returned to work with no restrictions.
Employee was working on miner track driving keeper in track pin when a piece of metal broke off lodging into left hand. Employee had metal removed requiring 1 suture and returned to regular duty.
Supply man was straightening up a load track rails he just loaded onto supply car when he cut his left middle finger requiring sutures, employee has returned to regular duty.
Employee was making a splice on shuttle car cable when utility knife slipped cutting top of left hand, 4 sutures required, employee has returned to regular with no restrictions.
Employee was using a belt knife to cut rope when knife slipped cutting right thumb sutures required. Employee has returned to regular duty with no restriction.
A bolt and plate was laying on the ground. When employee let down head on roof bolter it caught the roof bolt plate causing the roof bolt to fly up striking head just above right eye. Sutures required, employee has returned to regular duty with no restrictions.
Employee was cutting off wear runners under miner, when side lid fell down hitting head, sutures required. Employee has returned to reg. duty with no restrictions.
Employee was walking when he tripped grapping a roof bolt strap to keep from falling. Employee received a cut to his 2nd index finger on right hand, sutures required. Employee has returned to work with no restrictions.
