Accident type, without injuries

While doing a weekly examine a rock fall was found at Shell Branch Bleeder in the #2 entry at brk. 25-26. The size of the fall was approximately 4 to 5' thick 20' wide and 40' long. The ventilation has not been affected, cribs are being installed in the affected area.

While doing weekly examination a rock fall was found on 4 main Break 17 in the #4 entry. The size of the fall was 8' thick 18' wide and 30-35' long. The ventilation is not affected. Fall occurred due to a transition zone in the top. An action plan has been submitted and is still pending.

A roof fall was found at 67 Mains, on 6 track at break 8 between 4 & 5 entry. The fall was approx. 2' to 3' thick 18' wide and 30' long. 10 Heintzman Jacks with pizza pan heads will be set. This fall does not block any escape way or effects the ventilation.

Over-exertion in pulling or pushing objects

Employee was pulling on a pump suction line in a frozen pond when he felt a strain in his stomach. he went to doctor and diagnosed with a very small umbilical hernia but employee did not treat. Employee has returned to his regular job with no lost time or restrictions.

Over-exertion (Not Elsewhere Classified)

Employee was operating loader loading coal, when finish employee went to get off of loader. While climbing down loader ladder foot slip off step hurting back and knees. Employee became a lost time accident due to a slip disk. Became a lost time accident on 12/2/2014

Caught in, under or between a moving and a stationary object

Employee was leaning into mantrip giving out ear plugs unaware of his right foot was half way under mantrip, mantrip moved catching right foot under mantrip and twisting foot. Employee received a fracture to his right ankle. Employee has returned to work with a full release.

Struck by falling object

Employee had placed left hand on scoop bucket after hanging up 6" waterline when, water line fell striking fingers on left hand. Employee received 6 sutures to his 3rd digit finger and hairline fracture to 2nd digit finger on left hand. Employee has returned to work with no restrictions.

Struck by falling object

After cutting a speed reducer loose from 5 head drive the speed reducer slid from head drive falling on employee's left arm. Due to his injury employee received a fracture to his left arm, employee has returned to his regular work duties.
