Accident type, without injuries

A roof fall has occurred at Shell Branch Bleeder in #4 entry at crosscut #14, the size of the fall was 20' x 20' x 8' which involved a 14-9 remote control continuous miner. The area will be cleaned and additional roof support will be installed.

Over-exertion in lifting objects

Employee was in mine yard lifting a 3hp pump to bring into building when he felt something pull in his lower back. Employee became a lost time accident due to this injury.

Over-exertion in lifting objects

Employee was taking out garbage when he picked up 4 lb garbage can straining his lower back, employee has become a lost time due to this injury. Company was not informed of being a lost time accident until recently due because we were unable to reach employee.

Fall to the walkway or working surface

Employee Was at Laurel Fork Bleeders when he found a timber had fallen, while carrying timber to set back he slipped and fell hurting his back.

Over-exertion in lifting objects

On 4/23/13 employee was picking up a rock at the lo lo when he twisted and strained his left arm employee continue to work until 08/01/2013 having a herniated disk he became a lost time accident due to this injury.

Struck against a moving object

On 4/30/2013 employee riding in mantrip strained neck when mantrip struck parked mantrip. Employee became restricted duty on 7/23/2013 due to this injury.

Over-exertion in lifting objects

Employee was tossing a bag of sand on top of a 20 ton locomotive when he felt a sharp pain in his lower back. Employee has been placed on restricted duty.

Fall onto or against objects

Employee was servicing a mantrip in the shop when he slipped off the mantrip falling and striking his left forearm against side of the mantrip. Employee received a fracture radius to his left forearm. Employee has been placed on light duty to his injury.

Fall to the walkway or working surface

Employee was bolting top in 3 heading when he tripped over a rock falling to ground. Employee complained of neck hurting, employee was placed on restricted duty on 8/30/2013, on 9/03/2013 company shut down due to coal market, on 9/16/2013 due to coal market when employee was placed on unpaid furlough.

Employee was loading tray on roof bolter with bolts when he caught his left digit finger on tray while loading. Employee received hairline fracture to his finger, employee has returned to reg. duty with no restrictions.

Struck against a moving object

On 8/14/2013 employee was traveling on rubber tire buggy when he ran over a rock buggy bounce causing head to hit roof top, employee continue to work received a full release 8/27/2013.

Employee was using utility knife to cut holes in belt when he cut his left hand, 3 sutures required, returned to work with no restrictions.

Struck by falling object

Employee was working at head drive when draw rock fell striking left leg. Employee received a fracture to his left fibula.

Employee was using a slate bar to move head motor, slate bar slipped striking employee in mouth breaking 3 top and 2 bottom teeth. Employee has returned to work with no restrictions.

Struck by falling object

Employee was at the end of Lo Lo structure when he placed his left hand on back of carrier dolly when rock came across dolly striking left hand fracturing his left ring finger. Employee has been placed on restricted duty.

Fall from piled material

Employee was standing on a pallet of block building a stopping when he fell off the pallet striking his left knee on the ground. Employee received a fractured to his left patella. Employee has returned to his regular job with no restrictions.
