Accident type, without injuries

A roof fall was found in #3 heading at break 17 in the intersection, ventilation was not affected no equipment or persons involved, the fall size was apporox. 20' wide x 9' thick x 30' long. The area has been cribbed and additional 16' rope bolts have been installed.

A roof fall was found at 11:45 am by MSHA inspector and carried out and reported to safety dept. at 1:45pm. The fall was reported to MSHA hotline at 1:50 pm. The fall was located in the East Main at Break 23-24, in#4 entry, the fall was approx. 18'wide,20'Long,10'thick.The area was timber and dangered off.

A rock fall was found in the East Mains #5 entry brk. 22 intersection off the section, no ventilation, or machinery involved, no miners hurt. The fall was approx. 5' thick, 20'Long, 15' wide. The affected area has been dangered off.

A roof fall was found in the East Mains at brk. 22 in the #1 entry, the fall was approx. 15'W, 20'L, 6' thick and Brk. 23 in #2 entry, the fall was approx. 15'W, 50'L, 6' thick. Ventilation was not affected, no equipment or persons involved. The area has been timbered and dangered off.

A roof fall was found at Marion Branch at approx. 12:15 pm, in the #1 entry at the drift mouth behind a stopping, the fall was approx. 5' thick, 20' wide and 10' long, this area has been dangered off.

A roof fall was found at Marion Branch in the #2 entry 50ft. behind the fan, the fall was approx. 20' wide, 10' long, 3' thick, this area will be cleaned and supported as required in the roof control plan.

A roof fall was found at Right Fork Submains in the #5 entry between brk. 38 & 39, the fall was approx. 7' high, 20' wide, and 60' long, this area is located in a worked out area and in the process of being sealed off. The area has been cribbed and dangered off.

Joy Continious Miner was pillaring in the belt entry, a fall occurred in #2 heading extending thru #4 heading covering the Joy Continous Miner, no ventilation was effected, the size of the fall was approx. 90' long, 20' thick, and 12' high.

A roof fall was found at 1 Seam Mains Brk. 26 between 3&4 entry, the fall was approx.5'x20'x100' the area has been dangered and sealed off. The information was called to the safety director at 2:05 pm.

A roof fall was found in the old works at the Short Block Mains in the #4 entry, the fall was approx. 20'x30'x5' wide, the area has been dangered off.

Struck by rolling or sliding object

Employee was standing at roof bolting machine when scoop backed out of right break into #4 entry. Bucket of scoop caught corner of rib and broke rib loose causing rib to roll onto EE and roof bolting machine. Employee received a fractured collar bone and 6 left ribs were fractured, employee became lost time on 12-10-09.

Employee was loading tire into loader bucket and tire fell on right foot.

EE was moving metal into a rack when he caught his left thumb between the piece of metal and the metal rack crushing the tip of his left thumb.

Employee was operating scoop with left hand outside of deck, left hand got caught between bumper stop and deck, causing injury to his left index finger. Employee became lost time on 4/08/09, left index finger tip removed due to this injury on 4/7/09.

Employee twisted lower back while avoiding a piece of drawrock that fell, employee has been placed on restricted duty.

Fall from ladders

On 7//21/09 employee was plastering a stopping when ladder fell spraining his left ankle, employee had returned to regular duty,it came to my attention recently that the employee had returned to work on 7/22/09 but not to regular duty, light duty. Employee was on restricted duty for 1 day.

Struck by falling object

Employee was bolting top when a piece of draw rock fell hitting right forearm, 5 sutures required, employee was placed on light duty.

Over-exertion in lifting objects

Employee was loading structure into scoop bucket when alleged he hurt his right shoulder, neck, and chest. Employee was placed on light duty 4-21-09.

Caught in, under or between a moving and a stationary object

Employee was placing guard on tailpiece when glove caught between bearing and proxmity switch causing injury to 2nd digit finger on right hand, sutures were required. Talked with employee on safe work habits. Employee was placed on light duty.

Employee was sawing timbers, when he cut his left index finger with saw, sutures required.

Struck by falling object

Employee was helping on roof bolter when a channel strap fell hitting left index finger. Sutures required, employee has returned to regular duty.

Employee was bolting top in when drill steel fell hitting right middle finger fracturing it. Employee returned to reg. duty with no restrictions, just received x-ray report 6-11-09.

Employee was bolting top in #3 heading when a piece of drawrock fell hitting him in the mouth, 5 sutures required. Employee has returned to regular duty with no restriction.

Struck by falling object

Employee was setting a stiff jack at 6 headdrive, when a piece of drawrock fell, hitting left hand ring finger. 5 sutures required, employee has returned to regular duty with no restrictions.

Over-exertion in lifting objects

Employee was trying to remove a lge. rock off belt when rock was set down on left hand, he continue to work, on 2/27/09 due to this injury fractured left little finger, surgery required. Employee returned to reg. duty with no restrictions on 2/28/09

Employee had used a utility knife to cut a plastic tie, when it slipped cutting his left forearm, employee had received 6 sutures. Employee returned back to reg. duty with no restrictions on 1-16-09.
