A roof fall was found at break 35 on the # 9 beltline, it was approximately 40' by 20' 10' thick.

During pillaring a planned fall occurred, the pressure from this fall effected the ventilation by blowing out the brattice line, production was stopped, workers brought out, and necessary steps were taken to restore ventilation. Ventilation has been restored.

A roof fall was found off 8 track at brk. 31 in #5 entry, extends thru #2 entry to the right, the size of the fall is approx. 60' long x 20' wide x 8'to 10' high. No ventilation, or escapeway affected. The area has been cribbed and dangered off.

A roof fall was found in 1st left at break 2 and 3 in the #4 entry at Double Submains, the size of the fall was approx. 20'W x 80'L x 6"thick. area of the fall has been cribbed off.

A roof fall was found in the #4 entry of Plant Mains, at Brk.10 from the outside, the fall was approx. 5'thick, 20'wide, 20'long. Ventilation or escapeway was not effected. The area will be timbered and dangered off.

A rockfall occurred in the #3 entry on 16 beltline at break 10, the size of the fall was approx. 35'L x 40'W x 6'Thick. No ventilation or machinery affected. Area is being cleaned and additional roof support is being added.

A rockfall occurred in the #3 entry at brk.3, this is the #10 beltline, the size of the fall was approx. 25'L x 30'W x 8'-10'Thick. No ventilation or machinery was affected. Area is being cleaned and additional support is being added.

A roof fall was found in 9 mains at brk. 31 in #5 entry, this is located in old works 5 brks. inby section. No ventilation or machinery effected, the area has been crib off on both sides of entry. The fall was approx. 20'W x 40'L x 5'thick.

A rockfall was found during a weekly examination at Holly Bush Submains, the fall was located at brk. 40 in the #2 entry extending thru 2 left entry. No ventilation or machinery has been affected. The area has been crib and dangered off, and additional roof support has been added to #1 entry.

A rockfall was found during a weekly examination at 67 Mains, toward 001 section in the #1 entry, between brk. 11 & 12. The size of the fall was approx. 20'w X 30'L X 3' thick. No ventilation or machinery affected. The area has been dangered off.

A roof fall was found at 4 seam mains at break 10 in the #1 entry, the fall was approx. 7'Thick x 30'long x 16' Wide. No ventilation or machinery effected. The area has been timbered and roped off.

A roof fall was found in 8 main track entry at break 18. The fall was approx. 6'x24'x16' wide. No ventilation was affected, the fall was on top of the roof support arches that were in placed prior to this fall.

A roof fall was found in the Southmains (4 belt) at Brk. 3 between 1 & 2 entry, the size of the fall was approx. 20'L x 20'H x 7'W. No equipment or ventilation effected. The area has been cribbed and dangered off.

A rock fall has been found during a weekly examination at East-West Mains break #58 in the #1 entry, The fall area is adjacent to #25 seal, the size of the fall was approx. 30' x 25' x 5'. Additional roof support of cribs beams and steel arches will be installed in the fall area.

A roof fall was found in Old 9 Mains, in the #4 entry between brk. #23 thru brk.# 26. The fall was approx. 5 'thick, 20' wide, 180' long. The area has been dangered off. No ventilation or machinery effected.

A roof fall was found Gasswell Mains, located in old 9 mains, in 3 left, in the # 3 heading. The size of the fall was approx. 16'L x 20'W x 6' thick. The area has been timbered and dangered off.

A rock fall had occurred on 14 beltline break 39 in Holley Submains, 3 entry, the size of the fall was approx. 6'H x 40'L x 60'W. Additional roof supports of cribs, beams, and 10' rope bolts were added to the effected area, also additional rope bolts were added to surrounding area's inby the fall.

At 13 belt drive at a ( 500 KVA power center) there was an electrical fire in the transformer of the power center located in 67 mains, the power center was de-energized, miner's were evacuated, fire was extinguish, all local agencies were contacted, no injuries, and power center has been removed from service.

Struck by falling object

Injury was cleaning gob in 3 left with continous miner, he started backing out of crosscut when corner of rib rolled off on him receiving a fatal injury to his upper torso. Size of rib striking he was approx. 4'L x 36"W x 16" thick.

Employee was checking to see what was wrong with head on roof bolter. EE asked helper to raise bolter head so that he could look, unaware of having placed left hand at pinching point, resulting in mashing ring finger tip off and cutting middle finger below nail bed, later having to remove middle finger below nail bed.

Struck against a moving object

Employee was passenger in mantrip when mantrip spinned out on track hill causing mantrip to roll backwards, mantrip hit 20 diesel motor that was on track. Employee became lost time due to head injury.

Employee was stepping out of railrunner when he twisted his right ankle. Employee receive a fracture to his right ankle. Due to this injury he became a lost time on 9/14/2010.

Over-exertion in pulling or pushing objects

Employee was in 3 entry belting up, Employee was pulling on a top roller to place on belt rail. Employee says lower middle back was hurting due to pulling on roller. Employee became a lost time on 7/20/2010.

Struck by falling object

Employee was working on stopping in the return when a piece of rock fell off of rib hitting employees left foot, employee became a lost time on 6/1/10 - 6/7/10 with bruised left foot. Employee has been released to reg. duty with no restrictions.

Over-exertion in lifting objects

Lifting wooden concrete forms approximately 25 lbs.

Struck by falling object

Employee was changing out 10" metal pipe 9' long, when pipe fell, landing on left foot behind steel toe on boot. Employee has been placed on light duty due to this injury.

Struck against a moving object

Employee was operating mantrip, when mantrip spun out on hill. Mantrip started rolling backwards, mantrip hit 20 ton diesel motor that was on track. Employee was placed on light duty with bruised left hip.

Fall to the walkway or working surface

Employee was walking to mantrip when stepped on rock slipped and fell on left knee. Employee was placed on light duty.

While operating roof bolter, employee was drilling hole into roof when drill steel broke, cutting left index finger. Employee received 11 sutures and been placed on light duty.

Employee was pulling belt roller over waterline when soap dispenser nipple blowed out causing water pressure to hit right eye. Employee has been placed on restricted duty with inflammation to the right eye.

Caught in, under or between a moving and a stationary object

Employee was breaking loose a rock that was on the continuous miner tail when rock broke loose catching right index finger, sutures. Employee has returned to regular duty.

Employee was using slate bar to dig under headdrive, employee used hammer to hit slate bar when a piece of slate bar broke off striking his lower lip, employee received 2 sutures and returned to regular duty.

Struck against stationary object

Inspecting a weld and as he moved his head he scraped his head on a sharp piece of metal causing a small laceration to top left of head.

Fall onto or against objects

Employee was walking by tailpiece when he tripped and fell hitting chin on tailpiece sutures required. Employee has returned to work with no restrictions.

Employee was installing a coarse cleaner coal screen, when his left foot hung causing him to fall, cutting his leg on sharp edge located in the chute. Employee received 10 staples to left leg.

Struck by flying object

Employee was in locker room trying to open pad lock with 2 hammers, hitting hammer against hammer when a piece of hammer broke off lodging into right wrist. Employee has returned back to work with no restrictions after object being removed.

Employee cut left hand while using utility knife cutting rubber packing, employee received 2 sutures and returned to work with no restrictions.

Employee was using lifting jack on headdrive. Lifting jack bar slipped out while jacking headdrive striking employee upper lip, 4 sutures required. Employee has returned to reg. duty with no restrictions.

Employee is a roof bolt operator, while setting canopy a piece of drawrock fell cutting left index finger, employee received 2 sutures and has returned to regular duty with no restrictions.

Struck by falling object

Employee was traveling on track to the outside when a piece of drawrock fell, hitting mouth, 7 sutures required. Employee has returned to regular duty with no restrictions.

Struck by flying object

Employee was grinding on lo lo structure when something went into his left eye. Employee went to doctor on 5/19/10, object was removed from eye. Employee returned to work with no restrictions, employee was wearing safety glasses, a guard was on the grinder at time of accident.

Employee was cutting on belt when knife slipped cutting left thumb, 4 sutures required.

Employee cut left leg shin while using grinder working on lo lo structure, requiring 4 sutures. Employee has returned to reg. duty with no restrictions.

Employee was working on continous miner when he caught his left thumb in between two pieces of metal. Employee received sutures to left thumb. Employee has returned to regular duty with no restrictions.

Inhalation of radiations, caustics, toxic and noxious substances

On 12/01/10 we were notified by the Department of Labor that EE has pneumoconiosis.
