An unintentional fall of roof was discovered during a weekly exam on Big Block Submains break #5 in #2 entry. The fall was app. 7' x 20' x 20'.

An unintentional fall of roof was discovered during a week y exam on South Mains, break 95 #3 entry. The fall was app. 7' x 20' x 20'.

An unintentional fall of roof was discovered during a weekly check on the 3rd panel of Right Fork submains, break #35 in the #4 entry. The fall was app. 35' x 7' x 20'.

An unintentional fall of roof occurred on 012 section while retreat mining operations were being conducted. The continuous miner and the inby bridge were covered by the fall which was app. 20" x 70" x 7'. The fall occurred in the #3 heading, break #19 at spad number 11131.

employee was riding in 4 wheel buggy and caught his left ankle between the rib and the buggy frame.

employee stepped on a spray valve and material sprayed into his right eye.

Employee caught his left hand in the drill steel while installing roof bolts using gloves. He injured three fingers on his left hand. The glove was caught on a burr on the roof bolt itself and twisted his fingers.

Struck against a moving object

EE was riding in mantrip when a locomotive and the mantrip inadvertantly collided on track at #4 head drive. The key steel fell out of front drive unit on a hill and caused the mantrip to slide back down the hill striking the following locomotive. EE hurt his right knee and ankle.

EE was using a torch to cut metal for a coal turn chute when he twisted coming off the belt and fell.

EMPLOYEE ALLEGES HE STRAINED HIS BACK HELPING PULL BOLT MACHINE CABLE SLACK.

Rock fell, striking on the right shoulder and right elbow.

EE alleges he slipped and fell while crossing under a belt and struck his back on the track or blocks.

EE was struck on top of his head by drawrock which resulted in a neck strain, while emptying the dust box. EE worked until 2-7-06 then started missing time.

Employee was roof bolting and was struck on the left leg by a piece of draw rock.

Employee was hooking the connector tongue to the 20 ton locomotive when it fell on his right foot. He had safety metatarsal boots in use at the time of the accident.
