Accident type, without injuries

A roof fall was found at 6 left, off right fork submains, #3 entry brk.34, the fall is approx. 20' wide, 30'long, 10' wide, the effected area was dangered off.

At approx. 1:08 a roof fall was discovered at the 2nd panel on the left of Rightfork Submains. The fall was 5' high x 20' wide 50' long. Ventilation was not affected; no equipment involved. Top was laminated. Additional roof support of cribs and beams has been placed in affected area.

A roof fall was found in the out by area of South Mains, Brk.93 entry #4, the fall was approx. 7ft high, 30 ft. long, 30ft. wide, the area has been cribbed and dangered off.

A roof fall was found during the weekly examination in the old works of the short balck mains. Located in break 10 in the #3 entry. The fall is approx. 7ft thick by 40ft wide by 60ft long.

A roof fall was found in the South Mains Break # 86 the fall extended from the cross cut in #1 entending into the crosscut in #3 entry. the fall is approx. 6ft. thick by 20ft wide by 140 long.

A piece of rock fell on the miner cable behind the standoff. It was determined that a fall did not cause the miner to be stuck but the cable was grounded. This is not a reportable roof fall.

A roof fall was found at crosscut #55 to #57 on #2 beltline, the fall was approx. 65'x20', between 8' to 10' thick. Additional roof support of cribs and cable bolts were put in affected area, all breaks around the fall were checked, no injuries, beltline structure and possible waterline structure damage.

A roof fall was found during a weekly examination of the mine. The fall was found at 10:00 am. and was called out when the person making the examination got to a mine phone. The fall was located in the south mains break 81, entry #4. The fall is approx. 7 ft thick by 20 ft. wide by 40 ft. long.

A roof fall was found during a weekly examination of the seconday escapeway. The fall was called outside at 8:57 am. It is located in the old dollar branch works located in the #4 entry and break #7. It is approx. 7 ft. thick by 25ft. by 45 ft. Additional support will be added to the area.

A roof fall was found in the outby area of the South Mains, break # 72 entry 5 was approx. 50 ft. long by 20 ft. wide by 7 ft thick and break # 65 #4 was approx. 20 ft. by 20 ft. by 7 ft. thick. Additional support has been added to the area. The information was called out to the Supt. at 11:51 am.

Employee was breaking up a rib roll with a hammer when something hit the top of his right hand on 12-17-08 employee continued to work until 2-18-09 when he became a lost time due to having surgery to remove object on top of right hand. employee returned to work on 2-23-09 with no restrictions.

Struck by falling object

While operating roof bolter, wrench used on drill steel broke, hitting employee left hand. Employee received a fracture to his 4th metarsal, employee became a lost time on 10-09-08.

Fall to the walkway or working surface

Employee was carrying mandoor to place in a brattice, when he slipped and fell, twisting his back. This accident became a lost time on 9-16-08

Over-exertion (Not Elsewhere Classified)

Employee was trying to replace top roller using a bar for leverage. Jerked on bar and felt sharp pain down right shoulder and middle upper back.

Over-exertion in lifting objects

Employee was working at #1 tailpiece making a spill board; employee said when picking up a piece of sheet metal he strained his upper back. This injury occurred on 4-25-08; employee continued to work. On 7-1-08 employee became a lost time due to this injury.

Over-exertion in lifting objects

Employee was loading belt structure when he felt pain his groin area. Employee continued to work, went to doctor and was diagnosed with a hernia on 4-22-08. Employee became a lost time due to strain.

Fall onto or against objects

Employee had slipped on a rock falling into side of feeder hitting stomach, employee had been place on restricted duty on 04-04-08, employee will be a lost time on 04-18-08 for surgey, due to injury employee has a hernia.

Struck by rolling or sliding object

Employee was tramming roof bolter to left break in the belt entry, when a rib rolled off hitting him on the left side of his face. Employee received sutures to left side of his forehead, eye, and nose. Employee was placed on restricted duty until 6-24-08, employee has returned to regular duty with no restriction. Safety talk was held with employee on safe work habits.

employee was bolting top in #4 heading when a piece of drawrock fell off canopy striking left knee, until has been place on light duty.

Caught in, under or between (Not Elsewhere Classified)

employee had placed a pull rope with hook on tailpiece to pull tailpiece over, he got his fingers caught when rope was tightend, he had 2 fingers cut on right hand and was place on light duty 4-30-08, sutures required.

Over-exertion in pulling or pushing objects

Employee was helping move 002 section when he strained his back while pulling cable. Employee has been placed on light duty until 4-21-08

Employee was bolting top with a single head roof bolter in low coal, employee let the head jack down on his left knee, employee was placed on light duty with a bruised left knee.

Fall onto or against objects

EMPLOYEE WAS HANGING BELT WHEN HE SLIPPED AND HIT RIGHT SIDE AND RIGHT LEG ON SCOOP BUCKET, EMPLOYEE WAS PLACED ON LIGHT DUTY.

Employee was putting a bolt & glue in hole when a piece of rock fell from between the roof bolts, striking him in the left side of face.

Employee was tramming roof bolter up, when a piece of drawrock fell from top, hitting employee left shoulder. Employee was placed on light duty until 2-20-08. Employee has returned to his reg. duty.

Over-exertion in lifting objects

Employee hurt back lifting spare tire in supply truck , employee was moving spare to make room for sheet of metal in truck bed, employee was put on restricted duty until 02-11-08

Employee was gettting belt with scoop, belt hung on scoop then broke loose from scoop hitting him in right ear. 8 sutures required, employee has returned to regular duty.

Employee was pulling stuck roof bolter with miner when chain broke hitting employee on left side of forehead. 8 sutures required, employee has returned to regular duty.

Struck by flying object

While cleaning torch tips, employee accidentally bumped oxygen handle, causing oxygen to blow debris into right eye. Employee has returned to reg. duty, with no restrictions.

Employee was taking up belt slack to change tailroller out and belt clamp broke, hitting top of left hand, fracturing 3 bones on top of hand.

Roof bolt operator drill steel got hung as he reached to get wrench drill, steel fell out hitting top of left hand, employee has fractured his 4th metacarpal shaft. Employee has return to regular duty.

Employee was bolting top when something got into his right eye, employee was wearing safety glasses at the time, employee has been treated and returned to regular duty with no restrictions.

Employee was checking a blowed hose on bridge carrier when he told operator to turn pump on to see where leak was in hose, he had finger placed over where leak was causing oil to pump into finger causing right finger to swell. Employee has return to regular duty

Employee had came in 2 hrs. early, using his utility knife to cut a piece of belt, the knife slipped cutting his right upper leg, sutures were required, safety talk was conducted with employee on cutting away from self when using knife, employee has returned to reg. duty.

Struck by falling object

Employee was putting glue in bolt hole that was drilled when a piece of draw rock fell out from top hitting back of his head, 7 sutures were required. Employee has returned to regular duty with no restriction.

Absorption of radiations, caustics, toxic and noxious substances

Employee was 2 breaks outby the face in #5 entry plastering a brattice, while plastering brattice, some plaster went into left eye causing irritation, employee has returned to work 1-21-08 with no restrictions.
